Reactivity to repeated daily sodium lauryl sulfate (SLS) applications and patch test reactivity to SLS was studied in 23 females. Skin changes were quantified by transepidermal water loss (TEWL), dielectric water content (DEWC), laser Doppler velocimetry (LDV) and visual scaring DS). Dermatologic histories (HS) and susceptibility to sunburn (ST) were obtained and clinical skin dryness evaluated (DS). Great interindividual variation occurred in the degrees of changes in (he biophysical parameters measured; the variation was most apparent in TEWL. The subjects with HS I or more developed greatest TEWL increase after open SLS applications (p<0.05). DS showed poor correlation with SLS reactivity and only minor DEWC alterations were seen. 摘要:
A new, standardized, ready‐to‐apply patch test, the TRUE Test™, has been evaluated on 698 consecutive patients with suspected contact dermatitis. The patients were tested with 12 different allergens. Simultaneously, the same 12 allergens in pet. (Trolab®) were applied symmetrically to the opposite side of the upper back using the conventional Finn Chamber® technique. There were positive test reactions to all 12 allergens lusted in the patient group. The concordance of positive reactions between the TRUE Test™ and the Finn Chamber® test was 67%; 13% of all positive reactions were recorded only for the TRUE Test™ and 20% only for the Finn Chamber® method. Prevention of NiSO4induced allergic, contact dermatitis (ACD) using ZnSO4 indrinking water was studied in a guinca pig model Without ZnSO4intervention, nickel (Ni)‐exposure resulted in significantly higher (p<0.05) stimulation indices (SIs) as compared to non‐exposed controls, using NiSO4as an allergen in the lymphocyte transformation test (LTT). Oral intake of ZnSO4at both 250 μg/ml double‐distilled deionized water (DDD) and 500 μg/ml DDD resulted in lower SIs than those of control guinea pigs drinking only DDD: the 250 μg ZnSO4/ml group had significantly lower Sis than (p= 0.025) than controls. There was no significant correlation between intradermal test responses and the SI values of individual guinea pigs exposed to NiSo4Mean Zinc (Zn) concentrations in skin and in whole blood were not statistically different between the NiSO4exposed control and Zn supplemented groups, nor between Ni‐sensitive and non‐sensitive animals within groups. We investigated alterations of epidermal lectin binding sues, as well as of pemphigus and bullous pemphigoid antigen in 28 human patch test reactions, both allergic (nickel, formaldehyde, N.N′‐1.3‐dimethylbutyl‐N′‐phenylenediamine) and irritant (sodium lauryl sulfate). The epidermal reactivity to a panel of lectins and human antisera to pemphigus vulgaris and bullous pemphigoid antigens was compared with samples obtained from normal skin and from skin under tape occlusion. We observed selective perturbations of lectin and antibody binding in acute contact dermatitis, whether allergic or irritant. The main findings were a loss of terminal sialic acids and longer bi‐ and triantennary mannosyl residues as well as a loss of pemphigus vulgaris antigen. The only difference between allergic and irritant patch test reactions was in topography of loss of WGA binding sites: in the former, it was most pronounced in the lower and middle epidermis, whereas in the latter it was seen in the uppermost subeorneal layers. 1310 patients were routinely patch tested with a paratertiary‐butylphenol‐formaldehyde resin (PTBP‐F‐R), a resol resin based on phenol and formaldehyde (P‐F‐R‐2), and a mixture of these 2 resins. Approximately 2.5 times more patients with contact allergy to phenol‐formaldehyde resins were diagnosed when routinely patch tested with P‐F‐R‐2 in addition to PTBP‐F‐R. Although patch testing with a mixture of both resins was not as good as patch testing with the 2 resins; separately, it was better than testing only with PTBP‐F‐R, since 1.6 times more patients with contact allergy to phenol‐formaldehyde resins were still diagnosed. 542 patients (330 women, 212 men) with suspected allergic contact dermatitis were patch tested to standard series allergens between January 1983 and June 1987. Positive reactions were most frequently seen with nickel (l7.4%), ethyelnediamine (8.7%). formaldehyde (7.4%), colophony (7.0%), potassium dichromate (6.1%) and neomycin (5.7%). Patients with dermatitis involving the legs were significantly more likely to he allergic to significantly more likely to be allergic to ethylenediamine (p=0.01) and benzocaine (p= 0.04) than those with dermatitis not involving the legs. Neomycin allergy was not associated with dermatitis involving the legs. Patients allergic to ethylenediamine; were significantly more likely to be allergic to neomycin than patients not allergic to ethylenediamine (p=0.002).

In a human maximization test of 5% falcarinol isolated isolated fromHedera helix, 10 of 20 subjects were sensitized. No subjects gave irritant reactions to 5%, 10 became sensitive to 1%, and 7 to 0.05%, with 3 of these giving 3+ to 4+ bullous reactions.

The effect of cyclosporin A (CSA) on the elicitation phase of allergic contact dermatitis was investigated. Initially, the ability of CSA to penetrate normal human cadaver skin was determined using 3 different vehicles. 10% CSA in Labrafil was found to be the must effective combination at achieving skin penetration (0.33%). Patients with known contact sensitivities to a variety of allergens were selected. After a 48‐h pretreatment with 10% CSA in Labrafil, skin sites were exposed to the appropriate contact allergen. 72 h later, the development of an eczemutous patch test reaction was evaluated. A cleaning woman in a restaurant became sensitized to thiourea in a silver polish. The dermatitis persisted for several months and occasionally flared up after exposure to sunlight, even alter she no longer used the product Patch testing and photopatch testing showed that she had both contact and photocontact allergy.
