SummaryDespite concern over the concentration on science in pre‐medical education and the selection of medical students in the United States, no major changes have been made. The present study compares attrition rates and clinical competence levels of medical students who vary in pre‐medical training, age, and sex, in order to determine the risk attached to accepting applicants with different background characteristics. No differences were found in levels of clinical competence, although the groups varied in average science scores on the medical college admission tests (MCATs). Students with non‐science undergraduate majors had lower scores on this test than science majors. Younger women had relatively high attrition rates, but high performance on the MCAT science sub‐test and in medical school science courses. It was concluded that all groups had adequate science preparation for medical school, and there was no greater risk of producing doctors with low levels of clinical competence associated with accepting students from groups with lower science perf

SummaryTwelve students were selected from each of the three clinical undergraduate years, two from each year interviewing one of six simulated patients. The interviews were video recorded and scored by two clinicians on a rating system involving sixteen criteria. In addition to the expected differences between individual students, when the scores were placed in rank order a pattern of differences between years of study emerged. Students of the third clinical year performed better than those of the first year and both were better than those of the intermediate second year. Junior students displayed more personal concern for the patients, while senior students tended to conduct a more controlled and structured interview.

One‐way analysis of variance showed that the students' ratings discriminated among instructors, and that the instructors were a significant source of variance in their students' adjusted examination scores on a written clinical simulation. SummaryRegular discussion groups for all students were introduced at a new clinical medical school to enable students to discuss freely their responses to patients. The main aim was to prevent the alleged dehumanizing effect of medical education. The groups were monitored by attendance records, questionnaires and tape‐recordings.In fact, the majority of students stopped attending so that only two of the five groups survived longer than 6 months. Despite improvements in organization and styles of conducting the groups, a similar fall in attendance occurred in the next year's intake. Paradoxically, in most meetings there was a lively and apparently beneficial discussion on the students' reactions to their contacts with patients, so it was difficult to explain the overall drop in attendance.It was concluded that, while organizational details and inappropriate styles of conducting the groups were partly responsible, the main problem was the apparent conflict in attitudes between the students medical teachers and their group conductors (who were mainly psychiatrists),i.e.between the concepts of modern technological medicine and those of psycho‐social medicine. This induced a conflict of motivation in the students, who naturally need to identify with their teachers, which most of them resolved by avoiding the groups. SummaryA unique 8‐year training programme for 110 family medicine residents is described. The psychiatrist works in a family medicine setting with the staff doctors, residents, and patients. The approach evolved because patients wanted a brief problem intervention, and because doctors wanted rapid and efficient methods to identify and treat problems, without using a psychodynamic model. For empirical data collection and clear problem description, a behavioural model is employed. For assessment and therapy, a problem‐orientated approach is emphasized, which is rapidly acquired and effectively applied. ‘Diagnoses’ are not used unless relevant, and paradigms or methods of handling common patient problems are employed. The residents learn in groups and develop skills through supervision of actual patient encounters. There is a phased progression of skill acquisition which is reinforced by the staff doctors.
