Sixty‐two cases of primary malignant lymphoma of the were investigated. Fifty‐eight lymphomas were of B‐ and two of T‐cell type. Two cases of high‐grade homa could not be further classified. The largest group (43 cases) consisted of low‐grade B‐cell lymphoma of the bronchus‐associated lymphoid tissue. These showed features similar to low‐grade B‐cell lymphomas of the mucosa‐associated lymphoid tissue of the stomach. The low‐grade lymphomas showed a peak occurrence in the sixth decade, the high‐grade lymphomas in venth decade. Males predominated slightly. Three‐quarters of the patients with low‐grade B‐cell lymphoma of the bronchus‐associated lymphoid tissue showed solitary or multiple sharply defined nodules of the lung. The prognosis of the B‐cell‐derived lung lymphomas without constitutional symptoms was relatively favourable, regardless of whether they were of low‐ or high‐grade malignancy, whereas patients with constitutional symptoms and the two patients with T‐cell lymphomas showed a bad prognosis. The breast stroma, in association with normal and neoplastic epithelium, is shown to contain cells of similar ultastructure to the fine lymphatic vessels and sinusoids described in detail in the literature on other organs. As in other organs, the lumen of these structures is in continuity with the tissue spaces, allowing a free flow of lymph and its content, fluid and cells, from the tissues to the drainage system. The lymphatic system is, thus, open‐ended—the endothelial cells of its finest branches having overlapping junctions that open passively on demand. There is, therefore, no need to postulate that cancer cells actively invade the surrounding tissues. In addition, the breast stroma contains endothelial cells in which the luminal side is not apposed to that of the same or another endothelial cell. These, like other endothelial cells, are attached to the connective tissue elements on one side only. It is suggested that they provide a series of potential spaces that can conduct fluid rapidly through the tissues, providing the epithelial cells with a copious supply of nutrients. Clinical, pathological and flow cytometric parameters have been analysed by univariate and multivariate analysis to define those parameters of important prognostic influence in 235 cases of surgically treated squamous carcinoma of the anus and perianal skin. Patients had been treated by anorectal excision (166 patients) or by local excision (69). Analyses were carried out on five data sets—the two surgical subgroups, two groups distinguished by site of tumour and on all 235 patients. Univariate analysis showed many parameters to be of prognostic influence, although histological typing of tumours into the more common histological subtypes was of no prognostic value. Parameters of independent prognostic significance in multivariate analysis were those indicating depth of spread, inguinal lymph node involvement and DNA‐ploidy. In this study the subdivision of the rarer types of anal canal tumour, such as mucoepidermoid carcinoma, microcystic squamous carcinoma and small cell anaplastic carcinoma, was relevant confirming that these tumours have a poor prognosis. It is now felt that surgery should not be employed as primary treatment in most cases of anal cancer and the results of this study have to be interpreted with caution when applied to patients treated with radiotherapy with or without chemotherapy. Up to 15% of abdominal aortic aneurysms are designated as inflammatory. They are characterized by marked fibrous thickening of the aneurysmal wall, with the fibrosis extending into the adjacent retroperitoneum. Thirty‐five abdominal aortic aneurysms were studied, 15 inflammatory and 20 atherosclerotic. Of the inflammatory group, 10 were symptomatic and five asymptomatic. For each resection specimen, 59 microscopic features (variables) were scored semi‐quantitatively. Discriminant function analysis showed that endarteritis obliterans, fibrosis around nerves or ganglia at the outer margin of mural fibrosis, and the thickness of the combined fibrotic media and adventitia gave a satisfactory high discrimination between atherosclerotic and inflammatory aneurysms. A study was made of the qualitative histological features of the small pulmonary arterial vessels of 25 adult citizens of La Paz, Bolivia (altitude 3600 m) coming to necropsy. Abnormalities found included muscularization of pulmonary arterioles, the development of longitudinal muscle in the intima of pulmonary arteries and arterioles, and the formation of muscular tubes lining the longitudinal muscle which extended through arterioles into the precapillaries of the lung. Arteriolar muscularization was found in three of the 13 Aymaras and in two of the 12 Mestizos studied. Intimal longitudinal muscle was present in four Aymaras and five Mestizos. Muscular tubes were found in only one case, a young Aymara. The features were very similar to those found in chronic obstructive airways disease. The appearances are consistent with a growth of new vascular smooth muscle in response to alveolar hypoxia as opposed to hypoxic vasoconstriction.

