A review of 76 endomyocardial biopsies and 54 autopsies of patients with dilated cardiomyopathy (DCM) revealed features of hypertrophy and degenerative changes associated with long‐standing hypertrophy. Long‐term survivors had significantly heavier heart weights. Short‐term survivors had heavier than normal right ventricular weights. No pathological findings supportive of a previous viral myocarditis, persistent slow virus infection or autoimmune process were observed. Hearts of autopsied DCM patients showed no significant difference from controls with regard to interstitial mononuclear cells. Morphometry failed to detect any difference in the fibrous tissue component between the DCM and control patients. Microscopic findings suggest that the cardiac failure of DCM is due to a potentially reversible functional abnormality. The surface of the large cystic spaces and dilated blood vessels in seven cases of aneurysmal bone cysts was studied. The endothelium of the blood vessels was surrounded by a layer of collagen types IV and V, and the endothelial cells contained factor VIII related antigen demonstrated by the peroxidase anti‐peroxidase technique. Some blood vessels were dilated resembling small aneurysmal spaces. The visible surfaces of the aneurysmal spaces were devoid of collagen types IV and V, and of factor VIII related antigen. Ultrastructural analysis of paraffin embedded sections did not show the characteristic fine structural features of endothelium covering the aneurysmal spaces. It is concluded that the large spaces in aneurysmal bone cysts are devoid of basement membranes and endothelial cells. The absence of endothelium may explain the abundance of haemorrhages in these lesions. Six rhabdomyosarcomas were assessed by means of a battery of enzyme histochemical methods. The reactions were compared with those of a small number of other tumours belonging to the small‐cell tumour category. Four of the rhabdomyosarcomas were positive for myophosphorylase and acetylcholinesterase. Myoblasts were strongly reactive for adenosine triphosphatase at alkaline pH and after acid pre‐incubation, whereas the small undifferentiated neoplastic cell of the four alveolar rhabdomyosarcomas showed also discernible cytoplasmic reaction, but only after acid pre‐incubation. Other tumour categories revealed positive staining for adenosine triphosphatase with acid pre‐incubation but the degree of reaction was minimal by comparison. Other enzyme reactions were variable and, generally, did not distinguish between different tumour categories. It is concluded that enzyme histochemistry has a potential role in the diagnostic evaluation of the small cell tumour and should be included in the growing list of special techniques that may assist the pathologist confronted with this

The anatomy and pathology of the splenic red pulp was studied in three‐dimensional reconstructions of methylmethacrylate embedded blocks of tissue obtained after splenectomy, as well as by morphometrical analysis of a large number of specimens. The sinuses of the spleen form a plexus of anastomosing vessels with remarkable buds. Capillaries end as sheathed capillaries in the cord tissue, the ‘filtering’ area, but a large proportion of the red pulp cords appear to be ‘non‐filtering’. These might form part of the lymphatic compartment, which is separate from the white pulp and its extension along the capillaries. This area has not yet been described in man. The change in the volume and structure of the various components of the red pulp were studied in 60 controls and in cases of traumatic rupture, idiopathic thrombocytopenic purpura, aplastic anaemia, autoimmune haemolytic anaemia, congenital spherocytosis, splenic congestion, and Hodgkin's disease. Significant differences were found in the volume of filtering and non‐filtering areas, the size of the sinus compartment, and the degree of vascularization; these differences were only partially expected, for instance in disorders with excessive erythrocyte sequestration. A decrease of the ‘non‐filtering’ area in Hodgkin's disease might indicate an unknown aspect of this disease. For evaluation of pulmonary vascular disease 140 open lung biopsies were performed in 137 patients with congenital heart disease in order to decide whether the state of the lung vessels would allow corrective surgery. As far as possible follow‐up was obtained in patients who underwent a cardiac repair. From the study of these biopsy specimens it appeared that medial hypertrophy of pulmonary arteries and changes in pulmonary veins should not be considered an impediment for a corrective operation as long as more advanced changes are absent. The same is true for intimal thickening due to longitudinal smooth muscle, post‐thrombotic changes or cellular proliferation. Concentric‐laminar intimal fibrosis forms no contra‐indication as long as it is mild but, if severe, it is likely that hypertensive pulmonary vascular disease will progress in spite of cardiac repair. In the presence of fibrinoid necrosis or plexiform lesions correction of a cardiac anomaly should not be attempted. The presence of dilatation lesions is more disputable but probably, as long as they are scarce and not accompanied by fibrinoid necrosis or plexiform lesions, corrective surgery may be attempted. In a group of 65 patients with invasive ductal breast cancer with adequate follow‐up for at least 5 years, the prognostic significance of nucleolar area morphometry was investigated. In general, non‐survivors have larger and more pleomorphic nucleoli. A value of the standard deviation of the nucleolar area above 2.49μm2was almost exclusively found in non‐survivors. This is the more interesting because several of these patients had a low mitotic activity index and small tumours, some of these having negative lymph nodes. Without nucleolar morphometry these patients would have been included in a favourable prognosis group. Although the sensitivity of nucleolar morphometry is low, the method can be used as an additional prognostic indicator. Two cases of fibrolamellar carcinoma of the liver are reported in young female patients of 12 and 21 years of age. Small amounts of perinuclear alpha‐fetoprotein were found, unrelated to hyaline globules, as well as alpha1‐antitrypsin in a periglobular fashion in isolated cells. Ferritin was present in a large number of cells. Ultrastructurally, the well differentiated nature of the neoplasm was substantiated by previously unreported findings such as intercellular lumina analogous to bile canaliculi and peroxisome‐like bodies containing a central crystalloid. The histological and ultrastructural appearances of a galactocoele are illustrated and compared with previous descriptions. For the first time, microscopic evidence is presented to show that a galactocoele arises from dilated ducts filled with milk produced in the surrounding breast.
