摘要:
Coronary artery disease (CAD) is the leading cause of death in black Americans, accounting for 25% of all deaths. Risk factors for CAD that have been established in white populations also have a high prevalence in black populations. Some risk factors, such as hypertension, have a greater prevalence in blacks. Risk factor reduction for this population remains as great a problem as it is for whites and other minority groups. The major challenges for nursing lie in implementing and evaluating primary and secondary prevention strategies beginning with children and extending to the elderly.

摘要:
Although Hispanic Americans have shared in the nationwide reduction in coronary artery disease over the past 25 years, mortality data indicate that coronary artery disease remains the leading cause of death in this population. This article reviews the incidence of mortality due to myocardial ischemia and the prevalence of cardiovascular risk factors including cigarette smoking, hypertension, serum lipids, diabetes mellitus, and obesity in Hispanic Americans. Management strategies necessary to reduce cardiovascular risk factors in Hispanic Americans as well as sociocultural issues relevant to nursing research and practice are discussed.

Elderly patients with coronary artery disease (CAD) present special opportunities and challenges for nurses. The astute nurse is aware of changes in the aging cardiovascular system and possible alterations in the presenting signs and symptoms of CAD. Risk factor reduction may need to be approached more selectively in the elderly. Older patients who are treated surgically may experience more complications. Implications for nursing care are reviewed.

Cardiovascular disease is the number one cause of death in American women over age 40. Following menopause, the risk for coronary artery disease (CAD) in women is the same as for men. In addition, the major factors that place men at risk are the same for women. However, differences exist in the presentation of symptoms, the ease of diagnosis of CAD, and the morbidity and mortality rates following myocardial infarction (MI), coronary artery bypass surgery, and angioplasty. Because of role differences in men and women, more attention must be paid to developing successful health-promoting and life-style-change strategies in women of all ages.

special populations have been underrepresented in cardiovascular research for many reasons. However, strategies are available to obtain greater unmbers of underrepresented populations in research. These strategies may be complex, time consuming, and costly, but they are necessary to ensure that eligible minorities participate in research to identify the major cardiovascular risk factors and appropriate interventions.

Myocardial infarction in pregnancy or the puerperium is an uncommon occurrence. Thus, when young pregnant women sustain a myocardial infarction, they present a complex challenge to cardiovascular nurses. Issues such as female gender, young age, and child-bonding must be considered in addition to assisting the recovery from the myocardial infarction itself. This case study describes the rehabilitative nursing care of a young woman who sustained a postpartum myocardial infarction.

This article reviews the literature relevant to the postmyocardial infarction (MI) patient and sexual activities. It begins with an analysis of the published accounts of the physiologic effects of sexual activity (including intercourse) and the anticipated concerns associated with coital deaths. The article includes an examination of the need for sexual counseling in any treatment and rehabilitation setting. In addition, general sexual adjustment guidelines for the uncomplicated post-MI patient are presented.
