A rare case of a folie à deux in a married couple is presented. The inducing partner (IND) suffered from paranoid hallucinatory psychosis and induced similar symptomatology in his wife. Different from most cases reported in the literature, the induced partner (recipient, REC) also experienced auditory hallucinations (commenting and conversing voices). While the IND recovered fully with a combination of psychotherapeutic as well as neuroleptic treatment, the REC was cured by the psychotherapeutic intervention and the recovery of the IND alone. According to the criteria of current diagnostic systems (DSM, ICD), both patients should have been classified as schizophrenic, due to the presence of the described auditory hallucinations. We conclude that the presence of hallucinations is less specific for a certain diagnosis than implied by such categorial systems and should be regarded as nosologically non-specific symptoms. Modern neurocomputational models of psychopathology, dimensional approaches in the description of hallucinations, as the findings of new functional brain imaging studies support this view. We propose a multidimensional diagnostic process of hallucinations and to classify cases such as the one presented as ‘induced hallucinatory psychosis’ or ‘folie à deux hallucinatoire’.

This study of 47 patients with low- and high-grade gliomas retrospectively focuses on the patients’ history with regard to affective and cognitive disturbances as well as to the presence of epileptic manifestations. Early emotional changes with a sudden onset were common in both patient groups, especially panic-like anxiety. It is suggested that patients with a high-grade glioma may present with affective disturbances with a sudden onset, while such disturbances in patients with low-grade gliomas could be of epilepti-form origin. It is also suggested that the biochemical bases of anxiety and epilepsy in glioma patients could be analyzed to understand underlying biopsychological mechanisms of malignant brain tumors.

We investigated the incidence and clinical characteristics associated with the use of aliases within a population attending a psychiatric unit over a 12-year period. The use of aliases was uncommonly recognised (0.7%). Alias users tended to be young. Many were non-nationals (43%). Identification usually occurred early in the admission and was often by self-admission (37%). A broad range of underlying diagnoses were noted with schizophrenia (31 %) and factitious disorder (22%) most common. A previous psychiatric (85%) or forensic history (30%) was frequently noted. Recurrence was noted in one third of cases and was significantly associated with a diagnosis of factitious disorder.

The SCID-II Personality Questionnaire, modified for DSM-IV and ICD-10 Diagnostic Criteria for Research (ICD-10-DCR), was administered to 58 consecutive patients with agoraphobia with panic disorder in order to screen for personality disorders (PDs) and assess diagnostic agreement between DSM-IV and ICD-10-DCR. The diagnostic agreement, as expressed by kappa values, was 0.78 for the presence of any personality disorder (PD), but it ranged from 0.51 for schizoid PD to 0.83 for dependent PD. There was a tendency for ICD-10-DCR to overdiagnose PDs, except for borderline and dependent PDs. The sources of disagreement can be traced to differences in the conceptualization of some PDs and differences in diagnostic criteria and diagnostic thresholds; these are further examined in an effort to improve diagnostic criteria and attain greater compatibility between the two diagnostic systems.

The German version of the 20-item Toronto Alexithymia Scale (TAS-20) was studied in 277 medical students. The factor analysis yielded a two-factor solution, quite in agreement with the results of a recent analysis of the French version of the same scale. The first factor corresponds to the difficulties to identify and to describe feelings, where as the second factor corresponds to the externally oriented thinking. TAS-20 proved to be a reliable scale to measure alexithymia; the usage of the total scale score is recommended.
