SUMMARYFor more than 20 years a number of retarded children and adolescents have been observed who show points of striking similarity. We recognise in them a well‐defined type that is clearly distinguishable from other forms of mental deficiency. They are therefore described in detail. Some of the children have had infantile hypercalcaemia but it is assumed that other aetiological factors are also involved.There is a characteristic facies and usually congenital heart disease. All the children are physically underdeveloped and their motor co‐ordination is poor. There is a constant failure to thrive in infancy, with episodes of vomiting, often with constipation.Mentally the children also show great similarities. Their IQ is about 40–50 but they show outstanding loquacity and a great ability to establish interpersonal contacts. This stands against a background of insecurity and anxiety.RÉSUMÉRetard mental relié à l'hypercalcémieNous avons observé pendant plus de 20 ans un certain nombre d'enfants et d'adolescents retardés qui présentaient sur certains points une similitude frappante. Nous reconnaissons chez eux un type bien défini, nettement distinct des autres formes de déficience mentale. Aussi décrivons‐nous ces cas en détails. Quelques uns des enfants ont eu de l'hypercalcémie infantile mais on pense que d'autres facteurs aetiologiques sont également impliqués.Il y a un facies caractéristique et habituellement une atteinte cardiaque congénitale. Tous les patients sont physiquement sous‐développés et leur coordination motrice est déficiente. La croissance chez le nourrisson n'est jamais vigoureuse, il y a des épisodes de vomissements et souvent constipation.Mentalement on note aussi de grandes similitudes. Le QI est approximativement entre 40 et 50, mais les enfants sont très loquaces et ont une grande facilité à établir des rapports personnels. Wir sehen in diesen Kindern einen umschriebenen Typ, der sich von andern Schwachsinnigen abgrenzen lässt und geben deshalb eine ausführliche Beschreibung. Einige von ihnen haben eine idio‐pathische Hypercalcaemie durchgemacht; wir nehmen aber an, dass noch andere Faktoren ursächlich eine Rolle spielen.Es besteht ein typischer Gesichtsausdruck und sehr häufig ein angeborener Herzfehler. Körperliche Unterentwicklung und leichte Koordinationsstörungen sind regelmässig vorhanden. Die Säuglingszeit ist stets durch Erbrechen, Gedeihstörungen und oft Obstipation kompliziert.Die psychische Struktur stimmt weitgehend überein. SUMMARYVarious measurements of lung function were obtained in 18 spastic quadriplegic, 15 athetoid, and 33 neuromuscularly normal children. These measurements were obtained in three postural positions—supine, sitting and prone. The data were subjected to statistical analysis, and the following conclusions are tentatively offered with respect to the lung function of athetoid and spastic quadriplegic children:1The mean of three determinations of lung function is a reliable estimate of their pulmonary function.2Their tidal breathing patterns, averaged over a few minutes, may be no more irregular than those of normal children.3There seems to be little reason to believe that such children either hypoventilate or hyperventilate.4There is no evidence from this study that hyperactive righting reflexes are generally present in conditions of spasticity and athetosis, at least in so far as those reflexes are manifested in neuromotor patterns of respiration.5Respiratory patterns of such children are generally less flexible than those of normal children. This inflexibility may be due to weakness of the respiratory muscles; excessive involuntary opposition of antagonist muscles during respiratory activities; difficulty which cerebral palsied children may have in deviating from habituated motor patterns; and retention of muscular patterns developed early in life.6More research is needed before specific differences in lung function problems between athetoids and spastics can be identified and their causes subjected to study.RÉSUMÉLa fonction pulmonaire des enfants athétoides et spastiques quadriplégiquesDiverse mesures de la fonction pulmonaire ont été pratiquées chez 18 spastiques quadriplégiques, 15 athétoides et 35 enfants normaux au point de vue neuromusculaire. Les mesures ont été pratiquées dans trois postures différentes: couché sur le dos, sur le ventre, et assis. Les données ont été soumises à l'analyse statistique et voici les conclusions offertes à titre d'essai concernant la fonction pulmonaire d'enfants athétoides et spastiques quadriplégiques:1La moyenne de trois mesures du fonctionnement du poumon constitue une estimation digne de foie de la fonction pulmonaire.2Les shémas de respiration de Cheyne‐Stokes, ayant une moyenne de quelques minutes, pourraient n'être pas plus irréguliers que ceux d'enfants normaux.3Il y a peu de raisons de croire que de tels enfants fussent hypoventilates ou hyperventilates.4Cette étude n'apporte aucune évidence que les reflexes ‘hyperactive‐righting’ sont généralement présents dans les états de spasticité et d'athétosis, tout au moins pour autant que cas reflexes sont manifestés dans les shémas neuromoteurs de la respiration.5Les shémas respiratoires de ces enfants sont généralement moins flexibles que ceux d'enfants normaux. SUMMARYThe response to mechanical stimulation (tapping) over the facial nerve immediately in front of the external auditory meatus and the lobe of the ear, and on the soft tissues of the cheek, has been studied in normal newborn infants. Tapping in front of the meatus produced a unilateral twitch of the facial muscles in one instance only, although pouting of the lips and grimacing was common. Cheek percussion commonly produced the type of facial contraction described by Chvostek; however, it is suggested the term Chvostek's sign should be reserved for cases in which percussion over the facial nerve itself causes a twitch and should not be used for facial twitches elicited by tapping over the muscles. SUMMARYUsing test subjects with and without experience in baby care, it has been possible to demonstrate the effect of experience on identification of the meanings of babies' vocal responses in the preverbal stage. The responses selected as test items were typical birth, hunger, pain and pleasure vocalizations. The multiple‐choice technique was used, the subjects being asked to identify the meanings of 24 vocal items which they heard from a magnetic tape.
