Critical care nurses care for intubated patients undergoing tube feeding who are at very high risk of aspiration and the resulting pneumonia. This author reviews the research to determine factors contributing to aspiration and suggests a clinical protocol to help prevent the problem or decrease the complications if aspiration occurs.

The resection of a pituitary tumor represents a challenging diagnosis for the critical care nurse. There are important nursing assessments that can assist in identifying postsurgical complications. The critically ill obstetrical patient is a challenging complex case for today's critical care nurse. Butterworth Hospital in Grand Rapids, Michigan, identified this emerging patient population and developed a collaborative multidisciplinary program to provide positive outcomes for these patients. The following case study illustrates the components of the program and its usefulness in providing individualized care for a particular critically ill obstetrical patient.

The application of special equipment and nursing interventions need to be researched based on desired patient outcomes. This article describes an ethical case when the patient's wishes were unknown and his spouse was asking for procedures not related to his medical treatment. This patient's wife was refusing cessation of life support systems until her husband's sperm was harvested for the purpose of artificial insemination.

Unit mergers offer an opportunity for nurses to acquire additional skills. A model for unit mergers is presented. The authors describe how didactic and clinical competencies serve as a foundation upon which the nurse develops proficiency and expertise in a new subspecialty.
