Risk factors for cement eczema were studied among workers employed in the Danish construction industry. 2 cohorts exposed to cement containing different concentrations of water‐soluble chromate were examined. A statistically significant decrease in the number of workers with allergic cement eczema was found in the cohort exposed to cement with the lower water‐soluble chromate concentration. Allergic cement eczema was found to have in greater extent of involvement than irritant cement eczema, if 3rd cohort was followed to evaluate individual risk factors. The degree of exposure to wet cement seems to have a certain, though not statistically significant, effect on the risk of developing irritant cement eczema. 摘要:
Euxyl K 400 is a preservative for cosmetics and toiletries containing 2 active ingredients, 1,2‐dibronio‐2,4‐dtcyynobutitn and 2‐phenoxyethanol. 2057 consecutive patients with contact dermatitis were patch tested with Euxyl K 400 2.5% pet. and ethanol. A positive patch test occurred in 24 patients (1.2%). The source of sensitization was traced in N patients to their cosmetics. Both leave‐on and rinse‐off products were responsible, Further patch tests with Euxyl K 400 0.5% pet. and phenoxyethanol 5% pet. were performed in 11 patients. A positive patch test to phenoxyethanol was detected in 1 of them. Only 3 patients showed to mild reaction to Euxyl K. 400 0.5% A provocative use test with a lotion preserved with Euxyl K 400 0.1% was positive in 5 of these 11 patients. Patients with Euxyl K 400 sensitivity showed a high prevalence of positive patch tests to other common ingredients of cosmetics. We report an outbreak of occupational allergic contact dermatitis from l,6‐diisocyanatohexane (old name hexamethylene diisocyanate or HDI), in the anti‐pill finish Evafanol AS‐1, in 2 clothing factories. The 19 Operatives Iron: 2 dressmaking mills all complained of work‐related dermatitis. 5 of the subjects had positive patch tests to the fabric with which they were working, and when all 19 were patch tested to Evafanol AS‐1 (1% aq). II were positive. 6 of these II were patch tested to MDI (l% pet.) and all 6 were positive. Patients who noticed worsening of their skin disease after using topical corticosteroid preparations were patch tested both with the commercial preparation and the corticosteroids itself. Between 1987 and 1989, 10 cases of contact dermatitis due to topical corticosteroids were detected in this way. The corticosteroids wee amcinonide (2 patients), hydrocortisone butyrate, clobetasol propionate (2), betamethasone valerate (2). prednicarbate and fluocortolone (2). Patch tests with the commercial preparations and the corticosteroids themselves elicited reactions almost identical in time course and severity Individual sensitivity seems to be more important for Lest results than test conditions. 9 of the 10 patients underwent further patch testing with a corticosteroid series corticosteroid series. In 2 patients, a true cross‐reaction between budesonide and hydrocortisone butyrate was found. All 9 patients showed further sensitivities to other corticosteroids. Most of the cross or concomitant reactions could be categorized into recently defined corticosteroid classes. In an attempt to define the optimal test concentration for isothiazolinones. 200 and 100 ppm of 5‐chloro‐2‐methyl‐4 isothiazolin‐3‐one 2‐methyl‐4‐isothiazolin 3‐one (MCI/MI) in aq. Solution were tested simultaneously in 918 consecutive eczema patients. 3.4% were positive to 200 ppm and 1.7% to 100 ppm. Indicating that up to 50% of the cases might have been missed. Among the 33 cases with positive reactions, 21 showed stronger reactions to 200 than to 100 ppm, while 9 had reactions of the same strength. The influence of basal transepidermal water loss (TEWL), skin thickness, blood flow and skin colour on susceptibility to sodium‐lauryl‐sulphate (SLS)‐induced irritant contact dermatitis was studied in 70 healthy volunteers. SLS 0.5% was applied as a patch test. The expression of surface‐bound immunoglobulin E by dendritic cells within cutaneous tissue has been compared in atopic and con Lad dermatitis, 45 patients were recruited into 4 groups using clinical criteria and patch testing to a standard series of allergens; atopic (12 cases), allergic contact dermatitis (14 cases), irritant contact dermatitis (10 cases) and the control group (9 cases); using clinical criteria and patch testing to a standard series of allergens. Skin biopsies from each patient were analysed by the indirect immunofluorescence technique. This differentiated 3 patterns of cutaneous IgE distribution: (i) no derectabel cutaneous IgE; (ii) detection of IgE solely within the dermis; (iii) detection of IgE within both epidermis and dermis. Detection of IgE within the epidermis was always associated with the presence of IgE within the dermis. In each ease, IgE was surface‐bound by dendritic cells. Immunoglobulin E was detected within both epidermis and dermis in skin blopsies from 8 (66.7%) atopic patients and 2 (20%) patients with irritant contact dermatitis. No other cases demonstrated IgE deposition within both the epidermis and dermis. Atopic patients were significantly more likely to have detectable IgE deposition. Within both epidermis and dermis, than patients with contact dermatitis (allergic and irritant groups combined.p=0.0011) or controls (p=0.0049). This finding suggests that the demonstration of IgE within both epidermis and dermis supports a diagnosis of a topic dermatitis.
