PurposeWe report a previously undescribed case of bilateral acute exertional compartment syndrome of the lateral compartment arising from pre-existing chronic exertional compartment syndromes.Case summaryA 21-year-old football player had a 1-year history of bilateral chronic exertional compartment syndrome. One day after practice his usual symptoms progressed instead of abated and he presented at 3:00 AM with bilateral acute lateral compartment syndromes. His lateral compartment pressures were > 100 mm Hg. Emergency surgical releases were performed and the patient recovered well with full return to competitive football in 4 weeks.DiscussionNo prior case of acute-on-chronic exertional compartment syndrome of the lateral compartment has been reported. In addition, this is a case of bilateral acute exertional compartment syndrome. The literature is reviewed with discussion of differential diagnosis, distribution, pathophysiology, and treatment of exertional compartment syndrome.RelevanceFailure to recognize and emergently treat progression of a chronic to an acute exertional compartment syndrome can be devastating.

PurposeTo report a very rare presentation of a stress fracture in a golfer: a stress fracture of the ulnar diaphysis.Case summaryA 44-year-old woman had a 4-week history of a sore left wrist. She had been playing golf daily and had had no sudden trauma. Extensor tendinitis was suspected. Symptomatic treatment was given. Radiographs showed a spiculated periosteal reaction of the distal ulnar diaphysis with no signs of fracture or bone destruction. The suggestion of a malignant lesion was raised. Magnetic resonance imaging showed a healing ulnar fracture and edema of the interosseous membrane.DiscussionIn golfers, stress fractures may be seen in ribs laterally and in tibias. We believe supination together with overuse of the hand flexor muscles caused the stress fracture presented, which has not previously been reported in golfers.RelevancePhysicians should be aware of the potential for ulnar stress fracture in golfers with wrist pain.

Radiographs and radionuclide bone scan supported a diagnosis of a stress fracture of the left midclavicle. Complete fracture healing followed a period of active rest.DiscussionA literature review of clavicle stress fractures is discussed. The open-hand diving technique used by this diver for water penetration may have caused repetitive stresses to radiate up the arm to the clavicle.RelevanceStress fractures should be considered in the differential diagnosis of clavicular pain in a diver.

PurposeA 28-year-old hypertensive professional football player suffered a lacunar stroke. Given the rarity of stroke at this young age, the contributory roles of hypertension and competitive athletics merit consideration.Case summaryA 28-year-old defensive lineman, with long-standing, albeit recently well-controlled hypertension, experienced slurred speech and veering to his right after a game. Computerized tomography revealed a small internal capsule infarct. Diagnostic investigation, including coagulation profiling, sickle screen, antiphospholipid antibody, carotid imaging, and echocardiography did not reveal a cause. The patient recovered within days with no residual impairment.DiscussionDespite the extreme blood pressure elevation associated with isometric straining in young athletes, stroke almost never occurs. Vascular changes resulting from chronic hypertension and/or marked volume depletion, and hemoconcentration resulting from restricted salt intake, diuretic use, and heavy sweating, offer the only explanations for this stroke.RelevanceThe case suggests physicians should exercise caution in using diuretics and salt-restricted diets to treat hypertensive athletes. Guidelines for treatment are discussed.
