The standing balance of 11 functionally stable and 15 functionally unstable knees in patients with chronic anterior cruciate ligament (ACL)-deficient joints was studied by means of stabilometry. All the patients had participated in sports on a competitive level before injury, and functional stability was defined as full return, without giving way, to the same sport at the same level as before injury. Forty physically active volunteers were studied as the reference group. When compared with the reference group, an impaired standing balance was found in the functionally unstable patients but not in the functionally stable patients. The results indicate that stabilometry is a useful method for evaluating knee functional instability objectively in competitive athletes with chronic ACL-deficient knees.

The purposes of this study were to determine test-retest reliability of the one-leg hop test when using hop distances (cm) and the hop index (distance hopped on the involved leg divided by the distance hopped on the noninvolved leg x 100) as criterion measurements and to compare two-data analysis strategies (the mean of three hops vs. the longest hop). Twenty-two men and 16 women who had undergone anterior cruciate ligament (ACL) reconstruction (mean time between surgery and testing 22 ± 7 years) performed the one-leg hop test on two occasions within a 2 week period. Test-retest reliability, determined using intraclass correlation coefficients (ICCs) (Type 2,1), was acceptable (ICCs > 0.75). Although hop distances calculated using the longest hop were significantly greater than those determined using the mean of three hops (p < 0.01), hop indexes were not affected by data analysis strategy. The clinical significance of these findings is that when the one-leg hop test is used to assess patients with ACL reconstruction, reliable hop distances and hop index scores can be produced on one test occasion.

A number of techniques for the surgical correction of dislocating patellae have been devised over the past century. A great deal of controversy exists regarding the components of a successful technique to prevent further dislocation from occurring and halt degenerative changes of the patellofemoral joint. The Green quadricepsplasty, which attempts to realign the deranged extensor mechanism proximally with concomitant lateral release and vastus medialis advancement, has been used for many years at the Boston's Children's Hospital. Thirty-one operative knees in 26 patients suffering from recurrent patellar dislocation, refractory to conservative treatment, underwent this procedure over a 10-year period from January, 1961 to December, 1971, and were reviewed retrospectively with a mean follow-up time of 14 years. Sixty-four percent of patients were found to have good or excellent results. Presence of pain preoperatively, patient gender, and duration of symptoms predating surgery, were found to be the most important determinants of ultimate outcome.

Iliotibial band friction syndrome (ITBFS) is the commonest injury among South African long distance runners. Although conservative management is the cornerstone of treatment, a significant number of long distance runners develop chronic conditions requiring surgery. Cutting the posterior 2 cm of the iliotibial band at the level of the lateral epicondyle gave 93.7% good or excellent results. The operation is done under local anaesthesia as an outpatient procedure. There is a very low minor complication rate with no long-term morbidity.

We examined use of selected performance tests to describe function in individuals with deficient anterior cruciate ligaments (ACL). Ten subjects with ACL-deficient knees and 10 control subjects were tested in four different tasks: straight running, slalom circuit, figure-of-eight running, and hop test. The times achieved by the subjects in the slalom circuit and figure-of-eight running tasks were expressed as ratios of the time obtained in the straight running task. All subjects had clinical ACL ligament laxity confirmed by knee arthrometry and knee functional status recorded using the Lysholm knee scoring scale (LKSS). Differences between groups were analyzed by independent t tests. Pearson product moment correlations were used to examine relationships between the performance tests and the LKSS and the arthrometer data for the ACL-deficient group. Results show that the figure-of-eight ratio is the best test for distinguishing normal and ACL-deficient knees. The arthrometer data and the LKSS were significantly correlated with the hop index but not with the other performance tasks. The three performance tasks (straight running, figure-of-eight running, and hop test) are easy to perform and require minimal equipment, but require a high level of patient cooperation, and should be repeated three times, recording only the best score.

Wrestlers commonly use dietary restriction and voluntary dehydration to attempt to gain an advantage by competing at a weight just below the upper limit for their category. Such practices have been reported to induce low serum testosterone levels and elevated serum cortisol levels during the wrestling season. To determine how dietary restriction/dehydration might affect these hormones during the wrestling season, weekly determinations of body weight, serum testosterone, and serum cortisol levels were analyzed in wrestlers during training and competition periods. Mean serum testosterone levels were low (15.6 ± 1.5 nM) and serum cortisol levels were increased (349.5 ± 39.4 nM). Testosterone levels decreased further during periods of heavy training and competition (p< 0.01). Cortisol decreased with one competition and tended to increase during the season. Hormonal variations did not correlate with significant changes in body weight. The mechanisms of the transient decreases in serum testosterone levels and the progressive increases in serum cortisol levels are unclear; however these findings do raise concern over the possible long-term hormonal consequences of training.

Characteristics of exercise adoption and maintenance were studied in healthy, middle-aged, sedentary men. Thirty-nine men were randomized to jog 120 min/week for 4 months and 22 men were controls. At 4 months, there were 19 “good adherers” (men who had run more than 11 km/week on average). A higher physical activity level and lower body fat at baseline were the strongest correlates of subsequent 4-month exercise adherence. After 4 months, men were free to continue voluntarily with jogging. At 8 months, 10 of the 19 “good adherers” still had a high exercise level. Exercise at 8 months was neither related to baseline characteristics nor to training-induced 4-month changes in physical fitness or cardiovascular risk factors, but was only related to mood changes occurring during the first 4 months of jogging, i.e., increases in “vigor” and decreases in “lack of energy” (Spearman'sr= 0.563,p= 0.012 andr= −0.428,p= 0.049, respectively). We conclude that predictors of adoption and maintenance of physical activity were not the same in our unsupervised exercise setting with healthy, sedentary middle-aged volunteers. We suggest that exercise-induced short-term changes in mood should be monitored as a potential reinforcing factor of long-term exercise adherence.

The 1989 Jeux Canada Summer Games held in Saskatoon, over a 2 week period in August, hosted over 3,500 athletes and coaches who participated in 28 sports. Over 300 medical volunteers managed 8,015 medical encounters, of which 75% were classified as nonserious and 25% as serious. Females presented with higher injury rates than males and encountered more injuries during competition. Injury rates calculated with respect to number of athletes and hours of exposure were compared. Wrestling presented with the highest incidence of injury, with the majority of problems occurring during practice sessions. Rates, time, distribution, and nature of injuries were analyzed and the effect of sex on these factors was investigated. Medical organizers must consider epidemiological information to prepare for the potentially high utilization of services, especially for the management of musculoskeletal injuries. Many athletes used the health services at these games and were provided with acute care, education, and rehabilitation. Prevention is the key to sports medicine and major games provide an environment where this can be introduced.

This anatomic study was performed to document the presence and location of fascial attachments near the distal insertions of gracilis and semitendinosus tendons. Dissection of 10 adult cadaver legs was undertaken. Fascial attachments along the inferior borders of both the gracilis and semitendinosus tendons were consistently found. These fascial attachments were noted to be an average of 9.7 cm medial to the tibial tubercle for the gracilis tendon and 7.6 cm for the semitendinosus tendon. Knowledge of this anatomy will allow the surgeon to harvest these tendons safely for anterior cruciate ligament reconstruction. Failure to recognize and divide these structures may result in premature division of these tendons when passing an open tendon stripper through a limited surgical approach.
