Weight training is a popular component of physical fitness in North America. This form of training remains relatively safe with few cases of life-threatening injuries. However, a series of studies have demonstrated that repetitive upper- and lower extremity weight training incorporating a Valsalva maneuver can increase arterial pressure to values as high as 480/350 mm Hg. This marked increase in arterial pressure is transmitted to the cerebral vasculature and increases cerebral arterial transmural pressure and may have the potential to initiate the rupture of a previously innocuous intracranial aneurysm. We report three cases of subarachnoid hemorrhage (SAH) associated with arm (bicep) curls and leg press weight training and discuss the possible link between this form of exercise and aneurysmal SAH.

To highlight the insidious effect of long term front row play on the cervical spine.Two rugby union front row players presented with neck pain in the middle of the 1991–1992 season. Both had been playing rugby from an early age in the front row. Both had represented their country at school level. They also had both played senior rugby in the Leinster province of Ireland from the age of 19 years. One of the players presented with neck pain with associated radicular radiation. The other player presented with an insidious chronic neck pain. There was no history of acute trauma save the week-in/week-out collisions that occur in the front row of the rugby scrum. Radiological evaluation revealed chronic cervical vertebral and disk degeneration. One of the subjects had an acute disk prolapse. This was surgically removed. The second subject had evidence of chronic disk degeneration. Both players have now retired from rugby at the age of 31.Both injuries would appear to be caused by the high loading that front row play places on the cervical spine.A high index of suspicion is needed when assessing cervical spine injuries in front row rugby players. Seemingly, minor injuries may signpost more serious, and potentially catastrophic neck pathology.

摘要:
The occurrence of a sports-related adjustment disorder can inhibit optimum athletic performance and scholastic achievement and impair the overall functioning of athletes. Physicians and trainers caring for athletic teams must maintain an acute awareness of the athlete's mental and physical well-being to detect subtle signs of a mood or psychological disturbance. The case of the collegiate basketball player discussed here exemplifies the subtle presentation and stages of an adjustment disorder with the predominant manifestation of depression mixed with anxiety and disordered conduct. This player's lack of participation in practice and game play led to a distorted view of his self-worth, which slowly eroded his ability to play basketball, attend classes, and enjoy previously pleasurable activities.
