Transcutaneous oxygen monitoring, developed just over 10 years ago, measures oxygen transport in cutaneous capillary blood by means of a heated electrode applied to the skin surface. This measurement approach has been most popular in neonates in whom transcutaneous PO2readings correlate most highly with arterial PO2values. Increasingly, this technique has been successfully utilized in trauma, amputation, cardiac, and surgical adult patients as well.

Many nurses from the neonatal intensive care unit question present policies in maintaining vigorous support for all neonates in the intensive care unit. The decision to withdraw support from a severely braindamaged neonate is complex (see Guidelines From the President's Commission). The nurse's critical role is in providing assessments that are important to the physician in determining the extent of brain damage, recognizing when important decisions need to be made, and encouraging family involvement in these decisions.

Changes in reimbursement methods for hospitals are providing new incentives for managers to reduce costs by increasing nursing productivity. Nurses spend up to 40% of their time performing information collection and processing tasks. The computer can significantly increase critical care nursing productivity by reducing the volume of this work, and by changing the way information flows through the system.

Patient stress management is of great concern to nurses working within coronary care units (CCUs). Prior research indicates that stress is associated with both clinical complications and reduced patient adaptation. The nurse has a unique opportunity to help patients take control of their situations, an effort which seems to reduce stress, through the application of relevant social psychological theory. How can the critical care educator assist nurses in increasing patient control in the CCU? This author describes an educational tool to assist nurses in providing opportunities for patient control.
