A monoclonal antibody (HMB‐45) was previously reported to bind to melanoma cells, and the junctional component of nevus cells, but not to normal adult melanocytes. We have tested HMB‐45 binding in several conditions under which melanocyte stimulation might be expected in adults, i.e.3 simple lentigines, 2 solar lentigines, 7 recent surgical scars from re‐excision of non‐melanocytic tumors), 2 surgical scars from re‐excisions of melanomas (after complete primary excisions), 9hemangiomas from non‐sun‐exposed skin, I basal cell carcinoma, I acute ecchymosis, I keloid, and I dermatofibroma. Positive controls included 6 malignant melanomas and I fetal skin sample. Melanocytes were strongly positively stained overlying hemangiomas, within or near recent surgical scars of melanocytic and non melanocytic tumor re‐excisons, near basel cell carcinoma, and in fetal skin. Melanocytes either were not stained or were stained only focally for trace amounts in the normal skin near the new margins of the wide re‐excision specimens for melanoma, i.e., at a distance from the scar, in the simple lentigines and in the fibrotic lesions. Thus HMB‐45 is staining an antigen which appears in adult melanocytes during stimulation and in fetal skin, as well as in melanomas. This stimulation is associated with condition that would have increased vascularity, suggesting a melanocyte response to a plasma factor, or other endothelial cell derived factor. HMB‐45 would not be a useful marker for residual melanoma cells in melanoma

Thirty plantar warts were analyzed for the presence of HPV‐1 type‐specific and PV genus‐specific capsid antigens by immunofluorescence (IF) using monoclonal and polyclonal antibodies and type‐specific HPV‐1 DNA employingin situhybridization methods. Fifteen of 30 plantar warts were positive by IF for PV genus‐specific structural viral antigens. Thirteen of the 15 productively infected plantar warts expressed intranuclear HPV‐1 type‐specific capsid antigens and viral DNA, which were detected in the same distribution in each individual wart. The 2 productively infected plantar warts that did not react with HPV‐1 type‐specific MoAbs did not react with HPV‐1 type‐specific DNA by in situ hybridization. The distribution of OKM 5‐positive dendritic cells in the epidermis was investigated in 75 cases of inflammatory dermatoses and in 14 cases of normal human skin by immunohistochemical and morphometric methods. Furthermore 6 cases of normal human skin, 14 cases of nevocellular nevi, 26 cases of malignant melanoma, 7 cases of contact dermatitis and 6 cases of mycosis fungoides have been examined with special emphasis on the expression of OKM 5 antigen on keratinocytes. OKM 5‐positive dendritic cells were present in normal human epidermis at a density of 46 ± 3.4 cells/mm2section area. However, there was a significant increase in cutaneous drug eruptions (166±17.2 cells/mm2; U‐test: p 0.05). Concerning OKM 5‐positive keratinocytes, a mean percentage of 10.2%± 5.0% OKM 5‐positive keratinocytes was found in nevocellular nevi, compared to 0.5%± 0.5% in the adjacent skin. The corresponding values for malignant melanomas were 52.5%± 3.4% (lesional epidermis) and 7.1%± 2.2% (adjacent epidermis). There were significant differences of both lesional and adjacent epidermis between nevi and melanomas (U‐test: p 0.05). Our cases of contact dermatitis revealed a mean percentage of 19.3%± 6.7% OKM 5‐positive keratinocytes, whereas in mycosis fungoides the corresponding value represents 42.7%± 6.2%. The differences between the percentage of OKM 5‐positive keratinocytes in normal epidermis and contact dermatitis as well as mycosis fungoides were significant (U‐test). Several monoclonal antibodies (MAB) have been produced using an eccrine carcinoma cell line as an immunogen. One such MAB, SKH1, reacted with both the secretory portion and coiled duct of the eccrine and with the secretory portion of apocrine gland. SKH1, however, did not react with myoepithelial cells, intradermal ducts of both types of sweat gland, or with other components of normal axillary skin including the epidermis and follicular apparatus. The reaction was strongest if the specimen was fixed with 80% methanol, and moderate on non‐fixed or acid‐alcohol‐fixed specimens. Only weak reaction was obtained on cold acetone‐fixed specimens, and reaction was negative with formalin‐fixed, paraffin‐embedded tissues. SKH1 reacted positively with the cytoskeleton of the eccrine carcinoma cell line, Colo‐16 and MCF‐7. Applied to pathological skin specimens, SKH1 reacted with the tumor cells of clear cell hidradenoma, syringocystadenoma papilliferum, and extramammary Paget's disease. SKH1 also reacted with the tumor cells of meta‐static adenocarcinomas arising from lung, breast and ovary. SKH1 did not react with the majority of tumor cells of eccrine poroma, but reacted with single–layered cells lining narrow ductal lumina. SKH1 did not react with epithelial cells lining cystic or ductal lumina of syringoma, but reacted moderately with the amorphous keratin–like substance filling the lumina. Rudimentary meningocele: a variant of “primary cutaneous meningioma” |

