The contrast agents used in urologic imaging have different effects on different organ systems, including variable toxicity. The newer non-ionic agents have fewer anaphylactoid and nephrotoxic effects, mostly by virtue of their low osmolality. However, the rarity of serious reactions and the high cost of the new agents call for their judicious use in selected high-risk patients only.

Magnetic resonance imaging has demonstrated an increasing utility in the evaluation of urologic disorders. Although magnetic resonance imaging has been shown to be superior to ultrasonography and computed tomography in the evaluation of a number of urologic problems, it remains a problem-solving modality. The biggest obstacle to increasing the use of magnetic resonance imaging remains its limited availability and relatively high cost. In the evaluation of renal tumors and bladder cancer, magnetic resonance imaging is recommended instead of computed tomography in patients with contraindications to iodine contrast media, or after computed tomography when pertinent clinical questions have not been fully answered. The role of radiology in staging prostate cancer is controversial but, if imaging is recommended, magnetic resonance imaging is the single best modality for locoregional and nodal assessment of the cancer. Magnetic resonance imaging has unique applications for evaluating urethral carcinoma, urethral trauma, and Peyronie's disease. Complimentary use of magnetic resonance imaging with ultrasonography appears to be the best approach to identifying nonpalpable undescended testis and scrotal pathology.

Maternal ultrasonography has led to a predictable increase in the incidence of neonatal hydronephrosis consistent with a pelviureteric junction obstruction. Early enthusiasm for pyeloplasty has largely been replaced by a more conservative approach. There is, however, no universally accepted gold standard for prospectively differentiating between apparent obstructions that would benefit from surgery and those that will remain stable or even resolve spontaneously.

Conventional high-resolution grey-scale ultrasonography is established as the imaging modality of choice in the evaluation of scrotal disease. Several studies have shown that colour Doppler ultrasonography can be effective in the evaluation of acute scrotal disorders and establish absence of intratesticular arterial blood flow in testicular torsion. Testicular microlithiasis, previously thought to be a benign asymptomatic condition, has been shown to have a high association with testicular malignancy.

Percutaneous transvenous embolization of varicoceles has become an accepted method of treatment in many centres. Regardless of the method of embolization used, results are almost as good as those of open surgery; the complication rate and time off work for patients also compares favourably. The advent of laparoscopic surgery has led to renewed interest in the management of varicoceles, and critical evaluation of all treatment modalities will be required to determine the best available therapy.
