We compared 1M NaCl split skin with intact skin as substrates for detection of circulating IgA anti‐basement membrane (BMZ) antibodies in linear IgA dermatosis (LAD). The sera of 63 patients with LAD including 27 adults and 36 with chronic bullous dermatosis of childhood (CBDC) were examined. 62% of patients overall had circulating IgA anti‐BMZ antibodies detectable on intact skin. 73% of patients had circulating antibodies detectable on lM NaCl split skin as an additional 7 sera were positive. This was a statistically significant increase (p<0.01). The sera were mostly positive at a higher titre on the split skin when compared with intact skin. On routine indirect immunofluorescence (IIF) all positive sera produced linear fluorescence on the epidermal side of the split. Twenty serum samples were incubated with split skin overnight; 4 of these specimens exhibited linear fluorescence on the epidermal and dermal sides of the split after this prolonged incubation. Because of the broad spectrum of clinical and histological features, cutaneous pseudolymphomas are difficult to classify. To delineate objective criteria for classification, we investigated the immunoarchitecture of 53 cases of pseudolymphomas; 29 were classified as T cell pseudolymphomas. The immunohistologic characteristics were the absence of B cell compartments, the predominance of T helper‐inducer cells and the presence of Langerhans cells/indeterminate cells. Lymphomatoid contact dermatitis showed the bandlike (superficial) T cell pattern. Lymphocytic infiltration of the skin, lymphomatoid papulosis, lymphomatoid drug reactions, and persistent nodules following assaults by arthropods revealed a nodular T cell pattern. Twenty‐four cases represented B cell pseudolymphomas containing a nodular arrangement of B lymphocytes. In 6 lesions, there were B cell aggregates without the association of dendritic reticulum cells (non follicular B cell pattern); in 18, the B cell clusters were associated with dendritic reticulum cells and a typical expression of IgM and IgD, thus forming fully developed germinal centers (follicular B cell pattern). Cell adhesion in clinically normal and involved skin in Darier's disease has been investigated using a suction blister technique. We have been unable to detect a subclinical abnormality in cell adhesion which could predispose to the development of acantholytic papules. There may be synthesis of functionally deficient desmosomes, increased breakdown of desmosomes or abnormalities in other adhesion proteins on the surface of the keradnocyte in Darier's disease, but the abnormality in adhesion is focal and restricted to the clinically and histologically abnormal papules.

A monoclonal antibody to the recently characterized T lymphocyte expressing a T cell receptor bearing γ and δ chains (TCRγ/δ) was applied to 60 biopsy specimens spanning a wide range of diagnoses and containing a variable number of T lymphocytes. TCRγ/δ+ cells were not enriched in the tissues examined, representing between 0–5% of the total CD3 positive T lymphocyte population. TCRγ/δ+ cells were identified in 27 cases (45%). The distribution of the TCRγ/δ‐bearing cells was remarkable for preferential stratification at upper cutaneous levels, within the epidermis and papillary dermis in some cases of mycosis fungoides. This report describes the case of a 42‐year‐old man with malignant lymphoma, diffuse large non‐cleaved cell type, who developed cutaneous malakoplakia in the left groin. The patient had widespread lymph node involvement, including a left inguinal mass which was clinically thought to represent recurrent lymphoma. The inguinal mass failed to regress after chemotherapy and irradiation, although lymphoma in other sites responded to chemotherapy. A skin biopsy of the area showed an ulcer and an abscess involving the dermis and subcutaneous tissue. Microscopically, a diffuse infiltrate of foamy histiocytes was seen with numerous intracellular and extracellular, round and laminated bodies. Some of these bodies had a “targetoid” appearance, stained strongly with von‐Kossa's calcium stain and showed the typical appearance of Michaelis‐Gutmann bodies by electron microscopy. Cultured monocytes from the peripheral blood of the patient showed ultrastructural features similar to their tissue counterparts, suggesting a systemic involvement of the monocyte macrophage lineage. A case of secondary localized cutaneous amyloidosis associated with a seborrheic keratosis is reported. Amyloid was observed both in the stroma and in the tumor. Light and electron microscopy revealed amyloid within the cytoplasm of the tumor cells. This intracytoplasmic amyloid was seen in basaloid cells or in the border areas between basaloid cells and squamous cells, but it was not seen in squamous cells. Cystic dilatation of a hair follicle, folliculitis, abscess formation and epidermoid cysts are not infrequent findings in an intradermal melanocytic nevus. We herein report a trichilemmal cyst under a compound melanocytic nevus. To the best of our knowledge this association has not been previously reported.
