Urinary bladder function depends on the integrity of both the reservoir and the sphincteric mechanism. Intact neural pathways are essential: the coordinated interaction between the somatic and autonomic nervous systems regulates this function. Urodynamic and neurophysiologic studies are essential both to evaluate the integrity of the system and to select and guide proper management.

Normal micturition can be thought of in two phases, storage and filling. Recent advances in the understanding of the complex neurologic and physiologic basis of voiding have focused on the role of calcium and neuromodulators affecting contractility. This fertile area of research will potentially lead to agents that increase facilitatory pathways and decrease inhibitory pathways and have an impact on the management of patients with voiding dysfunction.

The controversial discussion of urethral fluid dynamic in terms of resistance and energy loss has turned into a more comprehensive view of the mechanical balance of detrusor-outlet interaction. Biophysically and pathophysiologically meaningful and realistic conceptualization of voiding function now allows detailed analysis resulting in specific information about bladder outlet and detrusor function. These new urodynamic tools are hardly applied either in clinical urology or in research in obstructive uropathy, which still relies on the poorly specific uroflowmetry and symptoms.

Urodynamic measurement and investigation systems have experienced major developments and standardization over the past 20 years. The clinical value of urodynamic investigation for the diagnosis of urinary incontinence and voiding disorders and for the follow up of conservative or surgical therapies is now widely accepted. However, a number of theoretical problems have appeared, concerning the urodynamic measurements per se and their sensitivity and specificity, especially in the description of subvesical obstructions.

This review assesses innovations in female urinary incontinence under the topics of epidemiology, aetiology, clinical and urodynamic investigations and treatment. Advances are rarely sudden or exciting. Perhaps the most thoughtful contribution has been in geriatric medicine: the assessment of the elderly incontinent has been reviewed and we have been reminded of what are feasible and sensible investigations. A similarly cautious approach towards the use of symptoms alone as a screening test for urodynamic diagnosis in general practice has also been described. Otherwise, the published literature mainly confirms outcome and success rates for several modes of conservative treatment, while still questioning the mechanism of successful therapy.

Pharmacological treatment of urinary incontinence is based either on improvement of bladder storage function by inhibition of detrusor activity, or on stimulation of the urethral closure mechanism, or on reduction of the diuresis. Conversely, bladder emptying function can be improved by stimulation of the detrusor contractility and/or reduction of urethral outflow resistance. This review will mainly focus on drugs whose efficacy on lower urinary tract dysfunction has been proved by placebo-controlled clinical trials.

The impact of surgery on the management of voiding dysfunction as well as its potential to cause voiding dysfunction is discussed. As spinal regulation of voiding may be affected by long-standing toileting habits, the recognition and restoration of efficient voiding technique is important in the treatment as well as avoidance of voiding problems with surgery.

Surgery of female stress urinary incontinence aims primarily at the elevation of the urethrovesical junction to a high retropubic position. However, simultaneous correction of any coexisting genital prolapse is equally important not only for relief of the patient's symptoms but also to make normal unobstructed voiding possible. That is the reason why classic anterior repair still plays an important role for the repair of cystoceles although it is no longer recommended as a primary anti-incontinence procedure. Whether transabdominal paravaginal defect repair will be able to replace transvaginal techniques of cystocele correction remains open. Alternatives to bladder neck surgery include pelvic floor training, injectables, and artificial urinary sphincters (AUS). Recent developments are acknowledged.
