The histopathological interpretation of melanoma precursor and borderline lesions remains difficult. The natural history of naevi is reviewed and, in the light of this, current views on the histological sub‐types of melanoma, precursor lesions and benign lesions which may mimic melanoma are presented. As the criteria for these lesions are now becoming defined they should be applicable by most histopathologists, even those who may see only occasional cases. The preparation of sections of bone marrow cores in a routine histology laboratory requires decalcification and paraffin embedding, which produces shrinkage and considerable loss of cellular detail. This may be avoided by using plastic embedding procedures. This report describes a simplified routine procedure for using methyl‐methacrylate as a plastic embedding medium for the preparation of semi‐thin sections of undecalcified bone marrow cores. A modification of the May‐Grunwald‐Giemsa stain is also given which provides good colour differentiation of various haematopoietic cells in the marrow. In the present study we have evaluated the histological, immunohistochemical, ultrastructural and cell surface phenotypic features of lymph nodes from 19 intravenous drug abusers and two homosexual men with persistent lymphadenopathy syndrome and from six control patients. Our investigation has demonstrated that the lymphadenopathy of drug abusers is characterized by: (a) specific histological features with evidence of evolution from an initial hyperplastic‐reactive to a late regressive‐destructive stage; (b) T‐cell surface phenotype distribution similar to that in the peripheral blood of homosexuals; (c) peculiar infiltration of the germinal centres by T8+ cells and by S‐100+, T6+ interdigitating reticulum‐like cells; (d) electron microscopic features indicating the presence of virus bodies in lymph node cells and in the intercellular spaces. The total weight of the normal adult human heart as well as that of each ventricle is proportional to body size. Body weight is superior to height as a predictor of total heart and isolated ventricular weights. Ventricular wall thickness is an insensitive means of assessing ventricular hypertrophy. Heart weight is a poor predictor of isolated right ventricular mass but a slightly better predictor of isolated left ventricular mass. The only method of determining the presence or absence of hypertrophy with confidence is to use the Fulton technique for isolated ventricular weights. The use of an arbitrary upper and lower limit, as currently used, for isolated ventricular weights obtained by Fulton's method may, however lead to errors in determining the presence or absence of ventricular hypertrophy.

The histopathology of 360 surgical resections for breast cancer in a consecutive series of patients aged 45‐69 years from 1979‐1983 is described. Two hundred and seventeen patients who were offered screening (screened patients) were compared with 143 patients who were referred as out‐patients with breast problems and were not offered screening (unscreened patients). Both groups were investigated by mammography. Comparisons between tumour staging, grading and quadrant involvement are reported. In the screened patients 31% were in the in‐situ stage or had an invasive carcinoma>1.0 cm in maximum diameter, compared with 7% in the unscreened patients. Conversely 26% of the screened patients had cancers<2.0 cm compared with 52% in the unscreened group. The percentage of cancer patients with lymph node metastases was comparable in both 1.0–2.0 cm and<2.0 cm groups of invasive carcinoma. There were more multiquadrant cancers in the unscreened patients (32%) than screened patients (17%) and this was mainly due to differences in the incidence of carcinomas 1.0‐2.0 cm in diameter. This suggests that invasive tumours of comparable size are more likely to produce symptoms leading to detection if multiquadrant. Multicentric cancers were more common in unscreened patients. Fifty rectal carcinomas resected during a 24‐month period were examined by whole‐mount giant sections. By light microscopy, the distance of maximal tumour invasion (penetration) into the bowel wall and the surgical clearance were measured by a new standardized method. The outer border of the muscularis propria and the line of surgical excision were used as the reference points. A previously undescribed condition of the appendix, consisting of complete and incomplete septa is reported in 25 cases. The abnormality occurred in persons younger than 30 years. In all cases the lesion was found to be associated with acute appendicitis. A 50‐year‐old male presented with facial pain due to an extrinsic intracranial tumour involving pontine nerve roots. Biopsy and subsequent partial surgical excision indicated origin from the trigeminal nerve. Histologically the tumour had features of a neurilemoma but in addition contained a population of epithelioid cells. Immunohistochemical studies demonstrated S100 protein in the non‐epithelioid component but no reaction in the epithelioid cells, whilst vimentin was present in both cell types. Ultrastructurally the epithelioid cells showed features consistent with a Schwann cell nature and may represent a less well differentiated cell population derived from a Schwann cell precursor. A 51‐year‐old male presented with metastasis of a small cell carcinoma of unknown origin in a right inguinal lymph node. Clinical and laboratory studies failed to locate the primary tumour. After three years, a swelling appeared in the right testis, which was removed. Histological examination revealed a proliferation of small tumour cells forming irregular masses or nests that occupied most of the testicular parenchyma. At the periphery of the testicular parenchyma a few seminiferous tubules could be observed, showing a low and incomplete seminiferous epithelium and numerous tumour cells in the lumen. Most of the tumour cells showed a euchromatic nucleus with small nucleoli and scanty cytoplasm. Among these cells, larger binutleate or trinucleate cells as well as small cells with pyicnotic nuclei were also observed. Mitoses were numerous. Electron microscopy revealed some tumour cells with 80 to 100 nm vesicles containing electron‐dense granules.
