摘要:
A systems model is presented that has the potential to expand understanding of the complex nature of health‐related outcomes The horizontal axis is a systems axis of context, process and outcome and the vertical axis consists of the client, provider and setting Examples from outcome research in Latin America and the Caribbean are provided Each section is explored from the perspective of primary health care research and some of the issues of design, measurement and analysis are raised The systems model is offered as a framework for conceptualizing research on the impact of nursing care on client, provider and setting outcom

摘要:
Action research aims to generate and test action theories and thereby to develop and potentially emancipate practitioners The precise nature of such action theories and what their generation and validation can potentially develop in practitioners are described The possible costs of emancipatory action for nurses and a novel and potentially powerful strategy to minimize such costs are discussed

摘要:
Feminism and nursing have historically travelled separate paths Nursinghas only recently begun to embrace feminist philosophy, and consequently nurse researchers are just beginning to undertake truly feminist research There appears to be little consensus between feminist researchers' interpretations of the concepts of method and methodology Prior to embarking on the development of a feminist research proposal it is imperative to differentiatebetween these two concepts This paper explores the concepts of method and methodology and explains their interrelationship Feminist research methodology, with its emphasis on a non‐hierarchical, reflexive, interactive approach is highlighted and its impact on the researcher's choice of an appropriate method is discussed It is only through addressing these issues that nurseresearchers will be effective in pursuing truly feminist research that can act as a catalyst for the emancipation of women and the nursing professi

摘要:
Quality of life (QOL) of older persons living in institutions is a critical consideration in international economic policies and health care decisions Yet, there is little relevant research to support decisions about culturally acceptable and effective nursing care for this population This qualitative study explored and compared perceptions of older residents and nurses in three long‐stay institutions in south‐west England It addressed issues viewed as important to a good QOL for residents and ways in which nurses promote QOL A convenience sample of 10 residents aged 65 and older (80% female) and nine nurses (n=five registered general nurses and four enrolled nurses) were interviewed using a semi‐structured schedule Interview data were analysed using an analytic induction method of content analysis While theme frequency and examples differed, themes common to both residents and nurses were individuality, professionalism, connectedness, and physical functioning Resident responses tended to be personal, succinct, and sometimes negative Nurses' responses were more positive, detailed, and reflective of general professional responsibilities Implications for nursing education, research, and practice are discussed Improvement in QOL will be addressed differently based on social and cultural settings However, there may be commonalities that are applicable across cu

摘要:
The prevalence of pressure sores has been frequently reported However, such measures fail to identify how many patients develop sores, while longitudinal changes in prevalence rates are difficult to interpret This paper provides the first report of the incidence of pressure sores across the population of a single NHS Trust hospital During 1991, 360 patients developed sores, an incidence of 4 03 patients per 100 admissions Within orthopaedic wards, the incidence was higher, with 10 3 patients per 100 admissions developing sores

摘要:
Pressure sores result in patient morbidity and mortality as well as commanding considerable resources within the British National Health Service To assess the impact of preventive measures, accurate and reliable rates of prevalence and incidence are required This information is likely to be used as a measure of quality of care as well being included in future purchaser/provider contracting A number of prevalence studies have been carried out in the United Kingdom using a variety of populations and different methods The methods employed in these studies need to be considered before any meaningful comparisons can be drawn In particular, there are variations in the reported rates possibly related to the under‐reporting of pressure sore prevalence This paper describes a recent point prevalence study which suggested that this may be the case The need for caution when comparing crude ratesis highlighted and the importance of standardizing the methods used for determining prevalence rates is discuss

摘要:
This paper outlines the results of a national census of practice nursesin England and Wales It not only shows that the numbers of practice nurses have increased dramatically in recent years but also demonstrates how theirrole has evolved Many practice nurses are now involved in health promotion, and home visiting, as well as advice and counselling The findings also indicate that many practice nurses express a need for training Continuing education for practice nurses is important in ensuring practice nurses are fullyqualified and trained for the tasks they undertake In addition, it also enables them to develop their role and work alongside other community health professionals
