Denying a pregnancy can result in consequences as tragic as neonaticide—the killing of an infant in the first 24 hours of life. Between 150 and 300 neonaticides are committed each year but the actual prevalence of pregnancy denial is unknown. Adolescents are especially vulnerable to pregnancy denial because the unexpected event creates such enormous anxiety and fear for them. Pregnancy denial often eludes parents, teachers, and health care providers. Nurses can advocate for resources and pregnancy screening in primary care settings, and help educate parents and teachers about the problem.

Home visiting may be a promising strategy to improve pregnancy outcomes, and home visiting by lay workers may be more accepted by pregnant women. Lay workers may impact on social and environmental risk factors as well as on health care utilization. As with any primary prevention strategy, these programs may be more successful if implemented with responsibility shared between the health care system and the community. This article reviews the state of the science related to lay home visiting during pregnancy in the United States. Using a variety of search terms, an exhaustive review of the literature was conducted using several large electronic databases. Studies of lay home visiting during pregnancy have documented mixed results. Many weaknesses exist in the studies available, including use of descriptive or quasi-experimental designs in most of the studies, absence of a clearly specified set of interventions, and lack of cost analysis. Gaps in our knowledge of the impact of lay home visitors on pregnancy outcomes persist.

摘要:
Rheumatoid arthritis (RA) is an autoimmune disease characterized by inflammation and joint involvement. Most adults with RA experience sleep disturbances, including longer times before falling asleep, numerous awakenings during the night, and early morning wakening, resulting in excessive daytime sleepiness and fatigue. This article will review what is known about sleep disturbances and the biologic basis in adults with RA, the influence of ovarian hormone levels in women with RA, how medications may influence sleep in RA, and complementary and alternative therapies that may be useful in reducing sleep disturbances.

摘要:
A dramatic increase in the number of people who are overweight or obese in this country presents a growing public health problem. Successful weight loss requires diet, physical activity, and behavior modification interventions. Weight loss diets with distinctively different macronutrient profiles have resulted in short-term weight loss. There is great interest in identifying the most effective strategies to achieve long-term weight loss. Individualizing weight loss interventions, including diet, will be important to facilitate significant weight loss on a population basis. Nurses and nurse practitioners can play an integral role in supporting their patient's weight loss efforts.

摘要:
The Native Women Enjoying the Benefit (WEB) program trains nurses to perform breast and cervical cancer screening examinations for Native American women. Several evaluations have shown that the program improves nurses' knowledge and skills, and nurses value its positive impact on clinic, nurse, and patient behaviors. Beyond effectiveness, program longevity often rests upon equilibrium that results from alignment between a program's values and principles with those of its sponsoring organization and pertinent surrounding environments. We examined how Native WEB values align with those of its 2 most relevant environments—the medical institution (immediate environment) that sponsors it and the broader health care context (distal environment). We found that social justice views articulated in Catholic social teaching served as a convenient synthesis of the 2 environments' values and principles. We used this conception of social justice to determine whether the Native WEB program reflected the perspectives of its immediate and distal environments.
