Patient attitude toward and adherence to the medical regimen were examined in two groups of cardiac patients. One group was treated medically and the other was treated surgically. Attitude, as measured by the Miller Attitude Scale, was found to he a strong indicator of adherence to the medical regimen for both myocardial infarction (MI) and cardiac bypass patients. Attitudinal differences between Ml and cardiac bypass patients were found for two prescribed actions of the medical regimen—taking medications and stopping smoking. Significant attitude-adherence relationships indicate that for optimal compliance, health personnel could utilize data from the Miller Attitude Scale in planning an individualized rehabilitation program for the cardiac patient.

The information that can be obtained from the continuous acquisition of gas exchange during exercise testing is reviewed. The parameters discussed include maximal oxygen uptake (VO2 max), gas exchange anaerobic threshold, oxygen uptake kinetics, oxygen deficit, and the oxygen pulse. How these different parameters are derived is explained. The prediction of VO2 maxfrom maximal treadmill time and/or workload is also discussed, and factors shown to affect the accurate prediction of VO2 maxare presented. Characteristics of gas exchange such as the anaerobic threshold and oxygen uptake kinetics are reviewed as possible factors for discrepancies in predicting VO2 maxFinally, the implications of gas exchange during exercise in patients with coronary heart disease are discussed.

Six cardiac patients, three with fixed-rate pacemakers and three with heart rate-responsive coronary artery disease, underwent three months of typical cardiac training. Improvements in peak oxygen uptake (61.7% and 63.1%), submaximal heart rate-blood pressure products (-17.6% and −16.6%), total cholesterol/high-density lipoprotein cholesterol (-1.29 and −0.46), and anthropometric measurements (- 1.7% and −3.3% body fat) were remarkably similar for the fixed-rate and heart-rate responsive groups, respectively. This study demonstrates that a physiologic cardiovascular training response can be obtained in patients with fixed heart rates.

To determine the feasibility and potential value of outpatient exercise rehabilitation of cardiac transplantation patients, we evaluated the hemodynamic and perceptual responses to graded exercise of two patients before and after early exercise training. The patients, a woman aged 33 years and a man aged 24 years, underwent orthotopic cardiac transplantation for treatment of end-stage cardiomyopathy. A submaximal treadmill exercise test (Naughton protocol), using an end point of nine METs, was performed before (T1) and after (T2) an exercise program that began within one week of hospital discharge. One subject completed 18 exercise sessions in a 49-day period, and the other subject completed 15 sessions in 56 days. Exercise sessions included up to 30 minutes of a combination of treadmill walking and cycle ergometry at an intensity designed to elicit perceived exertion ratings (Borg scale) of 12 to 13. The exercise sessions were well tolerated by both patients. Resting tachycardia and a delayed heart-rate response to exercise, consistent with the denervated state of the transplanted myocardium, were evident during the exercise sessions and tests. At T2, systolic blood pressure and perceived exertion ratings were consistently lower for any given exercise intensity than at T1. Heart rates were similar during both exercise tests. Results indicate that early outpatient exercise programs for cardiac transplantation patients are feasible and that exercise training and the recovery process from surgery result in an improvement in exercise tolerance.

Renal function was studied in five male coronary heart disease (CHD) patients at rest, during 15 minutes of treadmill running, and during a marathon (42.2 km) road race that was completed in the mean time of 299 minutes. Approximately 70% of the maximal oxygen uptake (VO2 max) was utilized in both runs. In addition, urinary excretion rates of the CHD patients were compared to that of five competitive runners in the marathon race. Urine production decreased from the resting control of 1.0 to 0.7 ml/min, osmolar clearance decreased from 2.3 to 1.5 ml/min, and free water clearance increased from −1.4 to −0.9 ml/min during the 15-minute run, whereas creatinine clearance did not change significantly. Creatinine clearance decreased, urine production and osmolar clearance were further reduced to 0.3 ml/min and 0.5 ml/min respectively, and free water clearance increased further to −0.2 ml/min during the marathon run. Sodium and calcium clearance rates decreased during the 15-minute run but were reduced more in the marathon run. The CHD patients had higher sodium excretion rates during marathon running than did the competitive runners. Transient hematuria and proteinuria were seen in both the CHD and competitive runner groups during the marathon race, even though renal function remained normal. Our findings indicate that these trained CHD patients made appropriate renal adjustments to maintain fluid and electrolyte balance during prolonged strenuous exercise.
