We report the pathological changes in liver biopsies from io patients (four female transexuals and six impotent males) after treatment with 150 mg methyltestosterone daily for periods of up to 3 years, and in a hemihepatectomy specimen from a female transexual who developed a liver adenoma after 37 months of this treatment. Hepatocyte hyperplasia and mild focal sinusoidal dilatation was found in most cases. In some cases there were microcysts and dissociation of hepatocytes. In nine patients there was accumulation of hepatocytes between the endothelium and the supporting collagen of hepatic veins which often resulted in either partial occlusion of their lumina or disruption of their walls. These vascular changes do not appear to have been reported before. It is postulated that a single pathological process‐hyperplasia, perhaps related to the anabolic effect of methyltesterone‐could be partly responsible both for the formation of cysts through mechanical obstruction of hepatic veins and for the formation of nodules and tumo

At rare intervals in the literature, reports have appeared of a skin tumour containing structures resembling Pacinian corpuscles. Four such tumours collected over the course of 20 years are described and the literature reviewed.

In 63 resected thoracic aortic aneurysms, the commonest histological finding (45 cases, 71.4%) was cystic medionecrosis. These cases formed two groups, 29 with widespread fragmentation and loss of elastic tissue (elastopathy) and 16 cases without elastopathy who were older and included most of the 18 cases of dissecting aneurysms. Thirteen patients had the Marfan syndrome, Io showing cystic medionecrosis with elastopathy, indistinguishable form the cases with no Marfan stigmata although partial ‘dissections’ were mainly found in the Marfan patients, Histological appearances ranged from normal to complete loss of media. Cystic changes in muscle fibres apparently preceded elastic fragmentation. Fourteen cases (22%) had aortitis: 4 were syphilitic and 3 of other known aetiology. A modified trichrome stain is described for the intrahepatic localization of the hepatitis B surface antigen; HBsAg containing cells exhibit specific green metachromasia contrasting with the granular brown colour of non infected hepatocytes and with the deep eosinophilic colour of ground glass cells of HBsAg‐negative alcoholic or drug hepatitis. The technique is simple and reliable for routine screening of HBsAg positive material; its sensitivity is greater than H&E, similar orcein and inferior to immunohistochemistry as performed on frozen sections. Histological diagnosis can be made on the same slide, since several other morphological details are provided in the trichrome stained preparations. With this technique 387 biopsies from HBsAg seronegative individuals were negative; full cytoplasm metachromasia was mostly seen in asymptomatic HBsAg carriers, focal or partial staining in patients with histological evidence of liver cell necrosis. Two cases of primary carcinoma of the trachea are described. Both patients were female and had smoked cigarettes for many years. One of the tumours was a squamous cell carcinoma and the other an oat cell carcinoma, the commonest and the rarest forms of tracheal cancer respectively. Lobular carcinoma of the breast has been studied using histochemical methods for mucosubstances; immunocytochemical methods for casein and actin; the ruthenium red electroncytochemical method for acid glycoproteins and an immunoelectroncytochemical method for casein. Mucosubstances and casein showed a similar cytoplasmic localization, but casein production was much more intense and also showed a more diffuse cytoplasmic localization. Occasionally casein assumed the form of target‐like ‘inclusions’ as seen characteristically with the mucosubstances. The neoplastic cells were not stained by antisera against actin. Ultrastructurally, some cells showed an intracytoplasmic lumen with microvilli and/or an irregular outline at one extremity which was covered by microvilli. An electron‐dense ‘fuzz’ and casein coated the microvilli of cells exposed respectively to ruthenium red and an anticasein serum followed by peroxidase‐anti‐peroxidase complexes. It is concluded that lobular carcinoma shows evidence of epithelial rather than myoepithelial differentiation with the emphasis on epithelial secretory cells engaged in intensive milk protein production. All 10 tumours tested for oestrogen receptors were positive in contradistinction to ductal carcinoma with a lower incidence of positivity.
