The contribution of electron and immunofluorescence microscopy to renal biopsy diagnosis is illustrated by the results obtained in a personal series of patients with various types of glomerulonephritis. Introductory notes on the ultrastructure of the glomerular capillary and on immunological processes are also included. Immunofluorescent staining has particular value in demonstrating IgG‐containing deposits in early membranous glomerulonephritis at a stage when ordinary microscopy is inconclusive. It is capable of throwing light on the mechanism of glomerular damage in severe extracapillary proliferation and in some cases of recurrent haematuria, but is less successful in separating minimal change disease from proliferative processes. Electron microscopy reveals the precise site of immune deposits and fibrin together with basement membrane changes, the microtubular structures common in SLE, and other details. An asymptomatic thymic tumour, classified as a carcinoid tumour on the basis of its light and electron‐microscopic appearances, is described. This tumour was atypical in having foci of melanin pigmentation, the pigment being contained in dendritic melanocytes and melanophages. Some antigenic determinants can be preserved in tissues after plastic embedding. In the present study liver tissue was fixed with glutaraldehyde or paraformaldehyde, dehydrated with ethanol and toluene and embedded in araldite which was then polymerized at 37°C. Immunofluorescence was performed on semi‐thin sections etched with hydrogen peroxide. This procedure allows correlation with light microscopy (on the same stained semi‐thin section) and with electron microscopy (on adjacent ultra‐thin section). Four hyperplastic lesions of the hard palate are described. They were characterized by the presence of an atrophic surface epithelium, an oedematous matrix and the presence of large numbers of autofluorescent granular cells. In addition there were numerous mast cells. Histochemical and electronmicroscopical studies suggest that the granules are composed of lipofuscin. The pathogenesis of these lesions is discussed.

The fiftieth case of idiopathic cholesterol pneumonitis (ICP) is presented and the 49 previously reported cases have been reviewed. This review draws attention to certain features which have not previously been emphasized, including marked male predominance, single lobe involvement, the distribution of the lesions, dense pleural fibrosis, smoking history and the close histological association between giant cells and disrupted elastic fibres of the alveolar walls. The clinical and radiological similarity between ICP and primary bronchogenic carcinoma is described. Various aetiology theories are discussed, and the similarity between ICP and spontaneous lipid pneumonia in the mongoose is noted; the hypothesis is put forward that ICP, like the mongoose pneumonia, is an expression of disordered production or disposal of excessive surfactant.
