In order to establish whether the serotonergic disorder affecting the parkinsonian brain also modifies hypothalamic-pituitary function in Parkinson’s disease, 10 patients (aged 57–66 years) and 10 normal controls (aged 55–69 years) were tested with the serotonergic agonist d,l-fenfluramine (60 mg p.o.), with CRH (100 µg i.v.) and with placebos. Plasma ACTH/cortisol levels during tests were evaluated and compared. Both groups showed similar levels of ACTH and cortisol in basal conditions and after placebo administration. A slight physiological decline in both ACTH and cortisol levels during the placebo test was observed in normal controls and parkinsonian patients. CRH induced similar ACTH/cortisol increments in all subjects. In contrast, d,l-fenfluramine significantly increased plasma ACTH/cortisol concentrations in the normal controls, but not in the parkinsonian patients. In order to gain a better insight in the serotonergic disorder affecting the parkinsonian brain, the growth hormone (GH) response to the 5-HT1 serotonergic receptor agonist sumatriptan was tested. Sumatriptan was injected subcutaneously in 10 de novo parkinsonian patients (aged 58-69 years) and in 9 age-matched normal controls. On different occasions, subjects were also tested with GH-releasing hormone (GH-RH; 1 µg/kg body weight in an intravenous bolus) and L-arginine (30 g in 50 ml normal saline over 30 min), which releases GH from somatostatin inhibition, to determine whether GH secretion in response to alternate secretagogues is preserved in Parkinson’s disease. In addition, a control test with the administration of normal saline instead of drug treatments was performed. Plasma GH levels were recorded over 2 h in all tests. Placebo administration did not change plasma GH levels in any subject. Similar GH responses were observed in normal controls and parkinsonian patients when GH-RH or arginine were administered. A significant GH increase was observed in normal controls after sumatriptan injection; in contrast, GH secretion was not modified by sumatriptan administration in parkinsonian patients. These data show that Parkinson’s disease is associated with an impairment in the 5-HT1-receptor-mediated serotonergic transmission in the control of GH secretion, suggesting that this specific defect might alter other serotonergic-mediated mechanisms in the parkinsonian b

Changes in sleep state and night-time body temperature were studied in 13 healthy young men. Temperature data showed serial dependence that was removed when considering three main segments of the thermogram: an initial rapid drop between 60 and 140 min after lights off, followed by a slower decrease to the thermal minimum and a final temperature increase until waking. The first segment was fitted by a two-term function, while the other two segments were fitted by linear regression. Temperature data obtained after fitting these models were stationary and used to calculate the fast Fourier transformation and the cross-correlation functions between the hypnogram and the thermogram of each subject. Attaining stationarity has unmasked short oscillations during sleep and some temperature series showed the prescence of ultradian oscillations with a period of 90–100 min. Significant cross-correlations between sleep stages and body temperature were found, and the parameters of the two-term function fitted to the first temperature drop were related to the sign of the correlation and to that time series (sleep or temperature) which preceded the other during the nigh

摘要:
We report 3 patients who experienced nonconvulsive status epilepticus following generalized convulsions during antidepressant treatment. All showed prolonged confusion with some psychiatric symptoms, and their electroencephalograms (EEGs) showed either bilateral synchronous multiple spike wave discharges or continuous diffuse sharp and slow waves. When diazepam was administered intravenously to 1 patient, the clinical symptoms and EEG improved markedly.

Demographic, psychiatric, and electro-encephalographic (EEG) data were compared with brain autopsy findings for 73 elderly psychiatric patients. A late onset of depression (60+ years) was more likely than early onset to be accompanied by Alzheimer’s disease. There were strong associations between clinical and histopathological indicators of Alzheimer’s disease. This was particularly evident for diffuse EEG slowing. Clinical as well as histopathological indicators of Alzheimer’s disease were intensely related to EEG features, mainly EEG slowness (positive), reactivity to eye opening (negative) and dominant frequency (negative). Cerebrovascular pathology showed fewer EEG correlations. The results support the usefulness of the clinical electro-encephalogram in patients suspected to have Alzheimer’s
