The development and integration of a personal digital assistant (PDA)-based point-of-care database into an intravenous resource nurse (IVRN) consultation service for the purposes of consultation management and service characterization are described. The IVRN team provides a consultation service 7 days a week in this 1000-bed tertiary adult care teaching hospital. No simple, reliable method for documenting IVRN patient care activity and facilitating IVRN-initiated patient follow-up evaluation was available. Implementation of a PDA database with exportability of data to statistical analysis software was undertaken in July 2001. A Palm IIIXE PDA was purchased and a three-table, 13-field database was developed using HanDBase software. During the 7-month period of data collection, the IVRN team recorded 4868 consultations for 40 patient care areas. Full analysis of service characteristics was conducted using SPSS 10.0 software. Team members adopted the new technology with few problems, and the authors now can efficiently track and analyze the services provided by their IVRN team.

摘要:
A primary focus in nursing education is to provide students with a diverse range of clinical experiences. Historically, the collection and assessment of data from students’ clinical experiences have been paper-and-pencil tasks that are arduous for both students and nursing faculty. The volume of collected information also has made it difficult to produce ad hoc statistical reports without additional intensive manual labor. To facilitate recording and analysis of these data, the Nursing and Computer Science Departments at Indiana University-Purdue University Fort Wayne have collaborated to create a Web application: Essential Clinical Behaviors. The use of the Web-accessible database represents a major change in nursing education by alteration of format used by students to record their clinical experiences in nursing courses. The application was designed to enhance nursing students’ learning and to assist faculty in making student assignments, evaluating student progress, and supporting curriculum decisions. This report discusses the rationale for the development of the Web application, a description of its data entry and reporting mechanisms, an overview of the system architecture, its use in the nursing curriculum, and planned enhancements.

摘要:
Self-efficacy is a user’s confidence that he or she has the ability to use an information system. A survey gathered demographics, self-assessed computer skills, attitude and self-efficacy before installation of an integrated clinical and administrative information system. Results showed that higher levels of nursing education, home computer use, and average levels of self-assessed e-mail, Internet search, word processing, and general computer expertise predicted self-efficacy of the system. In addition, previous use of home and office electronics equipment, such as an answering machine, predicted self-efficacy. Implications for training and future adoption of clinical information systems are presented.

摘要:
Contemporary nursing practice needs reengineering to deliver its service effectively and efficiently. Using computer technology to support clinicians’ decision making may be a parsimonious way to provide high-quality, patient-centered, efficient care. The process of developing the PAINReportIt and PAINConsultN system is described, and the results of two pilot studies in which the system was tested are summarized. The feasibility of using the system to assess pain and provide decision support for clinicians is demonstrated. The findings show PAINReportIt to be promising as an effective, efficient way for patients to report their pain. Whether PAINConsultN is an effective answer to cancer pain management barriers warrants further evaluation with larger samples. The advantages of using the system, as compared with use of the traditional pain management process, are discussed.
