In this review the several types of cell damage and cell death which may be found in liver biopsy specimens are defined. We describe the different processes which occur at the portal/parenchymal or septal/parenchymal interface,viz.periportal spillover, periportal hepatitis, classic or lymphocytic piecemeal necrosis and biliary piecemeal necrosis. The diagnostic implications of these lesions in relation to the clinicopathological diagnosis and prognosis in various liver diseases are discussed.

In this study we report the histopathological features in a series of 88 biopsies from patients with cutaneous Kaposi's sarcoma; immunohistochemical examination for Factor VIII related antigen has been carried out on frozen and paraffin‐embedded sections. The patient groups comprised 50 elderly patients, seven of whom were immuno‐compromised, six patients with AIDS and one patient who had received a renal allograft. The findings were similar in these three groups. Histological staging was carried out with subdivision into stage I–patches characterized by angiomatoid and glomeruloid structures; stage II–plaques characterized by confluence of angiomatoid and glomeruloid structures and with spindle cells; and stage III–nodules in which the spindle cells were proliferative and frankly sarcomatous. Factor VIII related antigen was demonstrated in the central vessels of glomeruloid lesions whilst the surrounding vascular network contained both antigen‐negative and antigen‐positive vessels; in stage II and III lesions the spindle cells were consistently positive only in frozen section material. The findings were similar in the three patient groups. Our results suggest that Kaposi's sarcoma evolves from stage I through to stage III, that both blood vessels and lymphatic vessels are involved in the vascular proliferation and that the spindle cells are derived from vascula

Expression of therasoncogene product p21 (rasp21) in benign and malignant human colonic tissues was studied using the monoclonal antibody RAP‐5 and the avidin‐biotin‐peroxidase technique. Histologically normal colonic mucosa and hyperplastic mucosa adjacent to carcinomas (transitional mucosa) were found, in most cases, to be negative for reactivity with the antibody or showed weak staining of a few epithelial cells. Similar findings were observed in hyperplastic and juvenile polyps. Of the 145 adenomas studied, 47 (32.4%) showed detectable levels ofrasp21 expression. RAP‐5 immunohistochemical staining was significantly associated with the degree of epithelial dysplasia (P<0.01) and the size of adenoma (P<0.05), but not with the histological type. Fiftyfour of 70 primary adenocarcinomas (77.1%) were reactive with RAP‐5 and usually demonstrated a higher percentage of stained cells and more intense cytoplasmic staining than that observed in adenomas. Although metastases often displayed a similar or even higher levels ofrasp21 expression compared with the primary carcinomas, in 10 cases one or more metastatic lesions showed lower levels ofrasp21. Semi‐automatic image analysis was used to assess the architectural features of normal colorectal mucosa and ulcerative colitis with and without dysplasia. Eight measured and derived morphometric variables were compared with the histological grading. The main data‐set variation was due to: (1) the area of mucosa and epithelium per unit length of muscularis mucosae; (2) mean mucosal and epithelial height; and (3) the percentage epithelium and number of crypts per unit length of muscularis mucosae. Discriminant analysis using the variables mean epithelial height and mean lamina propria area per unit length of muscularis mucosae separated normals (n=10) from high‐grade dysplasia (8). The classification rule allocated low‐grade dysplasia (8) to the high‐grade category and 60% of regeneration cases (10) to the normal mucosa group. Scatter plots of the two discriminating variables separated normal and regenerative mucosa from dysplasia. Histological review of overlapping cases allowed redesignation of a high‐grade dysplasia lesion as low grade. Architectural morphometry may be of use in assessing premalignant mucosal changes in ulcerative colitis as a guide to patient surveillance

The clinical and histological findings in 15 cases of carcinoma arising from the extrahepatic biliary tract are reviewed. The clinical findings are in agreement with previously reported series and the dismal prognosis is confirmed. The prognosis does not appear to be related to tumour size, site, mitotic count or laboratory data on presentation. Only histological grading of the tumour is related to prognosis. Perineural tumour infiltration was a prominent feature and dysplasia within adjacent bile duct epithelium was present in five cases. The value of carcinoembryonic antigen (CEA) as a histological tumour marker was investigated. All tumours from the upper and middle biliary tree expressed this antigen. Although immunocytochemistry is seldom needed to assess malignancy in large surgical resection specimens anti‐CEA helps delineate the extent of tumour infiltration and discriminates between normal and dysplastic or malignant epithelium; anti‐CEA may also be helpful in identifying foci of malignancy in small sclerotic biopsy specim

Bone marrow trephine biopsy specimens from 24 patients with chronic granulocytic leukaemia were studied to identify bone marrow changes that suggest incipient or impending blast transformation. In the chronic phase of chronic granulocytic leukaemia, blasts and promyelocytes were seen predominantly in the paratrabecular and perivascular regions, while myelocytes, metamyelocytes and segmented polymorphs were seen predominantly in the central intertrabecular marrow space. The patients were divided into two equal groups according to the degree of perivascular and paratrabecular infiltration: those with minimal (one to three layers of blasts and promyelocytes) and those with marked (four to eight layers of blasts and promyelocytes) infiltration. Ten of the 12 patients in the former group did not develop blast transformation and remained in the chronic phase during a 32‐ to 84‐week follow up period, whereas nine of the 12 patients in the latter group developed blast transformation, with six undergoing transformation within the first 26 weeks. These findings suggest that the histomorphological pattern of bone marrow changes in chronic granulocytic leukaemia may be useful in recognizing a subset of chronic granulocytic leukaemia patients undergoing blast transformation in the ensuing 6 mon

A series of 256 bone marrow biopsy specimens was obtained at different times from a group of patients with acute myeloid leukaemia, chronic granulocytic leukaemia and acute lymphoblastic leukaemia, and was analysed in parallel with peripheral blood smears and bone marrow aspirate samples. In five of these biopsy specimens the bone marrow lesion was discrete, deeply seated within the marrow cavity and detected only in long‐core biopsies. Neither peripheral blood nor bone marrow aspirates obtained at the same time established the correct diagnosis. Most follicular lymphomas can be readily diagnosed on morphological grounds by finding closely packed pale‐staining follicles partitioned by scanty dark‐staining inter‐follicular tissue. The acquired immunodeficiency syndrome (AIDS) is most often a sexually transmitted disease acquired via male genital tract secretions. We performed a retrospective controlled study of testicular tissue taken from 100 AIDS patients at autopsy to determine the nature and spectrum of macroscopic and microscopic changes associated with AIDS in the testis. The AIDS patients showed a highly significant constellation of findings that included lymphocytic infiltration, perivasculitis, germ‐cell loss, interstitial fibrosis, decreased mean tubular diameter and increased mean tubular wall thickness. Only four cases showed evidence of testicular involvement with opportunistic infectious agents. In 48 cases gross pathological findings were present. Only four cases had histologically normal testes.
