ObjectiveTo report the results of surgery for tendinopathy of the main body of the patellar tendon.DesignRetrospective study.SettingA teaching hospital of the University of London.PatientsTwenty-eight patients reviewed at an average follow-up of 42 months from surgery for tendinopathy of the main body of the patellar tendon after failed conservative treatment.InterventionExploration of the affected patellar tendon, stripping of the paratenon, excision of pathological areas, and multiple longitudinal tenotomies.Main Outcome MeasuresPostoperative complications, ability to return to sport, and subjective satisfaction, as measured by formal clinical assessment or telephone questionnaire.ResultsAt follow-up, 23 patients were completely free of pain and had resumed full sporting activity at the same preoperative level. Three patients were improved enough to have returned to their preoperative sporting level or just below it. In two patients, the initial operation failed. In the patients who resumed sport, the average time from surgery to resuming full sporting activity was 7 months (range 6 weeks to 12 months). The most common early postoperative complications were wound hematoma and superficial infection. The most common late complications were related to the incision, with anterior knee pain on kneeling and skin dysesthesia.ConclusionSurgical decompression of the patellar tendon with multiple longitudinal tenotomies is an effective treatment for patellar tendinopathy. In the middle term, patients do not seem to relapse once they have recovered, whereas those who do not respond to surgery do not recover at all and may need a new operation.

ObjectiveTo determine the emotional and cognitive impact of injury and surgery on physical recovery in injured athletes.DesignA prospective longitudinal study comparing the psychosocial and physical recovery of competitive and recreational athletes.SettingTertiary-care sports medicine center.ParticipantsTwenty-seven athletes (15 men and 12 women) who required anterior cruciate ligament (ACL) reconstruction surgery.InterventionsA repeated-measures design used to compare the psychosocial and physical changes for 6 months after ACL surgery.Main Outcome MeasuresEmotional (mood) and cognitive (coping) functions and physical recovery (range of motion, physician-rated level of recovery, and physician permission to return to sport).ResultsThere was a significant time-effect difference in mood, with a greater mood disturbance and recovery rate for competitive athletes than recreational athletes. Differences in mood and pain coping were significant at 2 weeks and 2 months after surgery.ConclusionAthletes experience significant mood changes throughout rehabilitation, which may hinder rehabilitation early in the process. Longer-term rehabilitation was not impacted by mood or pain coping. Future studies might focus on examining the process over a longer time period (1–2 years after surgery). Physicians should be aware of these findings and appropriately counsel and motivate athletes toward more favorable positive psychological and physical outcomes.

ObjectiveThis study was conducted to identify the circumstances, timing, frequency, and types of injuries for a Junior A hockey team in an attempt to develop injury prevention strategies.DesignThis study was a prospective review of injuries sustained by a hockey team during one season.SettingPlayers were evaluated in a private practice clinical setting and during practices and games.ParticipantsTwenty-two hockey players (16–20 years old) were evaluated. An injury was defined as any event requiring the attention of a physician or trainer. The players sustained 83 injuries throughout the course of the season. Seventy-four were considered independent events.Main Outcome MeasuresThe effects of the following variables on injury rates were analyzed: position, game situation versus practice, and timing of injury during the game.ResultsPlayers were 20 times more likely to injure themselves during a game than practice. Although forwards sustained the highest total percentage of injuries, defensemen had the highest incidence of injury in games. More injuries occurred in the later periods and in the later minutes of each period. Injury incidence decreased for the second half of the season compared with the first half.ConclusionOur data suggest goaltenders are at the lowest risk of sustaining an injury, but forwards appear to be at a higher risk in practice situations and defensemen in games. Game situations place players at a much greater risk of injury than practice sessions, particularly in the later portions of each period and toward the end of the game. Given the propensity for injury in the early season and later in the game, endurance training before and during the season may be important in injury prevention.

