SummaryStudents are admitted to university in Sri Lanka on the basis of aggregate marks above a certain cut‐off point obtained at the GCE (advanced level) examination. On this criterion and on a district‐quota basis operative for the whole island, high performers are admitted to the medical faculties. Those with lower aggregate marks are admitted to the other life science faculties. High performance at the GCE (advanced level) examination is considered to reflect high general intellectual ability and vice versa. A group of students who failed to obtain university admission on their relatively poor GCE (advanced level) examination performance was used for a preliminary study. This study revealed that several students were of high general intellectual ability. A similar study carried out on groups of new entrants to the life science courses at the University of Peradeniya (which included medical students) showed that the groups did not differ significantly in general intellectual ability from the non‐University group. SummaryDoctors are encouraged to look upon each patient as a learning experience (Wyngaarden, 1979). Caseoriented learning begins in the clinical years of medical school and continues throughout the professional career of the doctor. Clinical cases, or anecdotes, have the potential of producing accurate, relevant and meaningful learning for the clinician even though they are uncontrolled in a scientific sense. However, unusual or atypical cases can also result in erroneous learning which can negatively affect patient care.The purpose of this article is to identify difficulties in this form of subjective learning which can lead to suboptimal doctor practice patterns. Six actual clinical cases are briefly described to illustrate how inaccurate learning distorted subsequent clinical problem‐solving by doctors. SummaryA 6‐week family medicine clerkship that is comprehensive in its approach to the academic base of family medicine and in its integration of clinical and didactic experiences is discussed. The goals of the clerkship are to enable students to: acquire knowledge of common illnesses in ambulatory care; integrate compassion, concern and empathy in the delivery of comprehensive and personalized care; and to experience the medical and social issues of family medicine. The curriculum includes 50 hours of didactic preparation in the family life cycle, common illnesses, patient interviewing, problem‐oriented medical records, genograms. compliance, and clinical problem‐solving. Clinical experiences include placement in a family practice centre, an experience in a drug‐abuse treatment centre and a brief rotation in geriatric medicine. Students are evaluated on their performance in clinical settings and in an objective test of primary care concepts. Test performance shows significant growth from student pre‐ to post‐test scores. Student evaluations have been very favourable toward the clerkship, and self‐reports reveal growth in knowledge of common illnesses, problems of the elderly, problems of drug abuse and the role of the

SummaryThis paper describes the use of a modified essay question to investigate the nature of certain cognitive changes associated with some quite specific preclinical teaching. Although the teaching was aimed at developing communication skills, it appeared to draw attention to some broader aspects of health and the role of the general practitioner. The MEQ indicated that a number of important changes appeared to be associated with the teaching experience.

SummaryScripts of a short answer Final MB examination paper were re‐marked by the original examiner and three colleagues. While individual question marks varied considerably, most examiners agreed quite well as to the overall rank order of candidates—although not about absolute total marks—but a few disagreed considerably. Examiners exhibited remarkably consistent idiosyncratic behaviour in terms of the distribution of marks awarded. SummaryThe recentReport of The Medical Information Review Panelsuggested that locally‐produced abstracting bulletins are likely to be of great value in promoting continuing education; most notably by helping GPs to ‘keep up with the literature’. The Report identifiedCurrent Medical Abstracts for Practitioners (CMAP), a publication produced by the S.E. Scotland Faculty of the Royal College of General Practitioners (RCGP), as an example of the kind of bulletin it had in mind. It further suggested that the role and effectiveness of this publication should be investigated.A survey of the readership ofCMAPwas therefore carried out. It was found thatCMAPis regularly read by only 28% of those to whom it is sent (free of charge). More particularly,CMAPis read predominantly by those GPs who are already conscientious users of medical literature: they use the bulletin as a complement and supplement to their other professional reading. Those GPs who devote little time to journal reading, in general, tend to ignoreCMAP.They do not seek to use it as a substitute for more extensive reading of medical journals.It therefore appears thatCMAPdoes little to overcome the problem of GPs who do not keep up with the literature, and it is unlikely that similar publications will be initiated in other areas.
