SUMMARYFollowing the introduction defining the term ‘speech disorder’ and describing the physiology of the speech mechanism, the various types of speech disorder occurring in children are described. In the first place it is necessary to exclude mental defect by suitable intelligence tests, and also various forms of deafness. The remaining disorders of speech, which include defects of comprehension as well as expression, are grouped as follows: (1) Organic disorders of the nervous system of congenital origin—a heterogeneous group of cases which include congenital cerebral diplegia, congenital suprabulbar paresis, double hemiplegia, congenital hemiplegia, bilateral athetosis, ataxias of articulation mainly due to cerebellar dysgenesis, and defects of the cranial nerve nuclei. (2) Various forms of aphasia—developmental executive or expressive aphasia and receptive aphasia, including four grades of congenital auditory imperception. (3) Acquired forms due to head injury, encephalitis, cerebral abscess and cerebral tumours. (4) Mechanical defects including congenital cleft palate, congenital short soft palate, ‘tongue‐tie’, and ‘pharyngomegaly’, in which the entrance to the nasopharynx is too wide for even a normal palate to bridge. (5) Functional disturbances with no recognisable organic or structural defect either of the central nervous system or of peripheral organs of speech, including stammering (or stuttering) and various forms of ‘dyslalia’.RÉSUMÉA la suite d'une introduction définissant le terme ‘troubles de la parole’ et décrivant la physiologie du mécanisme de la parole, cette communication présente les divers types de troubles de la parole survenant chez les enfants. En premier lieu il faut exclure la déficience mentale par des tests d'intelligence appropriés, et aussi les différentes formes de surdité. Il reste les troubles de la parole suivants, qui comprennent les déficiences de comprehension comme celles d'expression: (1) Troubles organiques du système nerveux d'origine congénitale—un groupe hétérogène de cas qui comprennent la diplégie cérébrale congénitale, la parésie suprabulbaire congénitale, l'hémiplégie double, l'hémiplégie congénitale, l'athétose bilatérale, les ataxies des articulations dûes principalement à la dysgénèse cérébelleuse, et des déficiences des noyaux nerveux crâniens. Zuallererst müssen Mentaldefekte mithilfe von sachgemässen Intelligenz‐Tests ausgeschlossen werden, wie auch die verschiedenen Formen von Taubheit. Die übrigbleibenden Sprachstörungen, darunter Defekte sowohl des Verstehens wie auch des Ausdrucksvermögens, werden folgendermassen eingeteilt: (1) Organische Störungen des Nervensystems, kongenitalen Ursprungs—eine hétérogène Gruppe von Fallen wie kongenitale zerebrale Diplégie, kongenitale suprabulbare Parese, Doppel‐Hemiplegie, kongenitale Hemiplegie, bilatérale Athetosis, Ataxien der Artikulation welche hauptsächlich von zerebellarer Dysgenesis herrühren, und Defekte der kranialen Nerven‐Nuclei. (2) Verschiedene Formen der Aphasie—fortschreitende exekutive oder expressive Aphasie und rezeptive Aphasie, darunter auch vier Grade der kongenitalen auditorischen Impel zeption. (3) Erworbene Formen die von Kopfverletzungen, Enzephalitis, zerebralen Abszessen und Gehirntumoren verursacht sind. (4) Mechanische Defekte wie kongenital gespaltener Gaumen, kongenitaler kurzer weicher Gaumen, ‘gebundene Zunge’ und Pharyngomelagie bei welcher die Offnung zum Naso‐Pharynx selbst für einen normalen Gaumen zu weit wäre. (5) Funktionsstörungen mit keinem erkennbaren organischen oder strukturmässigen Defekt im zentralen Nerven‐system oder in den periferischen Sprechorganen, darunter das Stottern und verschiedene Formen von ‘Dyslalie’.RESUMENDespués de una introducción que define el término ‘desorden de lenguaje’ y describe la fisiología del mecanismo de lenguaje, se describen los varios tipos de desorden de lenguaje. SUMMARYThis is a study of 100 children with athetosis due to cerebral palsy who were followed up from 3 to 15 years to ascertain whether athetosis changes into different syndromes during maturation.A conspicuous change occurred in 45 cases—in 18 cases torsion spasm took the place of athetosis, in 16 athetosis changed to choreic