Universal use of low-osmolality contrast agents for intravascular diagnostic procedures has been limited to a large extent by the high cost of these agents, which is generally 10 to 20 times that of high-osmolality contrast media. The question for the radiologist, therefore, is the definition of selected populations and situations for the use of low-osmolality agents to achieve the greatest patient benefit at the least cost. The authors examine controversial issues including the incidence of significant contrast reactions, the definition and incidence of nephrotoxicity, and the effects of contrast media on coagulability.

The development of sophisticated non-invasive imaging techniques has enabled radiologists to detect and diagnose renal masses better than ever in the past. Relative roles of different imaging procedures continue to evolve; sonography assumes a more important role for initial detection, computed tomography (CT) remains the mainstay for diagnosis, the role of arteriography continues to decline, and magnetic resonance imaging (MRI) gains a potential new role with the use of MRI contrast. The continuing controversies are a result of the difficulty in the specific diagnosis of small renal tumors.

The incidental adrenal mass is encountered with greater frequency than in the past because of the increasing use of cross-sectional imaging in clinical practice. The evaluation and treatment of an adrenal mass depends on clinical history, endocrinological assessment, and whether the mass is detected incidentally or after a directed search for adrenal disease. Although definitive characterization requires sampling of tissue, magnetic resonance imaging (MRI) can be used to evaluate adrenal masses in patients who cannot undergo safe biopsy or in cases when a tissue diagnosis is less critical. The preoperative detection and characterization of pheochromocytoma or paraganglioma is satisfactorily accomplished with MRI as well. Most masses associated with adrenocortical hyperfunction can be detected with computed tomography (CT).

This paper discusses the role of color Doppler ultrasonography (CDU) as it applies to the genitourinary tract. CDU is beneficial in evaluating both the native and allograft kidney in the diagnosis of renal arterial and venous stenosis, arteriovenous fistulas, pseudoaneurysms, arterial and venous thrombosis, and in the characterization and staging of renal masses. CDU has redefined the role of ultrasonography in the evaluation of the scrotum as it relates to testicular torsion, inflammatory disorders and varicoceles. Early results of CDU in prostate carcinoma and benign prostate hypertrophy have shown limited success. Identifying and assessing the neurovascular bundle may have an impact on the staging of prostate carcinoma. It is hoped that the future addition of ultrasound contrast agents will further establish the role of color Doppler in the diagnosis of a variety of genitourinary diseases and disorders.

Within the past 10 years, the study of erectile function has witnessed a rapid expansion of knowledge. New anatomic and physiologic theories, diagnostic procedures, and treatments have all been developed. As with all new medical concepts, there is active and healthy controversy. The papers reviewed demonstrate an early consensus of opinion for some ideas and a difference of opinion for others. Published ideas and conclusions have been strongly influenced by the incomplete evolution of the diagnostic methods, by' patient psychologic inhibition interfering with objective testing, and by particular perspectives of the investigator biasing the final interpretation. This update of the radiologic assessment and therapy of erectile dysfunction presents recently published works and background related to their development.

The specific cellular events leading to acute renal failure from acute ischemia, trauma, or drugs are being defined. The roles of proteinuria, decreased renal mass and hyperfiltration, alterations in dietary lipid, protein and phosphate, and blood pressure control are the major focus of research on the progression of chronic renal disease.

The major recent advance in the diagnosis of renovascular disease is the development of less invasive tests demonstrating both the anatomical lesion and the evidence of renal ischemia. The respective indications of surgical revascularization and angioplasty are relatively well classified. Technological progress could extend the indication of endovascular therapies.
