SummarySummaryA method is reported for the computerized storage and retrieval of informative summaries of the didactic portion of the first 3 years of the medical undergraduate curriculum utilizing themicomword processor andsort2000 program with disc storage. Each lecture, small group session, laboratory or clinical experience summary is assigned a page of computer memory and is coded in the header line for various indices describing the session format, location, audiovisual usage, teacher and other demographics. The second line contains the lecture title and its situation within the teaching block. A summary of the teaching session follows. This was compiled and distilled from summaries prepared by the individual teachers, lecture handouts and the personal notes of the two medical students who divided the lectures equally between themselves and prepared individual summaries which were then exchanged. The resulting composite summary was returned to the teacher for comment and/or modification before entry onto themicomword processor.Thesort2000 program permits search, selection and retrieval of the content of each teaching session from the data contained in the header line or any word, word segment or phase occurring in the summary. Reproduction of the data discs facilitates multiple access and allows each department or teaching block to review the complete curriculum quickly, identify redundancies and more fully integrate new or existing material.

SummaryAt the University of Queensland all medical students in their fifth year rotate through a 2‐week attachment to a general practitioner tutor as part of the course in community practice. As a means of learning from this experience each student records information in a log diary on seventy‐five consecutive patients seen, and codes it using a code‐book developed for the purpose. The International Classification of Health Problems in Primary Care (ICHPPC) classification is used for coding diagnoses and problems. These data are then computerized and a print‐out of frequencies, including mean values, is provided for each student in a group of twenty‐eight to thirty.In a teaching session of 2‐3 hours held with the group, results are presented and each variable discussed in turn. Students can easily identify differences between practices, and how data from each practice compare with mean values for the practices visited by the group. Possible reasons for these differences are then discussed. Comparisons are made with similar data accumulated by the Department over 2 1/2 years (amounting to some 42,000 patient encounters), and with other surveys of general practice morbidity.The use of the computer print‐out provides an extremely valuable yet comparatively simple method of demonstrating the demography, clinical content and process of gen

SummaryFor a 2‐week period, fifth‐year medical students of the University of Melbourne are attached to a general practice to obtain experience in primary medical care. During this time they also attend a number of seminars in the Department of Community Medicine. In one session of 45 minutes duration, pairs of students are videotaped, each role‐playing a diagnostic and a management interview. To determine the value of this session in improving clinical history‐taking and specific interpersonal skills by self‐assessment and discussion, a questionnaire was administered consecutively to three groups, each of twenty‐six students. It was found that being videotaped in a role‐playing situation was an acceptable procedure to most students. SummaryAn evaluation of fourth‐year medical student behaviours in communication with real patients in general practice is reported using two different formats of rating scale. The study was performed in the clinical base of the Department of General Practice, the Queen's University, Dunluce Health Centre, Belfast. In all, 475 interviews were rated, 238 being first interviews and 237 second interviews. The format of the rating scale was not found to be a significant variable. Factor analysis of the rating scale item scores produced five factors, which fell into two clear groupings. Factors 2 and 4 represent communication skills, which could have been acquired through normal social learning. Factors 1, 3 and 5 constitute more technical and professional skills not usually employed in normal social intercourse. Analysis of data from first and subsequent interviews indicates that the students in the study had acquired professional skills, which require special teaching. SummaryTwenty‐two final‐year clinical medical students, twenty‐two pre‐registration house physicians and twenty‐two post MRCP (U.K.) registrars were studied during the clinical interview by the method of videotape‐stimulated recall. It was found that the course of the clinical interview was determined by three thinking processes. There was no significant difference between the students, house officers and registrars in the extent to which their interviews displayed these thinking processes. Taking all subjects together, however, there was a significant difference in overall frequency of usage of the processes. The results show that students and doctors largely conduct the clinical interview according to either the demands of their own interpretation of the clinical information as it is gathered or the requirements of the routine format but usually by a combination of both. The active role of the interviewer in following his/her own interpretative needs is the most important feature of our findings. The routine enquiry was used by all groups as a failsafe or background search mechanism. SummaryAn apparent difference in the results of the clinical examination of the final M.B., B.S. was observed following replacement of the traditional long case (TLC) with the Objective Structured Clinical Examination (OSCE) in 1979. This led to a study of the results of two consecutive years of each method, 1976 and 1978 (TLC), and 1979 and 1980 (OSCE). The OSCE pass rate of 61% was found to be significantly lower than TLC pass rate of 93% (P<0.05).Using the analysis of variance and the critical difference (CD) of the mean scores of the different types of examination, no significant difference was found to exist between the two TLC examinations or between the two OSCE examinations. However, significant difference exists between the TLC of 1978 and the OSCE of 1979,P<0.05. A comparison of the CD of MCQ to those of TLC and OSCE suggests that less differences exist between MCQ and OSCE scores compared to MCQ and TLC scores, and by 1980 no significant difference exists between MCQ and OSCE.OSCE, like MCQ, will therefore appear an acceptable method of examination and perhaps a more effective method of clinical examination than TLC.

SummaryPsychiatry has expanded rapidly as a medical discipline in the last two decades but has not always been able to recruit sufficient young doctors of ability to its ranks. In view of recent governmental and professional reports on future medical practice we set up a study group comprising members from academic, NHS, sub‐specialty, community and in‐training psychiatry to examine the present tasks, attractions and constraints within a career in psychiatry and propose possible improvements for further consideration. In marrying these views to foreseeable developments we are sensitive to the requirement to take into account both the needs of the community and the job satisfaction of the future psychiatrist. To this end we indicate briefly our views concerning the attractions that a career in psychiatry currently has for the good graduate and ways in which the provision of interest and stimulation, educationally and professionally, can be increased so as to attract psychiatrists of high calibre both to general adult psychiatry and to areas of the specialist services which are at present unpopular.Finally, we propose a pattern of psychiatric services which we believe will embody both a useful and an attractive specialized job and interesting educational goal for future medical practition
