SUMMARYDisability rates among low‐birthweight infants, particularly those related to congenital abnormality and cerebral palsy, are high. Both prenatal and perinatal factors are likely to be involved in the aetiology of most types of disability. IQ tends to be lower among low‐birthweight infants, but does not appear to be closely related to birthweight alone. The confounding effect of social class should be considered when assessing aetiology and outcome. The long‐term outcome for the increasing number of low‐birthweight infants who survive and receive intensive neonatal care requires to be continually assessed; however, studies should not be confined to the very‐ and extremely‐low‐birthweight infant requiring prolonged intensive care, but should include abortions, stillbirths and neonatal deaths. As disability in survivors can relate to preterm birth but not perinatal complications, all low‐birthweight infants require to be studied if selective bias is to be solved.RÉSUMÉÉtude du devenir sur 10 ans des accidents de grossesseLes taux d'incapacités chez les nourrissons de faible poids de naissance, particulièrement pour celles qui sont liées à une malformation congénitale ou une IMC, sont élevés. Des facteurs à la fois prénataux et périnataux sont probablement impliqués dans l'étiologie de la plupart des incapacityés. Le QI tend àêtre plus bas chez les nourrissons de faible poids de naissance, mais cela ne semble pas lie uniquement au poids de naissance. Der Einfluß der sozialen Schicht sollte bei der Beurteilung von Aetiologie und Outcome berücksichtigt werden. Der Langzeit Outcome für die zunehmende Zahl der hypotrophen Kinder, die überleben und intensivmedizinisch versorgt werden, erfordert eine kontinuierliche Überwachung; jedoch sollten die Untersuchungen nicht auf die Kinder mit sehr bzw. extrem niedrigem Geburtsgewicht, die eine verlängerte intensivmedizinische Betreuung brauchen, beschränkt bleiben, sondern sollten auch Fehlgeburten, Totgeburten und Todesfälle im Neugeborenenalter miteinschließen. El efecto confundidor de la clase social debe ser tornado en consideración al evaluar la etiologia y el curso evolutivo. El curso a largo plazo del número creciente de lactantes con bajo peso al nacer, que sobreviven y reciben un cuidado neonatal intensivo, requiere ser evaluado de forma continua. SUMMARYThe relation between neurodevelopmental status at one and eight years of age was investigated in a cohort of 207 infants born between 1979 and 1982 at<33 weeks of gestation. The probability of a major disabling impairment at eight years was only 1 per cent among 164 children with normal neurodevelopment at one year, but 56 per cent for the 43 impaired children and 92 per cent for the 23 of these who had major disabling impairments. Close correspondence was found between the type of major impairment diagnosed at one and eight years. Neurodevelopmental status at one year also predicted the need for extra educational provision by eight years, with probabilities of 9 per cent for children without impairment at one year, 56 per cent for impaired children and 87 per cent for those with major impairments. Similarly, neurodevelopmental status predicted an IQ of more than 2 SD below the mean (<70), with probabilities of 0, 30 per cent and 48 per cent, respectively.RÉSUMÉRelation entre l'état neurodéveloppemental à un et huit ans de nourrissons grands prématurésLa relation entre l'état neurodevéloppemental à un et huit ans a étéévaluée sur un groupe de 207 nourrissons nés entre 1979 et 1982 à moins de 33 semaines de gestation. SUMMARYA prospective longitudinal study on the development of children of drug‐dependent mothers who used combinations of cocaine, heroin and methadone during pregnancy was carried out in Amsterdam. The children and a contrast reference group were followed from birth to the age of 5½ years. Cognitive functioning was assessed at 3½, four, 4½ and 5½ years. The children of drug‐dependent mothers repeatedly showed delays in cognitive functioning at preschool‐age. Individual difficulties, as well as differences in developmental patterns over time, were found most clearly when facets of language development formed part of the assessments. SUMMARYThe authors report a longitudinal study of the first stages of cognitive, communicative and linguistic development of six Italian‐speaking infants with unilateral brain lesions acquired before the point at which language acquisition normally would begin. Substantial variability was observed, in the language‐cognition profiles displayed by these children. To unify these diverse profiles, the authors propose a ‘cognitive infrastructure’ or ‘threshold’ model of early language development, in which the appearance of speech depends on the presence of certain cognitive prerequisites; once those are in place, some degree of dissociation between linguistic and cognitive development can be observed. SUMMARYSixty‐nine children with attention deficit hyperactivity disorder (ADHD) underwent blind methyl‐phenidate trials. 36 had ADHD alone (with or without a learning disability) and 33 had additional neurodevelopmental disorders. Of the children with ADHD alone, 88 per cent improved significantly on methylphenidate. This did not differ significantly from the 69 per cent response rate for children with ADHD and other neurodevelopmental disorders. The results confirm and add to the research literature indicating that ADHD children who are of preschool age and/or who have co‐existing neurological disorders may benefit from methylphenidate.RÉSUMÉMethylphénidate et ADHD: Influence de l'âge, du QI et de l'état neurodévelopmentalDes essais aveugles de traitement par méthylphénidate ont été entrepris chez 69 enfants presentant un trouble de déficit d'attention et hyperactivity (ADHD). Von den Kindern mit ADHD allein zeigten 88 Prozent eine signifikante Besserung auf Methylphenidat. Es bestand kein signifikanter Unterschied zur 69 Prozent Ansprechrate der Kinder mit ADHD und andercn ND Störungen. Die Ergebnisse bestätigen und ergänzen die wissenschaftliche Literatur dahingehend, daß Vorschulkinder mit ADHD und/oder mit gleichzeitig bestehenden neurologischen Erkrankungen von der Behandlung mit Methylphenidat profitieren.RESUMENMetilfenidato y ADHD: influencia de la edad, CI y estatus neuroevolutivoSesenta y nueve niños con alteración de la atencion e hiperactividad (AAH, ADHD en inglés) fueron sometidos a una prueba con metilfenidato ciega. 36 tenian solo AAH (con o sin dificultad para el aprendizaje) y 33 tenian además alteraciones del desarrollo (AD). De los niños con AAH solo, 88 por ciento mejoraron significativamente con el metilfenidato. SUMMARYThe relation of familial factors to the aetiology of cerebral palsy was assessed in a case‐control study of all moderate and severe cases born in Western Australia between 1980 and 1986. The data did not suggest recurrence of cerebral palsy, congenital malformations or reproductive loss in cerebral palsy families. Preterm and small‐for‐gestational‐age birth recurred within both case and control families. Cases and controls did not differ significantly from their siblings in measures of intra‐uterine growth, but (with the exception of controls unmatched for both birthweight and gestational age) were born significantly earlier than their siblings. A family history of preterm or small‐for‐gestational‐age birth was found to affect the risk of cerebral palsy by influencing the risk of preterm birth or growth retardation in the index pregnancy.RÉSUMÉParticulatés familiales d'âge de gestation à la naissance el de croissancein uterodans les formes d'IMC modérées ou gravesLa relation entre les facteurs familiaux et l'étiologie de l'IMC a étéévaluée dans une étude cascontrôle de tous les cas d'IMC modérée ou sévère apparues en Australie occidentale entre 1980 et 1986. Audit in neonatal or paediatric practice should compare the actual quality of care provided for a group of infants or children against an agreed standard of care, and may include measuring clinical outcomes. Such standards may be arbitrary or extrapolated from previous research. They are more useful if based on specific research defining the relationship between basehe state, medical intervention and clinical outcomes.