ObjectiveThe purpose of this study was to identify all injuries to members of an elite women's rugby team and to compare these injuries with published data on injuries in other women's contact and collision sports.DesignThis was a prospective cohort observational study conducted using a monthly log completed by the team's certified athletic therapist to closely monitor attendance at practices and games along with the type and severity of injuries.SettingRugby games and practices held in Ontario, Quebec, and the Netherlands.ParticipantsForty members of the Ontario Women's Senior Provincial Rugby Team over the 1997 season and the 1998 World Championships.Main Outcome MeasuresAn injury was defined as a rugby-related event that kept a player out of practice or competition for > 24 hours or required the attention of a physician (e.g., suturing lacerations) and in addition included all dental, eye, and nerve injuries and concussions.ResultsThere were a total of 35 injuries in 4,958 player-hours and 2,926 athletic exposures. This resulted in a rugby injury rate of 7.1 ± 0.4 per 1,000 player-hours and 12.0 ± 2 per 1,000 athletic exposures.ConclusionThe incidence of injuries in women's rugby is comparable with that in other women's contact and collision sports, indicating that the sport may be safer than stated in the literature and media.

ObjectiveTo determine the test-retest reliability of eccentric hip flexor and adductor peak torque and endurance using a Cybex Norm isokinetic dynamometer and to evaluate muscle soreness after exercise as a result of the testing procedure.DesignProspective test-retest reliability study.ParticipantsA total of 19 male volunteers (21–43 years old).InterventionTests included hip flexor and adductor peak torque strength measured at 60°/s (concentric and eccentric) and endurance measurements (total work done over 20 continuous concentric/eccentric contractions at 150°/s). Subjects were tested on three occasions at 1-week intervals.Main Outcome MeasuresRepeated measures analysis of variance was performed for all dependent variables including the subject's dominant and nondominant side among the three test sessions. Intraclass correlation coefficients (ICCs) were calculated to compare the data between sessions. In addition, simple linear regression analysis was completed to determine the relationship between session measurements for all measures with ICC > 0.75.ResultsThe main findings of this study are that eccentric peak hip adductor torque was reliably measured on a Cybex Norm isokinetic dynamometer using a continuous eccentric/concentric cycle protocol at a speed of 60°/s (ICC > 0.84). Peak concentric adductor torque and peak eccentric and concentric hip flexor torque were less reliable using the same protocol (ICC < 0.67). Endurance measurements, using total work done over 20 repetitions, were not reliable for hip flexor and adductor muscle groups using a continuous eccentric/concentric cycle protocol at a speed of 150°/s (ICC < 0.55). Posttest muscle soreness was found to be of significant concern with such a protocol.ConclusionThe results from this test-retest reliability study confirm the reliability of eccentric peak hip adductor torque measurements for use clinically and in future research. In addition, the need to assess posttest muscle soreness after isokinetic muscle testing is confirmed.

ObjectiveTo determine whether nucleoside analogues can suppress recurrent outbreaks of herpes simplex virus type 1 (HSV-1) in wrestlers, commonly called herpes gladiatorum.DesignDouble-blind and open study using valacyclovir over two wrestling seasons from 1996 through 1998.SettingWrestling community in the Minneapolis-St. Paul area of Minnesota.ParticipantsVolunteer sample of 42 male wrestlers from 13 to 31 years of age. For inclusion, patients had to have recurrent herpes gladiatorum and be active in a wrestling program.InterventionParticipants were treated during the first half of the season in a double-blind study using valacyclovir 500 mg a day, and in the second half as an open study with all wrestlers using valacyclovir 1,000 mg.Main Outcome MeasuresOutbreak of herpes gladiatorum based on clinical appearance or culture.ResultsParticipants were divided into two groups based on time interval since primary outbreak of herpes gladiatorum: less than 2 years or more than 2 years. For patients in whom primary outbreak was less than 2 years ago, outbreaks occurred in 21% (3/14) of those receiving valacyclovir 500 mg per day and 8% (2/25) of those receiving valacyclovir 1,000 mg per day. For patients in whom primary outbreak was more than 2 years ago, outbreaks occurred in 0% (0/7) of those receiving valacyclovir 500 mg per day and 0% (0/12) of those receiving valacyclovir 1,000 mg per day.ConclusionFor patients with a history of herpes gladiatorum of more than 2 years, valacyclovir 500 mg daily suppresses recurrent outbreaks. Further studies need to be performed to determine proper dosing regimen for suppression of outbreaks in patients with a disease duration of less than 2 years.
