Introduction of newer diagnostic and treatment modalities to the treatment of testicular disorders in childhood has resulted in considerable re-evaluation and refinements in management in these areas. Study of childhood testicular disorders is important because early treatment may have significant effects on subsequent fertility.

More than 200 techniques have been described for the surgical correction of hypospadias at any age. Nevertheless, there is general agreement among those involved in this particular kind of surgery that almost all procedures should be planned one-stage repairs, carried out at the earliest possible age in order to prevent problems in the development of gender identity. The techniques that we have used most frequently are described below.

Congenital urologic anomalies are common. Urologists are challenged to maintain or improve physiologic function while restoring normal cosmesis in patients with congenital anomalies. In addition, they must be knowledgeable enough in genetics to counsel families when presented with a newborn with birth defects.

Urinary tract infections (UTIs) affect up to 5% of children and adolescents in modern Western society. They occur because of either a deficiency in host defense mechanisms or because of bacterial virulence and adherence factors. Vesicoureteral reflux (VUR) is the single greatest risk factor to the kidneys in the development of recurrent UTIs. The general consensus is that evaluation should include voiding cystourethrography and renal and bladder ultrasound with perhaps the concomitant use of urodynamic characterization of the bladder if clinically warranted. Diagnosis should be made from clinical impression and urinalysis and therapy begun at once. UTIs should be treated with at least 3 days of an effective antibacterial agent but long-term antibacterial suppression is required in those patients who have anatomic or functional abnormalities.

The treatment of childhood renal tumors has advanced dramatically over the past few decades. During this time, significant improvements in imaging techniques have enhanced our diagnostic capabilities. In all children, a search for metastatic disease is indicated before treatment plans can be defined. Preoperative imaging can also provide useful staging information and aid the surgeon in planning the operative approach in a child with a large abdominal mass.

We review the use of change in estimated glomerular filtration rate (CFR)/time as a means for following the course of renal disease and recommend using the subcutaneous steady-state plasma clearance of iothalamate (Conray 60) as the preferred method. The advantages of this method are accuracy, simplicity in its demands on staff and patients, it is readily repeatable, practical, and relatively inexpensive.

With the introduction of cyclosporine a decade ago, the results of transplantation improved markedly. More recently, the use of multiple immunosuppressive drugs in sequential therapy has resulted in even better graft survival rates but has been associated with an increased incidence of lymphomas. Recent studies have shown improved growth in children treated with recombinant growth hormone after transplantation, and excellent patient and graft survival rates in infants who received transplants. In reports of pediatric transplants performed in the 1960s, 65% of the patients were alive 20–27 years later and most had good rehabilitation.
