Sixty‐three patients with pyoderma gangrenosum were seen and studied at the Mayo Clinic from 1971 to 1980. Biopsies from the erythcmatous border or necrotic edge of the pyoderma gangrenosum lesions usually demonstrated a characteristic pathogenic morphologic evolution. The early lesions revealed mild to moderate perivascular lymphocytic infiltrate associated with endothelial swelling. The fully developed lesions demonstrated necrosis in addition to a dense lymphocytic infiltration surrounding as well as involving the blood vessels. Extravasation of erythro‐cytes and thrombosis sometimes were seen. Ulceration, infarction, and abscess formation were found in the later stages of evolution. Direct immunofluorescence results were positive in the blood vessels of 36 of 65 (55%) specimens. IgM, C3, and fibrin were found in the papillary and retieular dermal vessels. IgG and IgA were only occasionally present. Pyoderma gangrenosum appears to be a reactive process that is manifested as a vasculilis. Biopsy material from the advancing active erylhc‐matous border has early characteristic derniatopathologic findings of lymphocytic vasculitis. Thirteen cases of fibroma of tendon sheath have been reviewed. This lesion presents predominantly in middle‐aged adults, particularly men, and usually arises in the dermis and subcutis of the extremities, most especially the hands. Histologically these tumours are always well circumscribed and may be tabulated. They are largely composed of interlacing bundles of hyalinised, hypocellular fibrous tissue with occasional more cellular areas. A constant feature is the presence of slit‐like vascular channels. The stroma in dermatofibrosarcoma protuberans (DFSP) stains with trichrome, but is not polarizable (birefi ingeiit. doubly retractile). The usefulness of polarizing neo‐plastic stroma as a diagnostic aid in distinguishing DFSP from dermatofibroma and fibromatosis was evaluated. Forty cases of dermatofibroma of all types, 12 cases of dermal fibromatosis, and 15 cases of DFSP were examined. None of the DFSPs contained polarizable collagen, whereas 11 of the 12 (92%) fibromatoses and 35 of the 40 (88%) dermatofibromas did. Of the 5 dermatofibromas that did not polarize, all were composed primarily of round to polyhedral histiocytic cells, including foam cells, frequently surrounding small obliterated capillaries. In selected instances, primary cutaneous neuroendocrine carcinoma (PCNEC) and small‐cell malignant lymphoma (SGML) of the skin may display similar clinical presentations and microscopic appearances, leading to diagnostic uncertainty. We applied monoclonal antibodies to epithelial membrane antigen (EMA), la antigen, and leukocyte common antigen (LCA) to 31 cases of PCNEC and J2 of small‐cell lymphoma cutis, using a combined PAP and ABC procedure, to determine whether or not such stains were capable of separating the two neoplasms. All cases og SCML were reactive for LCA, while this determinants was not seen in any example of PCNEC. Anti‐la antigen labelled 11 of 12 cases of SGML, and also failed to stain neuroendocrine carcinomas. Lastly, EMA was observed in 25 of 31 cases of PCNEC, but it was found in one lymphoplasmacytoid lymphoma as well. We studied 190 patients with an (gM hand at the dermo‐epidermal junction demonstrated by direct immunofluorescence. In 55% of patients the final diagnosis was lupus erythematosus, 49% discoid lupus erythematosus, and 6% systemic lupus erythematosus. In a selected group of 8 patients with progressive alopecia areata (AA) leading to AA universalis, immunological aspects (in the peripheral blood and the tissue) were studied during the period of the intitial attack of the disease. The peripheral T‐cell helper/suppressor ratio appeared not to be a reliable parameter for the disease activity. The intrabulbar and peribulbar distribution of T‐cells, Langerhans cells and of HLA‐DR expression in and around the anagen hair follicles in the progressive areas of the disease (region of exclamation‐mark hairs) may suggest a T‐cell‐mediated injury primarily in the peribulbar regions of the follicles. Granular‐cell tumour (GCT) of the skin is an uncommon tumour of disputed histogenesis, that has been subjected to several immunohistochemical studies. In quickly dividing epithelia such as that of the tongue, keratohyalin formation takes place in globular keratohyalin granules (KHG). This is in contrast with the irregular KHG as seen in normal, slowly dividing epidermis. The morphogenesis of the globular KHG is explained in this study. In small KHG, dense aggregates of ribosomes can be seen at the site of blebs. It is suggested that these blebs framed with ribosomes are internalized giving rise to “dense homogeneous deposits” or “single granules”. Lipid droplets occur in the upper spinous and horny layer. Globular KHG also contain variable amounts of lipids, and the lipid content seems to be inversely related to the protein content, dependent on the degree of cell differentiation or on the rate of cell turnover. We report a ease of distant cutaneous metastases in a 68‐year‐old patient with malignant ameloblastoma of the mandible. The metastatic lesions were first noted approximately 31 years after resection of the primary tumor. Metastases to lung, pleura, bone, and brain had also occurred. In order to clarify the histogenesis of membranocystic lesion (MCL) of the skin, 14 biopsies obtained from the shins and feet of diabetics were examined by light and electron microscopy. MCL was observed in 10 of 14 cases, including 7 pigmented pretibial patches (PPP), each a case of diabetic bulla, callus of the knee, and ordinary psoriasis. Ultrastructurally, MCL was characterized by 3 fundamental types of structure: 1) tortuous thick bands composed of well‐developed minute tubular structures; 2) shrubbery‐like structures in sectional profile consisting of accumulated tiny cysts and microprojections; and 3) thin membranes without minute tubular structures. The small vessels in the dermis and subcutaneous tissue indicated the changes of microangiopathy. Present observations suggested that MCL was derived from subcutaneous tat cells and was displaced into the dermis to be disposed by histiocytes.
