Physical Exercise and Anxiety Management Training for Cardiac Stress Management in a Nonpatient Population |

Previous research demonstrated the efficacy of both physical exercise and anxiety management training in reducing psychologic and physiologic measures that predispose individuals to coronary heart disease. Our study compared these two treatment approaches in the reduction of anxiety, heart rate, blood pressure, and the type A behavior pattern. Eighteen subjects who experienced mild daily stress were randomly assigned to either (1) a seven-week aerobic physical exercise (PE) program as commonly used in cardiac rehabilitation, (2) a seven-week anxiety management training (AMT) program, or (3) a no-treatment control group. Subjects in both the PE and AMT programs, but not in the control group, had significant reductions in self-report measures of state anxiety and systolic blood pressure. Subjects in the AMT group significantly decreased their scores on a self-report measure of trait anxiety. Only PE subjects showed significant decreases on the Jenkins Activity Survey scale for type A coronary-prone behavior. The PE subjects also showed significant decreases in heart rate with exercise and significant increases in serum high-density lipoprotein cholesterol, both of which are correlates of cardiac fitness. The results support the inclusion of both aerobic PE and AMT in stress prevention programs and suggest that aerobic conditioning may be an effective intervention for the type A behavior pattern.

The cardiorespiratory and performance responses to incremental-load treadmill (T) work and arm (A), leg (L), and combined arm and leg (A + L) work on the Schwinn Air-Dyne ergometer were investigated in 15 men and 15 women. In both the men and women, maximal exercise heart rate (HRmax) and maximal oxygen uptake (&OV0312;O2max, L/min) were similar (P < .05) among L, A + L, and T but significantly greater for A + L and T work compared to A work. Maximal performance time increased progressively and significantly from A to L to A + L work. In the men, &OV0312;O2maxproduced during A, L, and A + L work represented 85%, 95%, and 98% of T work, respectively; in the women, &OV0312;O2maxproduced during A, L, and A + L work represented 76%, 87%, and 94% of T work, respectively. For both the men and women, postexercise lactic acid was significantly greater for A work compared to L work but comparable between L, A + L, and T work. During the submaximal phases of the ergometer tests, heart rate and &OV0312;O2for A work was significantly higher and mechanical efficiency significantly lower compared to L and A + L work.

The effects of a rehabilitation program on the psychologic functioning of patients who experienced a myocardial infarction were studied. Patients were randomly allocated to either the Rotterdam outpatient cardiac rehabilitation program or to physician-encouraged home rehabilitation. Both groups (32 patients each) were tested three times over a period of six months with psychologic questionnaires that measured anxiety, stress, personal and social inadequacy, rigidity, dominance, self-esteem, and feelings of being disabled. The data show that the outpatient rehabilitation program leads to diminished sense of social inadequacy in the patient compared with home rehabilitation. Furthermore, feelings of anxiety and social inadequacy decreased in the patients in the outpatient rehabilitation group and feelings of self-esteem were enhanced. Three other important results were found: (1) the outpatient rehabilitation program had no demonstrable influence on work resumption; (2) it had a favorable effect on smoking habits since half of the smokers stopped smoking; and (3) participation in a cardiac rehabilitation program promoted habitual exercise. It is concluded that group cardiac rehabilitation on an outpatient basis is superior to physician-encouraged home rehabilitation.

Canonical correlation analysis was performed on 23 clinical coronary risk factors and on 29 psychologic characteristics measured in 4,351 patients from a preventive medicine clinic. Two canonical correlations accounting for 37% of the variance in the variables studied were identified. Clinical variables that loaded heavily on these correlations were tenseness, physical fitness (treadmill time), and no exercise program. Psychologic variables included somatic symptoms and tension. In this cross-sectional study, individuals who had high physical fitness and who exercised tended to have a low level of somatic complaints and tension.
