The assessment of squamous lesions of the uterine cervix is a major preoccupation of diagnostic histopathologists and this can overshadow the recognition of significant abnormalities affecting the adjacent endocervical glands. This review gives an account of recent advances in our knowledge of the histology and histochemistry of endocervical glands. The review of benign lesions concentrates on those that continue to cause diagnostic difficulty. Adenocarcinomain situ, lesser degrees of possibly pre‐malignant cervical glandular atypia and histological subtypes of adenocarcinoma are outlined. An unselected series of 148 cases of non‐Hodgkin's lymphoma has been studied by immunohistological methods. In each case, the morphological features displayed in paraffin sections were correlated with the immunophenotype, determined using a panel of monoclonal antibodies. Of the lymphomas 82% were B‐cell type, 28% of follicle centre cell origin, 17% lymphocytic or immunocytic and 30% of large cell type. All the B‐cell tumours expressed pan‐B antigen, and nearly all HLA‐DR. In most, light chain monoclonality was demonstrable. Nearly all had moderate to large numbers of T‐cells of both subtypes interspersed amongst the B‐lymphocytes. Follicle centre cell tumours expressed surface IgM and IgD and had numerous dendritic reticulum cells. Lymphocytic and immunocytic lymphoma expressed IgM but less IgD, and had fewer or no dendritic reticulum cells. Large cell lymphoma expressed either no immunoglobulin or only IgM, and contained ragged dendritic reticulum cells, giving the appearance of ‘burnt‐out’follicles. T‐cell lymphomas usually showed a clear preponderance of either helper or suppressor subtype. Additionally, they contained residual B‐cells and sometimes germinal centres. Ten cases each of papillary, follicular, anaplastic and medullary carcinoma of the thyroid were stained for thyroglobulin, calcitonin, epithelial membrane antigen (EMA), carcinoembryonic antigen (CEA) and cytokeratin (CAM 5.2). Monoclonal or affinity purified polyclonal antibodies, and an indirect immunoperoxidase technique were used. All the papillary and follicular tumours, 5/10 anaplastic and 3/10 medullary carcinomas contained thyroglobulin. Only the 10 medullary carcinomas stained positively for calcitonin. Three out of 10 papillary, 1/10 follicular, 0/10 anaplastic and 10/10 medullary carcinomas were positive for CEA. Nine out of ten papillary, 7/10 follicular, 2/10 anaplastic and 3/10 medullary carcinomas were positive for EMA. Ten out of 10 papillary, 10/10 follicular, 5/10 anaplastic and 10/10 medullary carcinomas were positive for cytokeratin. The presence of calcitonin and CEA is of value in the diagnosis of medullary carcinoma, and enable its distinction from anaplastic thyroid carcinoma. Thyroglobulin is a useful marker in thyroid carcinomas.

A woman of 62 years with Turner's syndrome died in congestive cardiac failure secondary to a large ventricular septal defect with biventricular hypertrophy. During her last few months the lifelong left‐to‐right shunt underwent reversal exposing her carotid bodies to hypoxaemia. The carotid bodies were not enlarged, thus demonstrating that hyperplasia of glomic tissue is not brought about by increased myocardial massper se.They were, however, abnormally cellular with more dark cells (a variant of chief cells), many of which were abnormally large and showed ultrastructural features of metabolic activity. These changes may represent the earliest histological response of the carotid body to hypoxaemia and later, the dark cells may mature into the more familiar and common light variant. It seems likely that this dark cell activity precedes, probably by a long period, the sustentacular cell hyperplasia and proliferation of nerve axons which we have reported elsewhere as the chronic reaction of the carotid bodies to hypoxae

The location of two basement membrane components, laminin and the 7‐S domain of type IV collagen, was studied in human lymph nodes using the peroxidase‐antiperoxidase method. Basement membrane antigens were present on the walls of blood vessels and of marginal, trabecular and medullary sinuses. Thin, fragmented fibre‐like staining was present also in parenchyma outside the germinal centres, in a pattern overlapping with reticular fibres as seen on conventional reticulin stains. In order to define the histopathological criteria for the diagnosis of malignant neoplasm involving pleura, we studied 11 pleurectomy specimens containing undoubted malignant cells. The most important feature for a diagnosis of malignancy was the pattern of neoplastic infiltration of the fibrous sub‐pleural tissue. These criteria were subsequently applied in a re‐analysis of 76 pleural needle biopsies with a diagnosis of cancer. The patterns most frequently seen were a multi‐pronged growth of at least four clusters of atypical cells in the fibrous sub‐pleural tissue, a solid pattern with a large collection of atypical cells and a confluent one where large numbers of small clusters of atypical cells appeared almost fused in the stroma. Less frequently, malignant cells covered the surface of fibromuscular tissue, or were found in an intravascular or intramuscular location, but often associated with any of the three main patterns. Five needle biopsies were considered suspicious and 11, considered benign, contained a prominent mesothelial reaction. Five cases of solitary (localized) fibrous mesothelioma were studied immunohistochemically, utilizing monoclonal antibodies against high and low molecular weight cytokeratin, vimentin, carcinoembryonic antigen, epithelial membrane antigen and factor VIII related antigen. In contrast to normal mesothelium and contrary to previous reports from malignant mesotheliomas all the tumours were negative for keratin. They were strongly positive for vimentin. Two cases were studied electron microscopically; no ultrastructural evidence of mesothelial cell differentiation was identified. Our immunohistochemical and ultrastructural findings argue against a mesothelial cell origin, whereas the presence of vimentin suggests a mesenchymal histogenesis. A 52‐year‐old woman underwent thoracotomy for the removal of a mass in the middle lobe of the right lung. Light microscopy showed a tumour with the morphology of a papillary adenocarcinoma with numerous psammoma bodies. Electron microscopy revealed the tumour cells to possess the lamellated intracytoplasmic inclusions characteristic of normal and neoplastic type II pneumocytes.
