Caring and expertise are fundamental to nursing. Technology, although often viewed as inimical to these human qualities in the provision of care, has the potential to support caring and enhance expertise. Nurses whose expertise embraces both technology and nursing can design and implement technologies that will help them fulfill their commitments to patients. Appropriate, well-designed, flexible technologies can improve the care of individuals and enlarge the store of nursing knowledge and expertise.

Information systems are going to change our understanding of nursing as a discipline and as a practice. The act of decision making will be the focus of the impact of information systems on nursing. The article models types of nursing knowledge, organizing them into concrete and abstract knowledge. Information systems will allow the inclusion of concrete science date, which as not been possible previously. Role performance is seen as a mechanism for the integration of the two sciences, and a new understanding for nursing decision making is proposed.

The capacity for comprehensive access to and integration of information and knowledge suggests a holistic approach that transcends time, settings, and providers. Moreover, the interface between nursing practice and nursing informatics provides an opportunity to create new science and to expand the boundaries of nursing knowledge.

摘要:
Given an appropriate conceptual model, such as the Wilson and Cleary model for linking clinical variables with health-related quality of life, ambulatory care information systems have the potential to facilitate holistic care for persons infected with the human immunodeficiency virus. Four examples of the manner in which ambulatory care system can support the provision of holistic care are the contextualization of care through inclusion of the patient's voice, improved coordination of care and knowledge-harvesting through systems integration, timely application of agency-specific information and domain information and knowledge to individual patient, and blurring of the boundaries of ambulatory care.

The article discusses how the Nursing Intervention Lexicon and Taxonomy (NILT) is being used to describe patterns of care for patients receiving home care nursing. These patterns of care include intensity (frequency of interventions), focus of care (NILT category or categories with the highest percentage of interventions), and comprehensiveness of care (number of NILT intervention categories reflected). Thus with a means for quantifying care in holistic ways, nurses can now ask important questions about clinical care and its outcomes using empirical nursing data about care (ie, categorized nursing intervention statements). Understanding how these data are expressed using nurses' natural language terms, and being able to categorize these interventions reliably, represent essential preliminary work for describing and defining data for inclusion in the automated record.

If older patients are to use the information superhighway, their ability and interest in using computers for clinical information exchange must be determined. Earlier discharge may alter the type of information that patients and families need to cope with recovery. The Gassert Model for Defining Information System Requirements for Nursing has been adapted to identify needed discharge information. Information requirements will be used to develop mobile computing technology for patients to use during recovery from cardiac surgery.

Diverse data and information are becoming increasingly important to clinicians, administrators, and educators as they work in rapidly changing settings. A number of classification systems or vocabularies have been developed that offer structure for client records and clinical information systems. Such structure is needed to generate reliable, valid, and useful data. The research-based Omaha System is one such classification system. It consists of nursing diagnoses/client problems, interventions, and client outcomes. The Omaha System is being used in many settings, including home care, nursing centers, colleges of nursing, and school health programs.
