AbstractBackground.The prevalence of psoriasiform dermatitis in HIV‐infected patients is similar to that in the general population, but its clinical severity and the immunosuppression in these patients pose special problems of therapeutic management. Furthermore, a distinctive clinical pattern has been reported in some cases. In order to assess these features in our clinical setting, we have done a retrospective study on the clinical records of all human immunodeficiency virus (HIV)‐positive patients with psoriasiform dermatitis requiring systemic treatment.Methods.The clinical records were reviewed of seven HIV‐positive patients who were referred between 1988 and 1994 to a University Hospital Dermatology Department from an HIV‐clinic because of psoriasiform dermatitis, resistant to topical treatment.Results.The clinical appearance was rather uniform, with the following common features: facial seborrhea, flexural and acral involvement, with pustulosis of the palms and soles, and frequent arthritis. Lesions appeared in nonterminal stages of acquired immunodeficiency syndrome (AIDS). Three patients developed cutaneous lesions after the diagnosis of HIV infection was made and showed the most severe clinical involvement and arthritis. Etretinate, followed by RePUVA, proved to be the most effective systemic therapy prescribed, with only rare adverse effects. Methotrexate was shown to be effective, but its use was accompanied by hematologic toxicity. Cyclosporine A treatment was moderately effective and was not associated with progression of AIDS.Conclusions.A characteristic Reiter‐like clinical picture was observed in AIDS‐related psoriasiform dermatitis. Etretinate and RePUVA were effective and safe in controlling the lesions. AbstractBackground.Various dermatologic conditions have been reported to occur with increased frequency in human immunodeficiency virus (HIV)‐positive individuals, but there are only a few studies comparing the prevalences of skin diseases in HIV‐positive patients with those in matched HIV‐negative controls.Methods.Skin lesions in 129 HIV‐positive patients and 258 HIV‐negative controls were studied prospectively over an 18‐month period from October 1991 to March 1993.Results.Oral Candida, tinea cruris, and ichthyosis were significantly more common in HIV‐positive patients compared to controls. AbstractBackground.No reports are available on allergic contact dermatitis in Saudi Arabia, although it seems to be a common skin problem. We attempted to explore certain clinical aspects in addition to the profile of sensitizing allergens in our area. As no standard panel for patch testing is available in our geographic region, we examined the suitability of the European Standard Series.Methods.Patch testing was performed on 271 consecutive patients with various forms of dermatitis, referred for evaluation of possible allergic contact dermatitis. The study included 147 women and 124 men. The patients were between 12 and 75 years of age.Results.Out of 271 subjects, 152 (56.1%) showed one or more positive reactions. Of these, 80 (52.6%) were women and 72 (47.4%) were men. Almost one quarter of the patients (25.7%) presented with hand dermatitis.Positive reactions to 21 out of the 22 allergens were found. Sensitization was most common to nickel sulfate (39.5%), potassium dichromate (32.9%), and cobalt chloride (30.9%). Reactions to the other allergens ranged between 14.5% and 1.3%. Less than one percent of the patients (0.66%) reacted to benzocaine and showed no reaction to primin.Conclusions.Allergic contact dermatitis is a common skin problem in Saudi Arabia. Further studies that address the prevalence and incidence of the disease are indicated. The European Standard Series is suitable for patch testing patients in our community; however, we suggest exclusion of benzocaine and primin. The addition of three allergens of local relevance, black seed oil, local perfume mix, and henna, are presented and discussed. The formulation of a regional standard series for patch testing dermatitis patients in our geographic area requires further collaborative studies.
