Six chordomas (three classic and three chondroid) were examined ultrastructurally and with a panel of monoclonal antibodies. The three classic tumours showed the presence of desmosomes and intermediate filaments on electron microscopy, findings which gave a direct positive correlation when the tumours were stained with monoclonal antibodies against low molecular weight cytokeratin proteins. These results suggest that chordomas are essentially epithelial neoplasms and underline the fact that monoclonal antibodies to cytokeratins cannot be used in the differential diagnosis of classic chordomavscarcinoma. Furthermore, the epithelial characteristics are lost as the tumour undergoes chondroid differentiation.

Seventeen prostatic‐type polyps of the lower urinary tract are reported. All occurred in males, and the mean ages of the patients with ureteric orifice, bladder and urethral polyps were 20.5, 60.2 and 36.6 years, respectively. The commonest presentation for the bladder and ureteric orifice polyps was haematuria, whereas that for the urethral polyps was either dysuria or haematuria. The polyps consisted of acini and papillae lined by prostatic‐type epithelium, as confirmed by immunostaining for prostatic‐specific antigen. Most had interspersed islands or complete covering of transitional epithelium on the surface. We believe that the histogenesis of prostatic‐type polyps may differ in the different sites. For the ureteric orifice polyp, the early age of presentation and the simple occurrence of prostatic acini beneath an intact urothelium suggest a developmental abnormality. For the bladder polyp, the late onset of disease and the commonly observed transition with cystitis cystica‐glandularis suggest that it may be a metaplastic variant of cystitis cystica‐glandularis. Forty‐one cases of spindle cell lipoma, an entity first described in 1975, are presented. Of these, 38 were identified in a systematic review of 2478 tumours of adipose tissue diagnosed and treated in one hospital over a 25 year period. This lesion, therefore, accounts for about 1.5% of adipocytic neoplasms and is outnumbered by conventional benign lipomas by approximately 60 to 1. The remaining three were referred cases. Seventy‐five per cent of the lesions arose, as expected, on the back of the neck, upper back or shoulder, almost exclusively in males between 40 and 70 years of age. Ten lesions were identified at unusual sites: six in the limbs, three on the face and one on the trunk. One lesion arose within skeletal muscle, a previously unreported phenomenon, and this case showed both cartilaginous and osseous metaplasia. Only one of the 41 tumours has recurred locally. Occasional biopsies of genital lesions due to chancroid, secondary syphilis (the condyloma latum) and granuloma inguinale are encountered in routine histopathological practice. These lesions have specific microscopical appearances enabling a confident, or at least a presumptive, diagnosis to be made in most cases, yet the value of biopsy in the diagnosis of these diseases is not generally appreciated. The ulcer of chancroid shows three zones—a narrow superficial zone of degenerate leucocytes and fibrin, a broader middle zone with characteristic vascular changes, and a deep zone in which there is a plasma cell and lymphocyte infiltrate. The condyloma latum shows hyperplasia of the epidermis which is infiltrated with polymorphonuclear leucocytes in addition to the mononuclear infiltration of the dermis. Large numbers of spirochaetes are found in the area of the polymorph exocytosis of the epidermis. Severely atypical metaplastic and dysplastic changes were noted in the lining epithelium of collecting tubules and pelvis of the graft kidneys in two cases of cadaver renal transplants on immunosuppressive treatment with azathioprine and prednisolone. These changes were observed in case 1 in a needle biopsy and a nephrectomy specimen, 3 and 3.5 years after transplantation respectively. In a second case, the patient received two cadaver grafts, and both transplants showed similar changes 2 years after transplantation. The risk of malignant disease in kidney transplant recipients is now well recognized. The implications of the severe dysplastic changes noted in these two cases are discussed.

A series of 237 appendices was studied immunohistochemically for neurogenous hyperplasia. This was observed in 195 cases. It was possible to trace a continuum from appendices with intact lumens, featuring intramucosal neurogenous hyperplasia often with co‐existent submucosal and muscular nerve growth, to obliterated specimens whose axial portions were composed of varying proportions of nerve tangles and fibrous tissue. Predominantly fibrotic specimens were considered as end‐stages of this process. Stromal, argyrophilic cells lying amidst the nerve elements were prominent in the early, non‐obliterated cases; their number decreased in the obliterated nerve rich specimens and such cells became inapparent in the late fibrotic stage. The light microscopic, immunohistochemical and ultrastructural features of a clear cell carcinoma of the breast have been studied. Both intraductal and invasive components were found. Histochemistry showed large amounts of intracytoplasmic glycogen and sparse neutral mucin in the tumour. The tumour cells were stained by antisera to carcinoembryonic antigen, keratin and epithelial membrane antigen, but not by antisera to alpha‐lactalbumin, desmin or vimentin. A case of squamous cell carcinoma of the lung showing extensive spindle transformation is presented. On light microscopy, the tumour showed sheets and fascicles of elongated fusiform cells resulting in a growth pattern which closely resembled a sarcoma. Immunocytochemistry using tissue‐specific antibodies against intermediate filaments demonstrated exclusive labelling of the tumour cells with prekeratin antibodies. Electron microscopy showed well‐formed intercellular junctions and thick bundles of tonofilaments within the cytoplasm of the cells further confirming the squamous epithelial nature of the neoplasm.
