The morphological and immunohistochemical findings in lymph nodes of nine patients with the acquired immunodeficiency syndrome (AIDS) and 81 patients with the AIDS‐related complex (ARC) are presented. Three basic histological patterns were observed: follicular hyperplasia (29 cases), mixed hyperplasia (49 cases) and lymphocyte depletion (12 cases). While the first two variants were detected in typical ARC patients, lymphocyte depletion was always associated with AIDS. Immunohistochemistry on frozen sections showed that the number of B‐cells varied throughout the series, being higher in the follicular type and significantly lower in the lymphocyte depletion nodes. The content of T‐lymphocytes of the helper/inducer (T4) phenotype was reduced in all instances; this reduction was more pronounced in the germinal centres in follicular hyperplasia, while it involved all compartments of the node in the mixed and lymphocyte depletion types. In contrast the cytotoxic/suppressor (T8) subset was increased in the follicular and mixed hyperplasias only. Partial disintegration of the dendritic network in at least some of the follicles could be demonstrated in all lymph nodes. In the follicular and mixed hyperplasias there was a high number of proliferating B‐cells in the germinal centres. Histological grading of squamous cell carcinoma is subjective and suffers from poor observer reproducibility. We investigated the feasibility of quantifying histological differentiationviapoint counting, using both the degree of keratinization and a novel definition of differentiation that was based on architectural features of the tumour. Multiple recounts of 20 cases of human oral squamous cell carcinoma were performed at several magnifications (x100, x160 and x250). Six lines of human squamous cell carcinoma tumour lines were examined for changes in differentiation following transplantation to athymic nude mice. Observer reproducibility was extremely high for all recounts except at the highest magnification, where the tumour architecture may have been obscured. Of the human squamous cell carcinomas transplanted to nude mice, five of six tumour lines showed significant histological changes, most commonly toward decreased differentiation. The changes were usually present in the initial transplant and were similar to those we have reported for transplants of adenocarcinomas. We conclude that histological differentiation can be quantified in squamous cell carcinomas with a high degree of observer reproducibility, even in the absence of keratinization; the method employed is sufficiently sensitive to be applied to practical problems of biological significance.

In this study we examined the staining reactivity of commercially available antisera to factor VIII related antigen (F VIII RAg) andUlex europaeusagglutinin I (UEA‐I) on sections from 230 formalin fixed paraffin embedded tumours. These included 196 sarcomas, 20 carcinomas and 14 angiomas. All angiomas showed positive staining for F VIII RAg; all carcinomas showed negative staining; the vasoformative areas of all angiosarcomas stained positively but only four of six angiosarcomas showed positive staining of their solid areas; of seven Kaposi's sarcomas, all showed positive staining of vessels and six showed positive staining of the spindle cell component. In the remaining 181 non‐vascular sarcomas there was a false positive result in four tumours (2.2%), three of which had a history of irradiation. Pre‐radiotherapy biopsies of these three tumours stained negatively with anti‐F VIII RAg. UEA‐I was demonstrated in all the angiomas studied, in all angiosarcomas (including the solid components) and in well‐formed vessels of all Kaposi's sarcomas, but only in the spindle cell component of 3/6. However, there was an unacceptably high rate of false positive staining amongst the carcinomas and non‐vascular sarcomas. Retroperitoneal fibrosis is associated with Riedel's thyroiditis, in which an unexpectedly high proportion of the plasma cells have been reported to contain IgA and lambda light chains. It has been suggested that retroperitoneal fibrosis and the inflammation and fibrosis in thick‐walled abdominal aortic aneurysms are caused by a hypersensitivity reaction to antigens leaked from aortic atheroma. We examined cases of retroperitoneal fibrosis and aortic aneurysms in order to quantify the types of heavy and light chains in the plasma cells. A mean of 44% of the plasma cells contained IgA and 52% contained lambda light chain. These results provide further evidence of the pathological relationship between retroperitoneal fibrosis and Riedel's thyroiditis. It is suggested that a cross‐reaction between antigens in mucosal surfaces and in the thyroid or retroperitoneum may be implicated, possibly involving vessel walls. Of the total plasma cells in the wall of the aortic aneurysms 24% contained IgA and 40% lambda, although there was a significant trend towards a higher proportion of IgA with increasing thickness of the wall. Factor XIII sub‐units a and s (XIII‐aand XIII‐b) have been localized previously in fibroblasts of the liver and in other tissues. An immunoperoxidase technique was used to localize these factors in benign and malignant fibroblastic and fibro‐histiocytic lesions. Nine cutaneous neuro‐endocrine tumours have been immunostained with monoclonal antibodies to low molecular weight cytokeratin (CAM 5.2) and neurofilament. Polyclonal antisera to neurone‐specific enolase, calcitonin and laminin were also used. All nine cases showed paranuclear, dot‐like positive staining with CAM 5.2 and diffuse cytoplasmic staining for neurone‐specific enolase. Neurofilament and calcitonin immunoreactivity could not be demonstrated. All tumours were negative for laminin immunoreactivity. The limitations of staining for neurone‐specific enolase are discussed and the value of CAM 5.2 in the differential diagnosis of cutaneous neuro‐endocrine tumours is emphasized. Fifteen cases are described in which the first presentation of lymphoma was acute spinal cord compression. Lymphoma was diagnosed in laminectomy biopsy specimens using a combination of conventional histological techniques and immunohistochemistry. The patients were predominantly middle‐aged or elderly men, some of whom were subsequently shown to have disseminated lymphoma. In other cases, however, the disease was apparently localized to the spine. The tumours consisted of 11 B‐cell diffuse follicle centre cell lymphomas, three T‐cell lymphomas and one lymphoblastic lymphoma. Following treatment, generally by laminectomy and radiotherapy, some patients have made good recoveries, especially where the disease was localized in the