We studied 5 primary cutaneous meningiomas. All were congenital. Four were nodules or plaques on the scalp, and one was a lumbar polyp. Two were alopecic. A skull defect was present deep to one lesion, and the lumbar polyp was attached to dura. The tumors were concentrated in the subcutis, where strands of meningocytes were embedded in dense collageous tissue. Meningocytes wrapped around colleigcnous fibers, producing “collagen bodies”. These formed the nidus for calcification that included psammoma bodies. Meningocytes also dissected between collagenous fibers, creating anastomosing spaces that mimicked a vascular tumor. Meningothelial‐lined clefts, several milimeters in length, were present in 4 cases. Two lesions extended through dermal defects into the superficial dermis, where adnexa were reduced or absent. The meningocytes contained vimentin and epithelial membrane antigen. They lacked cytokeratin, S100 protein, and endothelial markers. The meningothelial lesions described herein lack the nodular and sheet‐like growth patterns that typify meningiomas of the central nervous system and most primary ectopic meningiomas, including some that develop within the skin. This report describes 6 women with acantholytic lesions localized to the vulvocrural area. Clinically, multiple papules or solitary papules or plaques were situated on the labia majora or adjacent skin. None of the patients had similar abnormalities elsewhere on the body or family histories of similar eruptions. Two patients had concurrent candidal vaginitis. Microscopic findings consisted of acantholytic dyskeratosis resembling that observed in Darier's disease or acantholysis with minimal dyskeratosis similar to that seen in Hailey‐Hailey disease. In one patient, acantholytic dyskeratosis with a cup‐like configuration resembled the microscopic appearance of warty dyskeratoma. Immunofluorescence microscopy was negative in one patient. Solitary lesions were cured by local excision, but the papular eruption of one patient was recalcitrant to therapy. The literature contains descriptions of eight additional women with comparable lesions. The ultrastructure was studied of 2 cases of proliferative fasciitis (PF) which affected the fascia and subcutaneous fat. Two basic cell types were observed in the lesions: fibroblast‐like, and giant cells. The fibroblast‐like cells had myofibroblastic ultrastructural characteristics. The giant cells showed a well‐developed, rough endoplasmic reticulum, irregularly distributed filaments and clear lipid droplets in their cytoplasms. Ultrastructurally, the giant cells seemed to belong to a cellular line capable of synthesizing ground substances and extracellular fibers, but with a modified phenotype. A consistent feature was the presence of nodular aggregates of numerous proliferating perivascular cells. Transitional forms between perivascular, fibroblast‐like, and giant cells were observed. Two unusual fibroxanthomas were studied by light microscopy. The first case contained numerous osteoclast‐like cells and resembled malignant giant cell tumour of soft tissues, a variant of malignant fibrous histiocytoma. Multiple subcutaneous plaques and nodules appeared on the back and the dorsal proximal area of the extremities of a 9‐day‐old male infant after a complicated prenatal period necessitating cesarean section. The clinical and histological features were diagnostic of subcutaneous fat necrosis of the newborn. Light microscopy revealed adipocyte necrosis, a lymphohistiocytic infiltrate, and needle‐shaped clefts within adipocytes and macro‐phages. Ultrastructurally, there were aggregations of electron‐lucent spaces in the form of spindles and needles arranged in parallel within the altered adipocytes; macrophages surrounded these cells or their fragments and invaded the fat lobules. A new follicular cyst was reported. The lower part of the cyst wall was composed of both basophilic and shadow cells as seen in pilomatricoma, whereas the upper part of the wall consisted of clear cells. Our case apparently derives from hair matrix and outer root sheath.
