In this review we describe the application of markers which are useful for the diagnosis of soft tissue tumours in paraffin sections. Detection of intermediate filament proteins appears to be most useful for first screening of these neoplasms because all, except neuroblastomas, express vimentin; cytokeratin is expressed in synovial sarcomas, epithelioid sarcomas and mesotheliomas; desmin in myogenic tumours and glial fibrillary acidic protein in astrocytomas and gliomas. Tissue‐specific markers are: factor VIII—related antigen—endothelial cells; myoglobulin and skeletal muscle myosin—skeletal muscle cells; neuron specific enolase—neurons and cells of the APUD systems; and leukocyte‐associated antigen—leukocytes. Markers which are present in a variety of cell types and therefore do not serve as tissue‐specific markers are: S‐100 proteins, alpha‐1‐antichymotrypsin, creatine kinase M and actin. The S‐100 antigens have been detected in melanomas, granular cell tumours, chondrosarcomas and in some schwannomas and liposarcomas. Alpha‐1‐antichymotrypsin has been found in fibrohistiocytic and ‘true’ histiocytic tumours and creatine kinase M and actin in myogenic tumours. Thirty kidney tumours of various histological type were histochemically investigated under the light microscope by means of the ABC‐method. We used four biotinylated lectins which are known to bind also to normal renal tubular epithelial cells of different nephron segments. The nuclear grade, the histological growth pattern, the cell type and the histogenesis of the tumours were studied. The lectin ofLotus tetragonolobusbound to nine out of ten renal cell carcinomas with low nuclear grade, but in contrast no binding was seen in eight poorly differentiated ones. Four carcinomas mimicking collecting duct epithelium were also negative after treatment with this lectin, but showed strong staining after treatment with peanut agglutinin,Dolichos biflorusagglutinin and soyabean agglutinin. Five oncocytomas showed a high affinity only forDolichos biflorusagglutinin and only one case was positive withLotus tetragonolobusagglutinin; three tubulo‐papillary adenomas of the renal cortex without any oncocytes were negative with all of the lectins used. In a group of 335 patients with primary breast carcinoma the presence of immunoreactive carcinoembryonic antigen (CEA) and the binding of the lectin peanut agglutinin (PNA) in the primary carcinoma and in axillary lymph node metastases were investigated. The correlation between these results and a variety of established clinical, histopathologic, morphometric and biochemical prognosticators was studied. These features included lymph node status, tumour diameter, tumour type, nuclear grade, histologic grade, oestrogen receptor status, mitotic activity index and a number of nuclear measurements. The results indicate that CEA immunoreactivity of and PNA binding to tumour cells in primary breast carcinomas or lymph node metastases do not correlate with established prognostic factors in breast cancer.

Previous ultrastructural studies of pleomorphic adenoma have presented conflicting results with regard to the role of myoepithelial cells in the histogenesis of this tumour. In the present study specimens of ten major salivary gland pleomorphic adenomas were examined ultrastructurally and a number of cell types identified. The material was subjected to quantification using the stereological method of point counting. The results showed a wide spectrum of differentiation within these tumours in which typical myoepithelial cells were rarely encountered even in situations where they are reported to occur in routine histological preparations. Cells with some myoepithelial features were more numerous but duct cells accounted for the majority of tumour cells. The ultrastructural findings correlated well with previously reported immunocytochemical data and further support certain ideas about salivary gland tumour histogenesis.

Thirty cases of medullary thyroid carcinoma were investigated by immunoperoxidase staining techniques to evaluate the diagnostic significance of neuron‐specific enolase (NSE), carcinoembryonic antigen (CEA), somatostatin (SOM), a‐subunit of human chorionic gonadotrophin (a‐hCG), serotonin (5‐HT) and adrenocorticotropic hormone (ACTH) immunoreactivity as diagnostic markers in comparison to different calcitonin (CT) staining patterns. Twenty three cases exhibited a strong (group I) or moderate (group II) staining intensity for CT and did not need further immunocytochemical proof for classifying them as medullary carcinoma. From seven cases which showed only a weak or borderline CT‐immunoreactivity (group III), six stained positively for NSE and four positively for CEA. SOM‐positive cells were identified in six cases and a‐hCG or 5‐HT‐positive cells respectively in three cases of group III. Twenty follicular and 20 papillary carcinomas also included in this study did not react with any of the above mentioned antibodies. Therefore, NSE and CEA represent useful additional diagnostic markers particularly for the identification of medullary carcinoma with weak or borderline CT‐immunoreactivity. Five cases of Wilms' tumour with prominent papillary formation and focal carcinomalike epithelium are described. The morphology of these tumours is compared with a group of six renal cell carcinomas in childhood. A link between this type of nephroblastoma and renal carcinoma is suggested but pure renal carcinomas are considered to be a separate category. The biological behaviour of the papillary Wilms' tumours was found to be more aggressive than is generally believed. The course of the disease may be influenced by carcinoma‐like structures although this fact is not definitely established. A case of systemic lupus erythematosus with massive haemorrhage from the colon is described. Histological examination showed ulceration of the mucosa of the ascending colon with necrotizing vasculitis of the submucosal veins. Adjacent arteries were normal apart from one which had ruptured on the surface of an ulcer. This is the first clearly documented case of colonic venulitis in systemic lupus erythematosus and the differential diagnosis of gastrointestinal haemorrhage in this disease is discussed.

摘要:

A case of malignant lymphoma (immunoblastic with plasmacytic differentiation) of the colon complicating Crohn's disease is reported. The rarity of this complication in inflammatory granulomatous disease of the intestine is stressed. The possible relationship between intestinal lymphoma and Crohn's disease is discussed.
