Nephropathy remains a major cause of morbidity and mortality in diabetes mellitus and increasing numbers of patients require dialysis maintenance therapy. Diagnostic criteria of diabetic nephropathy are well established, but the relationship between the morphological changes of glomerular basement membrane thickening and increased mesangial volume and functional abnormalities, such as proteinuria and impaired renal function, are still not clearly understood. The intermediate filament profile of adrenal cortex and its related tumours has been evaluated. Most adrenocortical cells contained cytokeratin 8 and 18 as demonstrated by monoclonal antibodies CAM 5.2, M20, M9 and RGE53. Cytokeratin immunoreactivity was not confined to a functional zone of the adrenal cortex. Only a small number of the adrenocortical cells showed vimentin immunoreactivity. From normal adrenal cortex through adenomas, to carcinomas, there is a progressive decrease or even loss of cytokeratin immunoreactivity and an increase in vimentin immunoreactivity. Aberrant cytokeratin expression was not found in adrenocortical adenomas and carcinomas with the antibodies used. Awareness of the possible absence of cytokeratin immunoreactivity in adrenocortical carcinomas is important whenever antibodies to cytokeratins and vimentin are used for diagnostic purposes in poorly differentiated neoplasms.

Eighty cases of non‐Hodgkin's lymphomas were examined independently using the monoclonal antibodyKi‐67 and an argyrophilic method for the demonstration of nucleolar organizer regions. The evidence thatKi‐67 immunoreactivity may be used as a marker of cell proliferation is described and the nature of nucleolar organizer regions reviewed. The proportion of tumour cells with nuclearKi‐67 immunoreactivity and the mean number of nuclear organizer regions are shown to be linearly related (r=0.86,P<0.001) although some scatter was observed. These data suggest that the mean number of nucleolar organizer regions may reflect the cellular kinetics of a tumour. This study also provides further evidence supporting the thesis that the mean nucleolar organizer region score is related to the histological grade of non‐Hodgkin's lymphoma.Ki‐67 immunostaining and nucleolar organizer region staining would seem to provide comparable data, at least in non‐Hodgkin's lymphoma, but the latter method has the advantage of being applicable to conventionally fixed and processed para

Monocytoid B‐lymphocytes are a B‐cell subset present in subcapsular sinuses in some cases of lymphadenitis. We describe a case of lymphoma of this cell type. The tumour shows a distinctive morphology characterized by concentric strands of tumour cells around lymphoid follicles with hyperplastic germinal centres and conserved mantle zones. Electron microscopy of these cells shows short cellular processes as well as moderate development of endoplasmic reticulum. The phenotype of the tumour was monoclonal IgM‐kappa, distinct from other node‐based B‐cell subpopulations and suggesting a possible relationship to the lymphocytes of the marginal zone present peripheral to lymphoid follicles of the spleen. Two cases of olfactory neuroblastoma which presented clinically as intracranial lesions are described. Prominent features of epithelial differentiation were present, which led to initial diagnoses of poorly differentiated carcinoma. The true nature of the lesions was only established subsequently by careful histological examination, immunohistochemistry and electron microscopy. The potential towards epithelial differentiation in such tumours was emphasized and certain new histological features were described, including a biphasic epithelial and stromal pattern, papillae formation and positive staining for cytokeratin. Formalin‐fixed, paraffin‐embedded material from 15 haemangioendotheliosarcomas and eight malignant haemangiopericytomas was stained with an immunohistochemical technique for the presence of factor VIII‐related antigen (F VIII‐RAg), actin, laminin and for reactivity with the lectinUlex europaeusI (UEA‐I). Haemangioendotheliosarcomas stained with both F VIII‐RAg and UEA‐1; however, UEA‐I was found to be the more sensitive of the two, reacting also with the poorly differentiated tumours. Haemangiopericytomas reacted negatively with UEA‐I; surprisingly, some of these tumours exhibited a weak positivity in staining for F VIII‐RAg, possibly supporting the theory about intermediate forms between haemangioendotheliosarcomas and haemangiopericytomas. Laminin was found in most of the haemangioendotheliosarcomas and was useful in illustrating their vascular growth pattern. Two adult cases of a cutaneous lesion indistinguishable from typical infantile digital fibromatosis are added to the unique similar case so far reported in adulthood. The immunocytochemical localization of vimentin and muscle actin in the proliferating cells confirms their myofibroblastic nature and establishes closer relationships between the adult and the infantile variants of this entity. These two variants, however, appear to be clinically different, since all the adult cases were extradigital and did not recur after surgical excision. The term inclusion body fibromatosis underlines the histological hallmark of the lesion and should be used to identify this entity in place of recurrent infantile digital fibromatosis which does not seem any longer appropriate.

The pigmented neuroectodermal tumour of infancy is a rare neoplasm of uncertain histogenesis which, in the majority of cases, arises in the maxilla and pursues a benign course. Currently, it would be classified in the group of peripheral primitive neuroectodermal tumours. Histologically it is composed of two principal cell types: neuro‐blast‐like and melanocyte‐like. Three typical cases are presented herein, which appear to be the first examined with a panel of antibodies. The neuroblast‐like cells labelled positively for neurone‐specific enolase but were negative for S‐100, neurofilaments, glial fibrillary acidic protein, vimentin, cytokeratin, epithelial membrane antigen (EMA) and carcinoembryonic antigen (CEA). The melanocyte‐like cells stained positively for neurone‐specific enolase, vimentin and cytokeratin but were negative for S‐100, neurofilaments, glial fibrillary acidic protein, EMA and CEA. It has been suggested that cytokeratin CAM 5.2 is a useful marker to indicate malignant transformation and invasive potential in cervical neoplasia. In this study we examined normal ectocervical epithelium, endocervical squamous metaplasia, cervical intra‐epithelial neoplasia (CIN) and invasive carcinoma by the indirect immunoperoxidase method using commercially available CAM 5.2. Positive staining was seen in 12 of 42 (28%) invasive carcinomas and in 2 of 26 specimens of CIN III. No positive staining was observed in any case of CIN II (22 specimens), CIN I(19), squamous metaplasia (21) or normal ectocervical epithelium (16). These results suggest that although CAM 5.2 expression is found in only 28% of cervical squamous carcinoma, it is highly specific for malignant transformation of cervical squamous epithelium. In view of its potential diagnostic value in doubtful cases of CIN III and squamous cell carcinoma, the specificity and sensitivity of CAM 5.2 expression in cervical neoplasia need to be examined in other laboratories under various processing schedule

A malignant nerve sheath tumour arising within a thoracic ganglioneuroma is described. This is only the seventh such case described in the literature and the first at this site. The previously documented cases are reviewed.
