Haematoxylin and eosin, and Giemsa stained sections from 100 peripheral T‐cell lymphomas were examined blind on two occasions by four experienced haematopathologists in order to evaluate the inter‐ and intra‐observer reproducibility of the updated Kiel classification for these malignancies. In all cases, the T‐cell phenotype had been established previously in frozen section using a panel of monoclonal antibodies. Analysis by kappa statistics showed poor reproducibility, both overall and for most subtypes, with the exception of large cell anaplastic lymphoma. The distinction between low‐ and high‐grade lymphomas was also unsatisfactory. These results indicate the need for improved precision in the definition of histological categories of peripheral T‐cell lymphoma. Cervices from 42 hysterectomies performed from 1 to 91 months (mean 12.2) following conization were re‐examined in order to assess the possible effects of post‐traumatic regeneration on the endocervix. Twenty‐nine (69%) showed a continuum of abnormalities in which the shared finding was the presence of tubo‐endometrioid glands, accompanied in many cases by varying amounts of endometrial‐type stroma. Thus, 18 post‐conization cervices (43%) showed endometriosis, and a further 11 cases (26%) contained tubo‐endometrioid glands without demonstrable endometrial‐type stroma. These abnormalities were situated at the healed cone biopsy site, either superficially within the new transformation zone and/or within the cone biopsy scar. Post‐conization cervical endometriosis occurred from 2 to 91 months (mean 17.8) and tubo‐endometrioid metaplasia 2–24 months (mean 11.0) after the cone biopsy. It is concluded that cervical endometriosis and tubo‐endometrioid metaplasia are common complications of conization, and that they represent aberrant differentiation following injury. The demonstration of endometriosis and tuboendometrioid metaplasia in 69% of post‐conization cervices has implications for the interpretation of cervical biopsies and smears

Bellini duct carcinomas have recently been identified as a new entity in the spectrum of renal cell carcinomas and 10 cases have now been reported. The present paper adds detailed clinical and morphological data on six new cases. In addition, immunohistological and electronmicroscopical results support the origin of these tumours from the renal collecting ducts, especially the papillary ducts (Bellini ducts). We describe three cases of Wilms' tumour accompanied by central pericalyceal multicystic lesions closely resembling those seen in cystic nephroma. In continuity with these lesions or elsewhere in the kidney were areas of dydysplastic renal tissue of the type sometimes referred to as intralobar nephroblastomatosis. A close homology of tissue differentiation was observed between the dysplastic lesions and the interstitial components of the cystic lesion. These findings support the hypothesis that, in some cases at least, cystic nephroma has more in common with the group of dysplastic lesions referred to as the nephroblastomatosis complex than with the true renal tumours of childhood.

Congo red screening of 27052 routine biopsy specimens from 22827 patients over a 51/2‐year period in the Department of Pathology, University of Malaya detected 186 cases of amyloidosis. The categories of amyloidosis encountered and their prevalences in relation to each other were: systemic AL (5.9%); systemic AA (3.2%); isolated atrial (14%); primary localized cutaneous (7.5%); other primary localized deposits (3.2%); localized intratumour (58%); and dystrophic (8.6%). A third of patients with systemic AL amyloidosis had coexistent immunocyte abnormality. The commonest underlying pathology for systemic AA amyloidosis was leprosy. Notable among the types of localized amyloidosis revealed by this study were isolated atrial amyloidosis, which appeared to complicate chronic rheumatic heart disease, and intratumour amyloidosis complicating nasopharyngeal carcinoma. Other tumours in which amyloid deposits were observed included basal cell carcinoma, islet cell tumour and medullary carcinoma of the thyroid. Dystrophic amyloidosis was observed in fibrotic tissues, such as damaged cardiac valves and osteoarthritic joints. Thick malignant melanomas in general tend to have a poor prognosis, but exceptions occur where there may be no further recurrence. The reasons for this difference in clinical behaviour are not fully understood. We have looked at thick malignant melanomas (>3.0 mm) in the East of Scotland that have no evidence of metastasis after a minimum of 6 years follow‐up and compared the clinical and histological features with a similar group, associated with histological evidence of metastasis and/or death. S‐100 positive epidermal dendritic cells were counted in skin biopsies from 48 Kveim tests and four known foreign‐body reactions. Counts in histologically positive Kveim biopsies (mean 11.3 per 200 basal cells) were significantly higher than in either negative biopsies (5.1;P<0.001) or foreign‐body reactions (4.7;P<0.05). A similar difference was found, irrespective of the histological appearances, between biopsies from patients diagnosed clinically as having sarcoidosis (10.5) and those in which another diagnosis had been made (4.1;P<0.001). In biopsies from patients with sarcoidosis 70% had a positive Kveim test, 70% had a raised epidermal dendritic cell count and one or the other was positive in 90%. All cases in which both the Kveim test was positive and the dendritic cell count was raised had a final clinical diagnosis of sarcoidosis. In this study, 20 cases of eccrine spiradenoma have been examined using monoclonal antibodies to identify the nature of the epithelial, as well as the non‐epithelial, cellular components of this tumour. The results indicate that there is striking similarity between the epithelial cells of eccrine spiradenoma and the normal cells lining eccrine apparatus. The present paper describes the clinical, histological and immunohistochemical features of five cases of ‘organoid’ thymoma. The histological hallmark of this lesion is the prominent and diffuse ‘organoid’ pattern, defined by the presence of several areas of medullary differentiation. These areas, which are strictly reminiscent of the medullary area of the normal thymus, are scattered within a neoplastic tissue resembling the thymus cortex, the overall appearance mimicking that of normal thymus. All cases shared common clinical features: they were non‐invasive or minimally invasive tumours arising in young or middle‐aged female patients.
