Neurogenic bladder dysfunction is caused by a variety of neuropathological conditions. Its symptoms may mimic nonneurogenic urinary disorders, thus specific urodynamic diagnosis is indispensable. Therapy is aimed at protecting the upper urinary tract and improving the patient's quality of life. Neurourology has enhanced understanding and treatment of this dysfunction. Neurogenic dysfunction of the male genital system is still difficult to treat.

The current guidelines on the diagnosis and therapy of benign prostatic hyperplasia have resulted in a number of questions about the increasing variety of alternative treatment modalities. There are disputes about the different ideas of pressure-flow analysis, and about the general necessity for quantification of bladder outlet obstruction for treatment decisions. Over the past year, literature has confirmed again that obstruction can only be quantified with pressure-flow studies and this review aims to describe the advantages and disadvantages of such a procedure and to demonstrate the possible consequences.

Vesico-ureteric reflux is one of the most common anomalies of the renal tract that may present from birth through to adolescence with important consequences in adult life. An important clinical association between cystic dysplastic kidneys and contralateral vesico-ureteric reflux has been reported recently and the concept of renal scarring following early undetected vesico-ureteric reflux has been called into question. Although surgical correction may not reduce subsequent renal damage it does avoid long-term antibiotic prophylaxis. The Subureteric TeflonK(Mentor Medical Systems, Norwell, Massachusetts, USA) injection technique offers a highly successful, minimally invasive method to correct vesico-ureteric reflux. Concerns about particle migration appear to have been exaggerated experimentally and as yet are clinically unfounded. Inflammatory mediators are being studied as possible predictors of renal scarring and may help to decide when it is safe to discontinue antibiotic prophylaxis. With regard to antenatally diagnosed vesico-ureteric reflux, it remains uncertain whether a conservative approach is always justified or whether a group of infants would be better protected against renal damage by surgery.

There is increasing support for the view that nitric oxide is of physiological importance for lower urinary tract function, which should mean that the L-arginine/nitric oxide/cyclic guanosine monophosphate system in this region is an interesting drug target. New information on the muscarinic receptor subtypes mediating contraction of the detrusor has emerged, and has been the basis for the development of 'bladder selective' antimuscarinic compounds. There is also new information on the possible use of potassium-channel openers in the management of detrusor overactivity.
