Intersex often presents at birth with ambiguous genitalia but today can be diagnosed prenatally. The condition is the result of aberrant sexual differentiation – a complex process that involves the interplay of many genes recently identified by molecular biologists. The treatment philosophies and techniques of genital reconstruction have been in constant evolution as intersex patients have matured and provided meaningful follow-up. With proper management, the majority of individuals with intersex can develop normal self-image and sexual function.

This review summarizes the experience of intracytoplasmic sperm injection for the alleviation of severe male-factor infertility since its first success in 1991. Couples treated by intracytoplasmic sperm injection should be carefully counselled about the novelty of the procedure. Intracytoplasmic sperm injection with ejaculated spermatozoa can be applied in couples who failed to fertilize after conventional in-vitro fertilization and in couples whose semen parameters were too impaired for in-vitro fertilization. In patients with obstructive azoospermia intracytoplasmic sperm injection can be carried out with fresh and frozen-thawed epididymal spermatozoa or with epididymal spermatozoa. Some couples with non-obstructive azoospermia can also be helped by intracytoplasmic sperm injection if a sufficient number of vital spermatozoa can be isolated from testicular biopsy. Factors affecting the success of intracytoplasmic sperm injection include some technical aspects such as the immobilization of the sperm tail, the quality of the oocytes and the availability of motile (vital) spermatozoa. Because of the novelty of the procedure, a careful follow-up of the pregnancies and the children is strongly advocated.

Advances in reproductive medicine require the periodic revision of therapeutic methods. The emergence of assisted reproductive techniques has resulted in a new turn in the therapeutic aims of andrology. Therefore, in contrast with previous attempts to achieve a higher sperm count and better sperm motility, individual spermatozoa and their de-novo utilization have gained significance. Accordingly, reassessment of the role of artificial and alloplastic spermatocele, offering an alternative solution for the management of obliterative azoospermia, is topical.

After important papers in 1993 and 1994, there was little of interest in this field published in 1995. Vasectomy has been shown to be a clinically satisfactory procedure in the longer term, especially if associated with good preoperative counselling. It has also been shown that spermatozoa may persist in the semen for many years without resulting in fertility.

As a result nitric oxide is now established as the major peripheral relaxant neurotransmitter in human corpus cavernosum. Current research aims to characterize central and local mechanisms that modulate activity of the nitric oxide pathway in health and disease.
