The purpose of the present study was to determine the usefulness of symptom-limited graded exercise testing in evaluating percutaneous transluminal coronary angioplasty (PTCA). Angioplasty was considered successful when the luminal stenosis was improved by ≥40% with a ≥30 mm Hg decrease in the pressure gradient across the stenosis. Fourteen successful and four unsuccessful PTCA patients who underwent pre- and post-PTCA exercise testing were considered for analysis; these patients received the same drugs and dosages during the entire study period. In the successful group, nine of 14 (65%) had improvement in angina pectoris and/or ST-segment depression and one of 14 (7%) did not improve. Four of 14 patients (29%) had neither angina pectoris nor ST-segment changes in either test. In the unsuccessful group, one of four (25%) had reduced ST-segment depression. Maximum heart rate was significantly increased only in the successful group (P< .03), whereas functional capacity was slightly improved in both groups (P= NS). In conclusion, graded exercise testing is a useful marker for the analysis of improved ischemic status after successful PTCA and is beneficial in the follow-up of PTCA patients.

In this review, the results of exercise testing and progressive ambulation after coronary artery bypass graft surgery (CABGS) are discussed. The effects of exercise training in cardiac rehabilitation programs provide an interesting comparison with the changes observed after CABGS, Coronary artery bypass graft surgery reduces ischemic symptoms by directly improving myocardial perfusion, whereas exercise training improves functional status predominantly by altering peripheral adaptations, thereby reducing the work of the heart and its oxygen consumption. Despite the apparent beneficial complementary actions of CABGS and exercise training, few studies of CABGS patients in exercise programs have been performed. Although clinical improvement is usually noted, psychologic and medical problems can occur after cardiac surgery, and the rate of returning to work is often lower than expected. The available studies have demonstrated that trained CABGS patients achieve equal or greater increases in maximal oxygen uptake than do post-myocardial infarction trained patients, further improve their functional capacity after its initial improvement by surgery, and train safely.

The multivariate relationships between scores from the Jenkins Activity Survey (JAS) and various exercise parameters (i.e., intensity, attendance, and MET gain) were studied in a sample of 57 patients attending the Wake Forest University Cardiac Rehabilitation Program. Canonical correlation produced one significant variate pair (canonicalR= .54,P< .05). Interpretation of this analysis revealed that the JAS subscales were directly related to a dimension represented by intensity and MET gain, respectively. It appears that a primary cause of the improved MET capacity is a function of patient compliance to prescribed exercise heart rate range. The consequences of the JAS-defined type A behavior within rehabilitation settings are evaluated, and how these data relate to previous research on compliance is discussed.

The results of exercise electrocardiograms (ECGs) must be interpreted with the understanding that not every positive test is indicative of coronary heart disease (CHD). Mitral valve prolapse (MVP) mimics CHD clinically and does so often in the resting and exercise ECG. In the present study, 55 patients with MVP underwent exercise testing. Twenty-two (40%) had a positive response and underwent a repeat test after administration of beta blockers. Of these 22 patients, 19 patients' (86.4%) ECG tracings returned to normal. The results of the present study suggest that the ECG changes seen in MVP are not coronary in origin. Exercise testing after beta blockade should be performed routinely in patients with MVP and a positive exercise ECG. The elimination of false-positive tests by beta blockade should enhance the predictive value of exercise testing in the investigation of CHD. Furthermore, similar responses should lead to the suspicion of MVP.
