The mental pain hypothesis used to develop trazodone is illustrated, together with its medical and methodological implications. Trazodone possesses a well documented antidepressant activity, which indirectly confirms the above hypothesis. It lacks the aminergic effects of other antidepressants and has a marked adrenolytic activity. Therefore it is potentially indicated in depressive states with adrenergic hyperactivation, such as during alcohol withdrawal. The methodological aspects of the approach taken for trazodone are also discussed, in the context of the strategies of psychopharmacological research.

Blood cells may be used as a model in the central transport mechanisms of amino acids, precursors of amines, implicated in some hypotheses of psychiatric diseases. In a previous paper, we showed a deficit of tyrosine transport by red cells incubated in the plasma of depressed patients. In the present study, we have investigated the interaction of these peripheral transport mechanisms for tyrosine and tryptophan with other amino acid transports, such as the L and ASC systems, and the role of sodium ions in the extracellular medium. We also describe the inhibition induced by incubation at a low temperature, and a probable role of the membrane viscosity. The interest of incubating the cells in their own plasma, in order to have physiological conditions, is also discussed.

This study reports the efficacy and safety of trazodone as an antidepressant drug in a national telematic multicenter outpatient open clinical trial. In less than 3 months, 1,700 patients completed a 1-month treatment and among them, 65 % were considered as responders. The side effect profile of the drug included sedation, gastrointestinal discomfort and moderate hypotension. One case of trazodone-induced priapism is presented and briefly discussed.

The medical histories of 600 psychotic and 369 nonpsychotic subjects were examined for the occurrence of rheumatic chorea. There was significantly more rheumatic chorea in the histories of psychotic patients than in nonpsychotics (p < 0.01, chi-square). Neuropathological associations are discussed.

Ageing is certainly one of the most important current problems; in fact, according to statistics, the old population of the world is continuously increasing. Depression represents the most frequent psychiatric problem in the aged, if it is considered that almost 25% of the old population are affected by it. An early diagnosis and a proper treatment are crucial problems. Because of the numerous side effects of tricyclic antidepressants, today we are able to use some third-generation antidepressants which show unquestionable advantages in the therapy of depression in the elderly. Trazodone is one of these antidepressive drugs and represents a real leader among the antidepressants. Our care is to work so that these recent and efficient drugs may be correctly and properly utilized by physicians prepared to face this often totally new task.

Eight patients with chronic obsessive-compulsive neurosis were subjected to trazodone, an antidepressant acting on the serotonergic system. After 4 weeks of treatment, 6 showed significant reduction in the obsessional symptoms which was sustained after a 6-week follow-up. The 2 patients who did not show improvement had a significantly higher trait score than the others on psychometric ratings.

There are speculations that beta-adrenergic blocking agents are useful in the treatment of psychiatric disorders. Clinically, propranolol, an agent of this group, has been investigated in the treatment of various clinical disorders including schizophrenia, other psychoses, anxiety disorders, and stress reactions. This paper critically evaluates its efficacy in these disorders and provides a useful clinical perspective. In addition, propranolol produces a number of psychiatric side effects which are due to its central action or due to the peripheral receptor blockade. In addition, idiosyncratic reactions as well as withdrawal effects have also been reported to produce psychiatric side effects. It is important that the physician who wishes to use this drug know the existing knowledge of its usefulness in the treatment of psychiatric disorders as well as its psychiatric side effects.

Chronic pain is refractory to most currently marketed regimens. Part of the problem in achieving satisfactory results in treatment is understanding the mechanism that causes chronic pain. One hypothesis currently being investigated explores the role of serotonin. As such, products that block the reuptake of serotonin, such as trazodone and amitriptyline, are being studied. The rationale for the current hypothesis, including psychopharmacology and the role of serotonin reuptake blockers, will be reviewed.

Weight data taken from a 6-week randomized double-blind study comparing amitriptyline, trazodone and placebo were analyzed to determine differential weight changes between the two drugs. In 272 depressed outpatients doses were increased over a period of 4 days to reach a maximum level of 200 mg amitriptyline or 400 mg trazodone. After grouping the subjects according to initial weights (ideal, overweight, 20% above, and underweight, 20% below) mean changes were determined for each treatment. The results indicate that amitriptyline, which differed from placebo and trazodone, produced significantly higher weight gains in the ideal and overweight groups. Trazodone on the other hand produced a slight weight loss in the overweight group. Due to a low number of patients in the underweight group, the results were not significant statistically. The antidepressant effects of both trazodone and amitriptyline were the same and no correlations between Hamilton scores and weight change were found.