The production and detailed immunostaining properties of a new rat monoclonal antibody (ICR.2) to epithelial membrane antigen are reported. The antibody was selected for its ability to compete with the polyclonal antiserum (M7), used in the original immunohistological studies, in order that it might serve as a direct replacement in diagnosing epithelial tumours. Most of the staining reactions on normal tissues were identical to those previously reported with M7 but there were some important differences. They included: positivity of renal and adrenal capsular fibroblasts, perineurium, some myoepithelial and smooth muscle cells, occasional osteoblasts and squamous and thyroid follicular epithelum in the normal state. The intercellular canaliculi of sweat glands and secretory canaliculi of gastric oxyntic cells were clearly demonstrated. These staining reactions could be obtained with M7 when a sensitive detection system was used although the results were usually weak and inconsistent. Nearly all adeno‐, squamous and transitional carcinomas were positive. The remaining tumours fell into three major groups: (1) those which were consistently or nearly consistently negative–melanoma, seminoma, rhabdomyosarcoma, alveolar soft part sarcoma, adrenal cortical carcinoma, granulocytic sarcoma, paraganglioma, non‐Hodgkin's lymphoma, Hodgkin's disease and embryonal carcinoma; (2) those which were either negative or positive with distinctive patterns of staining–basal cell carcinoma, embryonal tumours: and (3) non‐epithelial tumours that were consistently positive–epithelioid sarcoma, synovial sarcoma, osteosarcoma, chordoma and myeloma–or positive in a significant minority of cases–leiomyosarcoma, malignant fibrous histiocytoma, clear cell sarcoma of tendon sheath, various neuroectodermal tumours. Thirty‐nine lesions, originally diagnosed as Paget's disease (19), superficial spreading melanoma (13) or Bowen's disease (7), were reviewed. Supplementary immunohistochemistry on routine processed tissue, using antibodies to cytokeratins (PKK1 and M717) and to S‐100 protein, was carried out. Two of the lesions, originally classified as Bowen's disease and superficial spreading type malignant melanoma respectively, were reclassified as Paget's disease based on the immunohisto‐chemical findings. Three cases of angiolymphoid hyperplasia with eosinophilia are reported at the site of tetanus toxoid vaccination. All presented as nodules on the upper arm, a known adverse effect of the vaccine following superficial inoculation. Histologically the nodules showed numerous small and medium sized vessels, some lined by hyperplastic endothelial cells with perivascular lymphocytic cuffing and an inflammatory infiltrate of plasma cells, eosinophils and scattered mast cells. Lymphoid follicles replete with germinal centres were identified in the dermis and subcutaneous tissue. Immunostaining revealed a polytypic lymphoplasmacytic infiltrate and a curious IgE reticulated pattern within the germinal centres, a feature described in both Kimura's disease and angiolymphoid hyperplasia. The present study suggests that atopic reaction to tetanus toxoid may be an aetiological factor in some cases of angiolymphoid hyperplasia.

An example of a cutaneous ciliated cyst in the buttock of a 42 year old woman is reported. The histological and immunohistochemical features of the cyst are presented; comparisons with fallopian tube are made and this rare lesion is discussed.

An ossifying malignant Leydig cell tumour of the testis in a patient with associated gynaecomastia is presented. The criteria for diagnosing malignancy in these tumours are briefly discussed. After left orchidectomy he received cyclical chemotherapy consisting of bleomycin, etoposide and cis‐platinum. At three months the gynaecomastia and pulmonary lesions have regressed and serum oestrogen has gradually fallen to 228 i.u./
