Summary:The surgical treatment of peripheral nerve entrapment is an area of interest to nearly all practicing neurosurgeons. What follows is a review of the more common upper- and lower-extremity entrapment neuropathies encountered. Discussion of each entrapment neuropathy begins with a review of the pertinent surgical anatomy, followed by a discussion of the clinical presentation and diagnosis. Treatment, beginning with the most effective nonsurgical methods is discussed, followed by the recommended surgical approach. Where appropriate, the expected outcome of surgical intervention is quantified. This review is designed to serve as a guide to the reader in the treatment of patients in his daily neurosurgical practice.

Summary:Extrapial, carotid-cavernous, direct arteriovenous shunts or direct carotid-cavernous fistulas, as they are commonly referred to, are usually due to head trauma. Patients usually present with ophthalmologic symptoms but can suffer a host of neuroradiological problems. Modern minimally invasive techniques have been developed for the successul treatment of most of these patients.

Summary:A brief history is given of the development of therapy for Trigeminal Neuralgia. The currently acceptable methods of treatment are described. They include medicine, Percutaneous Microcompression (balloon) (PMC), Percutaneous Glycerol Injection, Percutaneous Radio Frequency Heat (RF), Posterior Fossa Micro Vascular Decompression (MVD) and Posterior Fossa Partial Root Section (PRS). Details include operative technique, complications and results. There is a brief comparative evaluation. It is recognized that no method is perfect. It is pointed that an evaluation of the risk/gain factor will vary with the patient and the procedure. For certain indications and for certain patients' comfort in risk/gain acceptance, one method may be perferable to another.

Summary:In the intradural proximal internal carotid artery aneurysms, various concepts and analyses have been proposed. In this paper, concept of the carotid cave aneurysms including anatomical consideration and surgical technique is described. When the carotid cave aneurysm is found after subarachnoid hemorrhage or incidentally, surgical treatment is usually advised. Regarding surgical management, the pterional approach is used with unroofing the optic nerve and removing the anterior clinoid process; the carotid artery mobilized and the clipping is performed with a ring clip with laterally curved blades.
