Researchers are far from understanding the mechanism(s) of action of hormone replacement therapy (HRT) on the cardiovascular system. Moreover, the results of clinical trials that can clarify these dilemmas will not be available for several years. As clients seek guidance in making the pivotal decision about taking HRT, it is important that practitioners understand what the current research reveals about its risks and benefits. This article addresses the role of HRT in primary and secondary prevention and includes observational research, clinical trials (completed and in progress), benefits and risks, and implications for clinical practice in counseling patients.

This study described women's prodromal and acute symptoms associated with myocardial infarction (MI) based on interviews with 76 women who had experienced an MI in the previous year. Sixty-eight women experienced prodromal symptoms including unusual fatigue (70%), shortness of breath (53%), and pain in the shoulder blade/upper back (47%). All women experienced acute symptoms including chest pain/discomfort (90%), unusual fatigue (59%), shortness of breath (59%), and shoulder blade/upper back discomfort (42%). Although women in this study reported numerous prodromal symptoms, none had received a new diagnosis of coronary heart disease (CHD) prior to MI. Practitioners must develop an awareness of and a more comprehensive approach to treating women at risk for CHD. Further research to elucidate prodromal and acute symptom clusters is needed to assist practitioners in early diagnosis of CHD in women.

The clinician evaluating a woman with symptoms potentially indicative of coronary heart disease faces the challenge of choosing the appropriate diagnostic test. The use of noninvasive testing in women has been controversial due to a perception of diminished accuracy, limited female representation, and technical limitations that compromise efficacy. Recent meta-analyses and large observational series report marked improvements in accuracy for women undergoing exercise treadmill, echocardiography, and nuclear testing. Electron beam computed tomography is a relatively new technique, and the body of evidence is still developing. An adequate body of evidence supports the use of noninvasive testing for intermediate risk, symptomatic women and may result in improved diagnostic and therapeutic decision making.

Instruments used to measure emotional and functional outcomes of coronary heart disease show a fairly high level of consistency. Scores on depression scales are typically higher in women than men, yet this finding must be balanced with data showing that mean scores for depression are low. Both sexes report moderate levels of anxiety after a cardiac event; however, global mental health is rated as relatively high. Women report lower levels of physical activity and higher levels of disruption in functional activities than men. Women also report resuming household activities early in their recovery, a finding most likely reflective of traditional role responsibilities. Last, for both sexes, emotional distress and functional disability decrease over time and appear to be stable by 6 months after the event.

This study explored the role of the self in the experiences of women living with cardiovascular disease. The study, which used a naturalistic design, involved interviewing 13 women with a diagnosis of coronary heart disease (CHD) and analyzing their reports through constant comparative analysis. For women, living with CHD was characterized by changing images of the self. Women reported engaging in processes involving seeking meaning, creating mastery, and accepting the self. These processes were marked by the overarching theme of connectedness with significant others. The study findings enhance understanding of the role of the self in recovery for women with diagnosed CHD.

Heart disease is the leading cause of death in women. The recent Centers for Disease Control and Prevention (CDC) landmark document,Women and Heart Disease: An Atlas of Racial and Ethnic Disparities in Mortality, identifies the relationship between geography, race, ethnicity, gender, and the determinants of mortality. Overwhelming evidence supports that African American women, especially those residing in the South, experience higher heart disease death rates than women of other ethnic, racial, and geographic origins. This publication presents a unique opportunity for health care providers to use derived county-specific data in education, research, and provision of health promotion and disease prevention services to all women.

This article describes the use of multiple strategies to recruit women, particularly African American women, into a home-based, moderate-intensity walking intervention and compares African American to Caucasian midlife women on cardiovascular risk characteristics at entry into the program. One hundred seventy-three women aged 45 to 65 years were recruited using a variety of strategies. Baseline findings showed that, on average, the women in the program had modifiable cardiovascular risk factors with proportionately more African American women than Caucasian women having hypertension and low physical fitness. The findings also illustrate the importance of using multiple recruitment strategies to encourage midlife African American and Caucasian women to participate in intervention trials.

A syncopal episode in an elderly patient with a history of carotid disease, multiple cardiac risk factors, and new onset chest pain presents diagnostic challenges. This case study describes the experience of a woman with coronary vasospasm accompanied by conduction deficits. The patient's history, diagnostic work-up, and treatment program are presented. The relationship between the coronary artery anatomic defects and the conduction abnormalities are discussed.