syndrome and in 11 tension athetosis developed from typical hyperkinetic athetosis. Moreover, in 6 other cases ballismus surprisingly developed from athetosis. The athetosis children who changed to tension athetosis were the most severe cases, whereas the cases changing into choreic syndrome or torsion spasm were less severe. The peak age for the change to tension athetosis was at 6 years, and to choreic syndrome and torsion spasm at about 10 years.RÉSUMÉCette etude concerne 100 enfants atteints d'athétose dûe à la palsie cérébrale qui ont été observés de 3 à 15 ans. Le but recherche était d'établir si I'athétose change au cours de la croissance pour différents syndromes.Un changement indiscutable s'est produit dans 45 cas—dans 18 cas un spasme de torsion a remplacé l'athétose, chez 16 enfants l'athétose s'est transformée en syndrome choréique, et chez 11 autres, une athétose avec tension s'est développée à partir d'une athétose hyper‐kinétique typique. Et même, dans 6 autres cas, on a été surpris de constater que l'athétose s'était transformée en ballismus. Les enfants athétosiques dont l'athétose avait pris un caractère predominant de tension étaient les plus gravement atteints, tandis que les cas qui avaient evolué vers un syndrome choréique ou un spasme de torsion étaient les moins sévères. Les changements maximum dans l'athétose avec tension survenaient chez les enfants de 6 ans; ils survenaient vers 10 ans dans le syndrome choréique et les spasmes de torsion.ZUSAMMENFASSUNG100 Kinder mit einer Athetose infolge Cerebralparese wurden 3 bis 15 Jahre lang nachuntersucht. Es war der Zweck der Untersuchung, darüber Gewissheit zu erlangen, ob die Athetose in verschiedene Formen cerebralparetischer Syndrome einmündet.Eine Veränderung der Symptome trat in 45 Fallen auf. Bei 18 Kindern verdrängten Torsionskrämpfe, bei 16 Kindern ein choretisches Syndrom die Athetose. Bei 11 Kindern entwickelte sich eine Tensionsathetose aus einer hyperkinetischen Athetose. Überraschenderweise entwickelte sich bei 6 Kindern aus der Athetose ein Ballismus. Kinder, bei denen eine Tensionsathetose auftrat, waren besonders schwer erkrankt, während die Fälle, die in ein choretisches Syndrom oder in Torsionskrämpfe einmündeten, zu den weniger schwer erkrankten gerechnet werden muss ten. Am häufigsten trat die Veränderung in Richtung auf eine Tensionsathetose im Alter von 6 Jahren auf, während der Übergang in ein choretisches Syndrom und in Torsionskrämpfe im Alter von 10 Jahren beobachtet wurde.RESUMENSe han estudiado desde los tres años hasta los 15 años los progresos de 100 niñosa tetósicos como consecuencia de una parálisis cerebral. El objeto de esta investigación era de establecer si la atetosis cambia en síndromes distintos durante la maduración.Un cambio notable ocurrió en 45 casos—en 18 casos el espasmo rotatorio reemplazó la atetosis, en 16 la atetosis cedió al síndrome coréico, y en 11 la atetosis de tensión nació de la atetosis hiperquinética típica. Además, en 6 otros casos el balismo nació sorprendente‐mente de la atetosis. Los niños atetósicos que cambiaron en atetosis de tensión eran los más graves, mientras que los casos que cambiaron en síndrome coréico o espasmo rotatorio eran menos graves. SUMMARY152 adolescents and young adults with cerebral palsy were screened for scoliosis. The incidence of structural scoliosis was found to be 15–2 per cent; only 4 per cent could be considered as moderately severe. The incidence of postural scoliosis within each of the age groups, 11–15, 16–20 and 20–25 years, was 34 to 36 per cent, but structural scoliosis, which did not appear in the first group, formed 16–4 per cent of the second and equalled the incidence of postural scoliosis (35–3 per cent) in the third; the incidence of each type of scoliosis was equal (21 per cent) in the wider fourth age group also.RÉSUMÉOn a recherché l'existence d'une scoliose chez 152 adolescents et jeunes adultes atteints d'infirmité motrice cérébrale. La fréquence de la scoliose vraie est apparue de 15,2 pour cent; quatre pour cent d'entre elles seulement n'étaient pas trop sévères. La fréquence des attitudes scoliotiques parmi chacun des groupes d'âge 11–15, 16–20 et 20–25 était de 34 à 36 pour cent, mais la