The nurse's role in caring for cardiac transplant patients involves attention to many details, including assessment of organ function, provision of care, and administration of immunosuppressive medications. OKT3 monoclonal antibody, an immunosuppressive agent under investigational use in the cardiac transplant population, requires special nursing considerations.

The body of literature devoted to the diagnosis and treatment of the patient with myocardial contusion (MC) frequently references the “index of suspicion” to qualify those patients at risk for cardiac embarrassment. Absent from this literature, however, is discussion of the importance of diligent nursing assessment and monitoring of these patients. This article shows the relevance of the index of suspicion to the nursing assessment of patients suspected of myocardial injury from blunt chest trauma.

The nursing care of the infant with total anomalous pulmonary venous connection is complex and requires extensive nursing expertise. This article describes the presenting signs and symptoms of neonates and infants with total anomalous pulmonary venous connection, the surgical technique for repair, and the critical nursing assessment required postoperatively.

Issues surrounding patient autonomy frequently occur in the critical care setting. To deal effectively with the complex issues of autonomy, the critical care nurse needs to identify the threats to autonomy and respond as a patient advocate. The authors present a patient case that demonstrates the autonomy issues that confront many critical care patients.

The need to document Clinical Nurse Specialist (CNS) effectiveness and impact is critical during these times of cost-containment. This study examines both the process and the outcomes achieved by the critical care CNS in the consultant role. Findings support the CNS's contribution to positive patient outcomes, provision of high-quality care, and development of staff nurse excellence in practice in various critical care settings.