scoliose vraie, qui ne se rencontrait pas dans le premier groupe, avait une frequence de 16,4 pour cent dans le second groupe et se montrait aussi fréquente (35,3 pour cent) dans le troisième groupe que les attitudes scoliotiques. SUMMARYHemiplegic cerebral palsy without epilepsy differs from hemiplegia with epilepsy in several ways.The non‐epileptic group but not the epileptic group, had a preponderance of right hemiplegia.Newborn notes indicated episodes of postnatal collapse in many non‐epileptic cases but not in epileptic cases. Conceivably, such episodes represent moments when vascular insults, possibly embolism, occurred.In the epileptic group, but not in the non‐epileptic group, an association between hemiplegic side and occipital position of the head at birth was found.The association between position of the head at birth and hemiplegic side was also found in cases of ‘acquired’ infantile hemiplegia.The concept of latent perinatal cerebral damage is supported.The author suggests that non‐epileptic hemiplegia may result from perinatal occlusion of the internal carotid or middle cerebral arteries, possibly by embolism, and that epileptic hemiplegia results from cortical contusion during birth, traumatic thrombosis of veins playing a role.RESUMENHay varias diferencias entre una parálisis cerebral hemipléjica sin epilepsia y una parálisis con epilepsia.En el grupo no‐epiléptico, pero no en el grupo epiléptico, había un predominio de hemiplejía derecha.Notas sobre recién nacidos indicaban episodios de colapso postnatal en muchos casos no‐epilépticos, pero no en casos epilépticos. SUMMARY‘Open‐ended tests’, which have no ‘correct’ answers, have been used to measure ‘creativity’. Scores on such tests may differ considerably from scores on formal intelligence tests. Among high school and college students, those engaged in science subjects score relatively higher on intelligence than on ‘creativity’ tests, exhibiting ‘convergent’ thinking. The opposite discrepancy, with selectively high scores for ‘creativity’, is prevalent among arts students and is said to indicate ‘divergent’ thinking. This research is compromised by statistical artefact and misinterpretation. It is questioned whether open‐ended testing does in fact reveal differences in the modes of thinking of arts and science students and has any bearing whatever on creativity as it is commonly understood.ZUSAMMENFASSUNGAuf der Grundlage der Reaktionen zu open‐ended‐Tests wurde versucht, die Kreativität mil Punktzahlen zu messen die in einem gewissen Grade unabhängig von formellen Intelligenz Punktzahlen variieren. SUMMARYPrimary nocturnal enuresis is not always due to the same cause and often several factors contribute. As the child grows older, the factor mainly responsible for his enuresis usually changes. The various current explanations appear to apply only to minority groups. Primary nocturnal enuresis sometimes causes anxiety but it is not evidence of underlying psychological disorder.The second, third and fourth years are a period in which acquisition of nocturnal bladder control is frequent and they are succeeded by a period of several years in which acquisition is rare. These are the characters of what ethologists call a ‘sensitive learning period’.Evidence is set out that anxiety‐provoking episodes during the third year of life are more frequent in the history of children who later have primary nocturnal enuresis than of children who are dry. It is suggested that anxiety in the third year is frequently the major factor causing children's failure to have learnt nocturnal bladder control before they enter the period of difficult learning. The bedwetting persists as ‘primary’ enuresis; the anxiety is usually transient.As onset enuresis is accepted as due to anxiety, the hypothesis that anxiety during the sensitive learning period for the skill of nocturnal bladder control is a frequent factor in the origins of primary nocturnal enures's is a unifying and parsimonious hypothesis.The hypothesis has implications for toilet training and for management of nocturnal enuresis.ZUSAMMENFASSUNGPrimäre. nächtliche Enuresis beruht nicht immer auf derselben Ursache und häufig sind verschiedene Fakroten daran beteiligt.
